TGF-beta Polymorphisms Are a Risk Factor for Chagas Disease

Transforming growth factor beta 1 (TGF-beta 1) is an important mediator in Chagas disease. Furthermore, patients with higher TGF-beta 1 serum levels show a worse clinical outcome. Gene polymorphism may account for differences in cytokine production during infectious diseases. We tested whether TGFB1 polymorphisms could be associated with Chagas disease susceptibility and severity in a Brazilian population. We investigated five single-nucleotide polymorphisms (-800 G>A, -509 C>T, + 10 T>C, + 25 G>C, and + 263 C>T). 152 patients with Chagas disease (53 with the indeterminate form and 99 with the cardiac form) and 48 noninfected subjects were included. Genotypes CT and TT at position -509 of the TGFB1 gene were more frequent in Chagas disease patients than in noninfected subjects. Genotypes TC and CC at codon + 10 of the TGFB1 gene were also more frequent in Chagas disease patients than in noninfected subjects. We found no significant differences in the distribution of the studied TGFB1 polymorphisms between patients with the indeterminate or cardiac form of Chagas disease. Therefore, -509 C>T and + 10 T>C TGFB1 polymorphisms are associated with Chagas disease susceptibility in a Brazilian population.

The results showed that cardiovascular rehabilitation significantly improved the functional capacity of patients with CCC, and the responses were similar between patients without and with HF, indicating the importance of including it as a standard treatment strategy for CCC.

Socio-epidemiological factors and comorbidities associated with Chagas disease manifestations in two urban reference health care centres in Rio de Janeiro, Brazil

Summary: This study analysed the association between socio-epidemiological factors and comorbidities with clinical manifestations of Chagas disease (CD). The results showed that age and Brazilian birth state were associated with the cardiac form of CD. Men and older individuals were more likely to have the digestive form, while patients with arterial hypertension and diabetes were less likely to have the digestive form. Men had a greater likelihood of having a more severe cardiac presentation.

Summary: This study describes a pediatric case of PAH with a novel GDF2 mutation, resulting in the absence of circulating mature BMP9 and mature BMP9-10 heterodimers in the patient's plasma. Further in vitro studies showed that the mutation hampers the correct processing of BMP9, leading to the secretion of inactive pro-BMP9. Heterozygous carriers of the variant were asymptomatic, similar to previous reports.

Summary: This study investigated the prognostic value of biomarkers and new echocardiogram parameters in predicting outcomes of patients with chronic Chagas disease. The results showed that LV end-diastolic volume, strain analyses, 3-dimensional analyses, brain natriuretic peptide, and positive T cruzi polymerase chain reaction were independent predictors of cardiovascular events in CD patients.

Summary: This review analyzed the relationship between the increase in inflammatory markers associated with chronic fibrosing myocarditis and the reduction in inspiratory muscle strength in patients with chronic Chagas disease. However, there is limited literature in this field. More randomized clinical trials are needed to evaluate inspiratory muscle strength and the effects of training in order to better understand the prevalence and risk of inspiratory muscle weakness in patients with Chagas disease.
