Delivery Of Cascade Screening For Hereditary Conditions: A Scoping Review Of The Literature

Cascade screening is the process of contacting relatives of people who have been diagnosed with certain hereditary conditions. Its purpose is to identify, inform, and manage those who are also at risk. We conducted a scoping review to obtain a broad overview of cascade screening interventions, facilitators and barriers to their use, relevant policy considerations, and future research needs. We searched for relevant peer-reviewed literature in the period 1990-2017 and reviewed 122 studies. Finally, we described 45 statutes and regulations related to the use and release of genetic information across the fifty states. We sought standardized best practices for optimizing cascade screening across various geographic and policy contexts, but we found none. Studies in which trained providers contacted relatives directly, rather than through probands (index patients), showed greater cascade screening uptake; however, policies in some states might limit this approach. Major barriers to cascade screening delivery include suboptimal communication between the proband and family and geographic barriers to obtaining genetic services. Few US studies examined interventions for cascade screening or used rigorous study designs such as randomized controlled trials. Moving forward, there remains an urgent need to conduct rigorous intervention studies on cascade screening in diverse US populations, while accounting for state policy considerations.

Point/Counterpoint: Is It Time for Universal Germline Genetic Testing for All GI Cancers?

Summary: The use of germline genetic testing is increasing among cancer patients due to the availability of multigene panel tests, decreased costs, and improved insurance coverage. The US Food and Drug Administration's approval of genotype-directed therapies also contributes to this trend. Additionally, the National Comprehensive Cancer Network has endorsed germline genetic testing for certain cancer types. This article debates the advantages and disadvantages of offering germline multigene panel testing to all patients diagnosed with any GI cancer.

Biological determinants of blood-based cytokines in the Alzheimer's disease clinical continuum

Summary: Translational and clinical research has shown that neuroinflammation plays a critical role in the pathophysiology of Alzheimer's disease (AD). Blood-based biomarkers, such as cytokines and interleukins, have been studied as indicators of neuroinflammation in AD. However, inconsistent results have been reported due to lack of standardization. This study used machine-learning and statistical analysis to explore the impact of age, sex, and APOE genotype on cytokines. The results showed sex differences in IL12 and an APOE-related difference in IL10, with IL1 beta being significantly associated with MCI-to-dementia conversion. Further clinical research is needed to understand the role of plasma cytokines in neuroimmune and inflammatory dynamics in AD.

Despite the low risk of complications, there may be reluctance among physicians and patients to recommend or undergo this procedure. Patient education, specialist training, and new methods for patient safety are important factors in supporting the widespread use of lumbar puncture in Alzheimer's disease.

The foundation and architecture of precision medicine in neurology and psychiatry

This article proposes a PM architecture for central nervous system diseases, based on multimodal biomarkers, systems medicine, digital health technologies, and data science. Alzheimer's disease is discussed as an example, showcasing its potential for PM-oriented neuroscience drug development and advanced healthcare practice.

Summary: This review explores strategies for tailoring genetic counseling to meet the needs of diverse racial and ethnic populations. The three themes that emerged were community involvement, creation and use of culturally tailored resources, and modifications to the genetic counseling process. However, the effectiveness of these strategies could not be evaluated due to a lack of consistent outcome measures in the articles.

Promoting the integration of genetic counseling education and research across the spectrum of learners at a large academic institution

Summary: Genetic counselors play an important role in research teams and are seeking additional opportunities for research training. Therefore, a grant was obtained to develop an undergraduate minor in genomics and human health, promote a research program for genetic counselors, and initiate a combined PhD program in Molecular, Cellular, Developmental Biology, and Genetics with a track in genetic counseling. Early progress has been made in each component and the aim is to replicate this program at other sites.
