Outcomes of Robot-assisted Partial Nephrectomy for Clinical T2 Renal Tumors: A Multicenter Analysis (ROSULA Collaborative Group)

Background: While partial nephrectomy (PN) represents the standard surgical management for cT1 renal masses, its role for cT2 tumors is controversial. Robot-assisted PN (RAPN) is being increasingly implemented worldwide. Objective: To analyze perioperative, functional, and oncological outcomes of RAPN for cT2 tumors. Design, setting, and participants: Retrospective analysis of a large multicenter, multinational dataset of patients with nonmetastatic cT2 masses treated with robotic surgery (ROSULA: RObotic SUrgery for LArge renal mass). Intervention: Robotic-assisted PN. Outcome measurements and statistical analysis: Patients' demographics, lesion characteristics, perioperative variables, renal functional data, pathology, and oncological data were analyzed. Univariable and multivariable regression analyses assessed the relationships with the risk of intra-/postoperative complications, recurrence, and survival. Results and limitations: A total of 298 patients were analyzed. Median tumor size was 7.6 (7-8.5) cm. Median RENAL score was 9 (8-10). Median ischemia time was 25 (2032) min. Median estimated blood loss was 150 (100-300) ml. Sixteen patients had intraoperative complications (5.4%), whereas 66 (22%) had postoperative complications (5% were Clavien grade >= 3). Multivariable analysis revealed that a lower RENAL score (odds ratio [OR] 0.46, 95% confidence interval [CI] 0.21-0.65, p = 0.02) and pathological pT2 stage (OR 0.51, 95% CI 0.12-0.86, p = 0.001) were protective against postoperative complications. A total of 243 lesions (82%) were malignant. Twenty patients (8%) had positive surgical margins. Ten deaths and 25 recurrences/metastases occurred at a median follow-up of 12 (5-35) mo. At univariable analysis, higher pT stagewas predictive of a likelihood of recurrences/metastases (p = 0.048). While there was a significant deterioration of renal function at discharge, this remained stable over time at 1-yr follow-up. The main limitation of this study is its retrospective design. Conclusions: RAPN in the setting of select cT2 renal masses can safely be performed with acceptable outcomes. Further studies are warranted to corroborate our findings and to better define the role of robotic nephron sparing for this challenging indication. Patient summary: This report shows that robotic surgery can be used for safe removal of a large renal tumor in a minimally invasive fashion, maximizing preservation of renal function, and without compromising cancer control. Surgical checklist adherence across urology expertise levels impacts transurethral resection of bladder tumour quality indicators

Summary: This study found that in patients with non-muscle-invasive bladder cancer, the experience of surgeons was inversely associated with the presence of detrusor muscle in the specimen when the perioperative surgical checklist was adequately implemented. However, the experience of surgeons was directly linked to a higher probability of persistent disease during re-transurethral resection of bladder tumor. No differences were noted in 5-year recurrence-free survival based on surgical experience.

Summary: This study evaluated data from 2395 patients with primitive or progression to muscle invasive bladder cancer (MIBC) who underwent radical cystectomy and lymph nodes dissection, and developed a quick nomogram to predict death from cancer and relapse. The nomogram showed high accuracy in predicting both outcomes and can improve communication with patients.

Summary: The widespread use of PSA for prostate cancer diagnosis has led to high rates of overdiagnosis and overtreatment. A combination of the Prostate Health Index (PHI) and Proclarix tests can improve risk stratification of PCa patients at initial diagnosis. An artificial neural network model combining kallikrein markers in PHI and cancer-related markers in Proclarix showed increased accuracy in identifying aggressive PCa at initial diagnosis.

Summary: In this systematic review and meta-analysis, the cumulative bladder cancer (BC) recurrence rates and efficacy profile of different Bacillus of Calmette-Guerin (BCG) strains were compared. The study found that BCG Tokyo 172 had the lowest recurrence rate among studies with <= 3-year recurrence-free survival (RFS). No significant difference was observed among strains in >3-year RFS outcomes. The genetically different BCG strains RIVM, Tice, and Tokyo 172 showed some potential enhanced benefits.

Summary: A multicenter study found that intraoperative blood transfusions (iBT) during robot-assisted radical cystectomy (RARC) increased the risk of recurrence in urothelial carcinoma of the bladder (UCB), but showed no significant association with cancer-specific survival (CSS) and overall survival (OS). Post-operative blood transfusions (pBT) were not associated with worse oncological prognosis.

Summary: This study aimed to understand the ultimate fate of incidental adrenal masses and found that in the US, surveillance is more common than surgical excision for adrenal incidentalomas. Surgery is mainly offered for functional or malignant disease and the choice of surgical approach varies by physician specialty. Minimally invasive surgery predicted lower rates of complications and shorter hospital stay.

Summary: This study evaluated the impact of peritoneal reconfiguration on lymphocele formation in patients undergoing RARP with PLND. Summary: Efforts have been made to predict urinary continence recovery after radical prostatectomy. This study investigated the impact of surgical urethral length preservation on urinary continence after laparoscopic-assisted radical prostatectomy. The results showed that surgical urethral length preservation and intraoperative blood loss of less than 250 mL were associated with urinary continence recovery.

New multiport robotic surgical systems: a comprehensive literature review of clinical outcomes in urology

Summary: The da Vinci robotic platform has dominated the field of robotic surgery for the past 20 years. However, in the past decade, numerous novel multiport robotic surgical systems have been developed and introduced into clinical practice. This review describes the designs, uses, and clinical outcomes of these new surgical robotic systems in urologic surgery. It focuses on the Senhance robotic system, the CMR-Versius robotic system, and the Hugo RAS, while also discussing lesser-known systems such as Avatera, Hintori, and Dexter. The notable features of each system are compared, with a particular emphasis on how they differ from the da Vinci robotic system.

Health Care Utilization and Cost Associated With Urinary Tract Infections in a Privately Insured Spinal Cord Injury Population

Summary: Urinary tract infections (UTIs) following spinal cord injury (SCI) are common and result in increased health care utilization costs. This study characterized the risk factors and costs associated with UTIs in individuals with SCI. The results showed that younger age, female sex, thoracic level of injury, noncommercial insurance, and comorbidities were associated with increased odds of UTIs. UTIs were also associated with higher rates and costs of hospital admission, outpatient services, and prescription refills.
