Antimicrobial resistance and treatment: an unmet clinical safety need

Introduction: Infections due to multidrug-resistant (MDR) bacteria are burdened by high mortality rates. The development of new compounds to face the global threat of resistance is urgently needed. Combination regimens including old high-dose antimicrobials are currently limited by the risk of toxicity, resistance selection, and reduced efficacy. Following the Infectious Diseases Society of America call to develop 10 new antibacterials by 2020, new molecules are currently under development or have become available for use in clinical practice. Areas covered: We have reviewed safety characteristics and tolerability of old antimicrobials that are currently employed in combination regimens as well as new antimicrobials, including beta-lactams/beta-lactamase inhibitors, new cephalosporins, quinolones, and aminoglycosides. Expert opinion: The availability of new compounds that show in vitro efficacy against MDR represents a unique opportunity to face the threat of resistance and to optimize the current use of antimicrobials, potentially reducing toxicity. Agents that are potentially active against MDR Gram-negatives are ceftozolane/tazobactam, new carbapenems and cephalosporins, the combination of avibactam with ceftazidime, and plazomicin. Further data from clinical trials and post-marketing studies for drugs targeting MDR pathogens are crucial to confirm their efficacy and safety.

Summary: Fifty thousand hybrid circuits of five different types will be manufactured for the Phase-2 Upgrade of the CMS Outer Tracker. These circuits must undergo functional testing and visual inspection before being assembled into modules. Software tools are developed to carry out these tasks and store the results in the CMS production database.

Summary: This study analyzed the incidence of bloodstream infections (BSI) due to Candida in COVID-19 patients supported with ECMO, and found that the incidence of candidemia was significantly higher in the ECMO group compared to the control group. This result was confirmed by survival analysis and multivariable analyses.

Summary: This study conducted a systematic review and meta-analysis and found that patients colonised with extended-spectrum cephalosporin-resistant Enterobacterales (ESCR-E) before surgery are more likely to develop postoperative infections. Preoperative screening for ESCR-E in adult inpatients may help reduce the incidence of surgical site infections.

Summary: ITER plans to use two heating neutral beam injectors plus an optional one as part of its auxiliary heating and current drive system. The expected power per injector is higher than existing facilities worldwide. To enable this development, a Neutral Beam Test Facility (NBTF) has been established, consisting of two test beds called SPIDER and MITICA. SPIDER focuses on developing and characterizing efficient negative ion sources, while MITICA is a full-scale prototype of the ITER Neutral Beam Injector. The lessons learned from the NBTF implementation and future perspectives are discussed.

Extensional Earthquakes in the Absence of Magma in Northern Afar: Insights From InSAR

Summary: A series of earthquakes occurred in the northern Afar region, which were found to be caused by tectonic faulting without involvement of magma movements. Measurement models show that the extension from faulting corresponds to approximately 30 years of plate spreading. The observations indicate significant variation in the proportion of extension from faulting along the rift.

The Role of Gut Microbiota in the Clinical Outcome of Septic Patients: State of the Art and Future Perspectives

Summary: Sepsis, a life-threatening condition caused by an abnormal host response to infection, results in multiple organ dysfunction and has a high global mortality rate, with 11 million deaths per year in high-income countries. Studies have shown that septic patients often have dysbiotic gut microbiota, which is associated with increased mortality. This narrative review examines original articles, clinical trials, and pilot studies to evaluate the potential benefits of manipulating the gut microbiota in clinical practice, starting with early sepsis diagnosis and in-depth analysis of the gut microbiota.

Chronic Suppressive Antibiotic Treatment for Staphylococcal Bone and Joint Implant-Related Infections

Summary: Prosthetic joint infection (PJI) and fracture-related infection (FRI) are challenging conditions in high-risk patients. For those not eligible for standard treatment, combination therapy of debridement, long-term antibiotics, and chronic oral antimicrobial suppression (COAS) can be a reasonable option. This study retrospectively analyzed 16 patients who underwent COAS after debridement and antibiotic treatment, with a median follow-up of 15 months. The monitoring of patients included clinical evaluation, laboratory tests, and radiolabeled leukocyte scintigraphy (LS). The results showed that 62.5% of patients had no relapse after cure at the last evaluation, while side effects led to COAS discontinuation in 50% of the patients who experienced a relapse.
