Efficacy of Sofosbuvir Plus Ribavirin With or Without Peginterferon-Alfa in Patients With Hepatitis C Virus Genotype 3 Infection and Treatment-Experienced Patients With Cirrhosis and Hepatitis C Virus Genotype 2 Infection

BACKGROUND & AIMS: We conducted an open-label, randomized, phase 3 trial to determine the efficacy and safety of sofosbuvir and ribavirin, with and without peginterferon-alfa, in treatment-experienced patients with cirrhosis and hepatitis C virus (HCV) genotype 2 infection and treatment-naive or treatment-experienced patients with HCV genotype 3 infection. METHODS: The study was conducted at 80 sites in Europe, North America, Australia, and New Zealand Patients were randomly assigned (1:1:1) to groups given sofosbuvir and ribavirin for 16 weeks (n = 196); sofosbuvir and ribavirin for 24 weeks (n = 199); or sofosbuvir, peginterferon-alfa, and ribavirin for 12 weeks (n = 197). The primary end point was the percentage of patients with HCV RNA <15 IU/mL 12 weeks after stopping therapy (sustained virologic response [SVR12]). From October 2013 until April 2014, we enrolled and treated 592 patients-48 with genotype 2 HCV and compensated cirrhosis who had not achieved SVR with previous treatments and 544 with genotype 3 HCV (279 treatment-naive and 265 previously treated). Overall, 219 patients (37%) had compensated cirrhosis. The last post-treatment week 12 patient visit was in January 2015. RESULTS: Rates of SVR12 among patients with genotype 2 HCV were 87% and 100%, for those receiving 16 and 24 weeks of sofosbuvir and ribavirin, respectively, and 94% for those receiving sofosbuvir, peginterferon, and ribavirin for 12 weeks. Rates of SVR12 among patients with genotype 3 HCV were 71% and 84% in those receiving 16 and 24 weeks of sofosbuvir and ribavirin, respectively, and 93% in those receiving sofosbuvir, peginterferon, and ribavirin. On-treatment virologic failure occurred in 3 patients with HCV genotype 3a receiving sofosbuvir and ribavirin for 24 weeks. The most common adverse events were fatigue, headache, insomnia, and nausea. Overall, 1% of patients discontinued treatment due to adverse events. CONCLUSIONS: Among patients with genotype 3 HCV infection, including a large proportion of treatment-experienced patients with cirrhosis, the combination of sofosbuvir, peginterferon, and ribavirin for 12 weeks produces high rates of SVR. Treatment-experienced patients with cirrhosis and genotype 2 HCV infection had high rates of SVR in all groups.

Longitudinal assessment of the health-related quality of life of children and adolescents with chronic kidney disease

Summary: This multi-center longitudinal cohort study aimed to assess the trajectories of health-related quality of life (HRQoL) in children with chronic kidney disease (CKD) over time. The results showed that the HRQoL scores differed between children with different CKD stages, and the transition from dialysis to transplantation was significantly associated with the improvement in HRQoL. Children with CKD stage 1-5 and transplant recipients at baseline had stable HRQoL over time.

Summary: In this immunological study, cohorts of healthy younger and older adults were immunized with an adenovirus-vectored RSV vaccine either intramuscularly or intranasally. Differential gene expression was observed in blood samples after intramuscular prime vaccination, but not after intranasal prime vaccination. Intranasally primed participants showed more gene expression changes upon boosting. The gene expression changes mainly involved immune-related biological processes.

Delivery of biannual ultrasound surveillance for individuals with cirrhosis and cured hepatitis C in the UK

Summary: This study found that the uptake of biannual ultrasound surveillance in cured hepatitis C cirrhosis patients in the UK is low and there is inequity in its delivery. Only 19% of patients received biannual ultrasound in the first three years after achieving a sustained viral response, and this proportion decreased further to 9% during the follow-up years. The uptake of biannual ultrasound was associated with the hospital attended by the patient, as well as age and cirrhosis decompensation.

Summary: The CC genotype of the IFNL4 gene is associated with increased Hepatitis C cure rates. The Genedrive (R) IFNL4 is a CE marked Point of Care molecular diagnostic test for rapid detection of IFNL4 genotype status. Comparing buccal swab PoC testing with genetic sequencing, the buccal swab test showed better accuracy in identifying non-CC genotypes than CC genotypes.

The QuantiFERON SARS-CoV-2 assay showed potential as a T-cell evaluation tool soon after SARS-CoV-2 infection but has lower sensitivity for use in reliable evaluation of vaccination or more distant infection.

Summary: Serological analysis and infection outcomes of 2,686 participants with immune-suppressive diseases who received two COVID-19 vaccines revealed specific clinical phenotypes that might benefit from targeted COVID-19 therapeutic strategies. Some patients failed to develop anti-SARS-CoV-2 antibodies, with the highest vaccine failure rates observed in ANCA-associated vasculitis, hemodialysis, and solid organ transplant recipients. Decreased serological and T cell responses were associated with severe COVID-19. In summary, we identified clinical phenotypes that may benefit from targeted COVID-19 therapeutic strategies.

Summary: This study evaluated the psychometric properties of the PedsQL GCS and CHU9D in children and adolescents with common chronic health problems. Both instruments showed strong internal consistency, but weak convergent validity. Known group validity was demonstrated for PedsQL GCS, while CHU9D was only able to discriminate between certain health conditions.
