Summary: Glomerular filtration rate (GFR) is the best index to evaluate kidney function and its use is recommended to assess the risk of iodine contrast medium-induced acute kidney injury (CI-AKI). GFR can be expressed in absolute terms (mL/min) or adjusted to body surface area (mL/min/1.73 m(2)). Absolute GFR represents the excretion capacity, while relative GFR is used to assess the degree of kidney damage and sensitivity for CI-AKI.

Summary: This study compared the risk of acute kidney injury (AKI), dialysis, and mortality in patients undergoing contrast-enhanced CT with propensity score-matched controls. The findings showed no increased risk of AKI, dialysis, or mortality after contrast-enhanced CT in patients with an estimated glomerular filtration rate (eGFR) greater than or equal to 45 mL/min/1.73 m(2). However, caution should be exercised in patients with hypertension and an eGFR of less than or equal to 30 mL/min/1.73 m(2).

Summary: By evaluating the incidence of AKI after elective EVAR and related factors, the study found that the CV/eGFR ratio was associated with the occurrence of AKI and could serve as a predictive factor. Logistic regression analysis showed that baseline eGFR and the CV/eGFR ratio were related to AKI occurrence. The CV/eGFR ratio may be a potential indicator for limiting the amount of contrast media needed for EVAR.

Summary: This retrospective study conducted at Samsung Medical Center in Seoul, South Korea, aimed to investigate the prevalence and risk factors of vancomycin-induced nephrotoxicity in non-ICU pediatric patients. The study found that a longer hospital stay, concomitant use of piperacillin-tazobactam, and a serum trough level of vancomycin above 24.35 mu g/mL were independently associated with VIN. The study suggests that measuring serum trough level of vancomycin can help clinicians prevent VIN in pediatric patients.

Summary: The Swedish Society of Uroradiology has revised their guidelines for iodine contrast media-induced acute kidney injury (CI-AKI) and are more cautious due to lack of prospective controlled studies and low-grade evidence. The imprecision of glomerular filtration rate (GFR) estimating equations also adds uncertainty. However, randomized hydration studies suggest an upper limit risk of about 5% for outpatients with moderate to severe kidney damage and multiple risk factors. The guidelines also include limits for systemic contrast medium exposure in the form of gram-iodine/GFR ratio.

Summary: Through a systematic consensus process, technical recommendations were established for the acquisition, processing, and analysis of renal 2D PC-MRI data in human subjects to promote standardization of renal blood flow measurements and result comparability. The international panel of experts reached consensus and provided specific recommendations on data acquisition, processing, and reporting. These recommendations may encourage widespread adoption of renal PC-MRI and provide reference values for multicenter studies.
