Summary: This study compared prescribing behavior and outcomes for IBD patients during the COVID-19 pandemic and before. The study found a decrease in systemic corticosteroid prescription and an increase in poorly bioavailable oral corticosteroids during the pandemic. Treatment with ustekinumab and vedolizumab also significantly increased. Despite treatment adaptations, steroid-free outcomes were comparable to pre-pandemic levels.

Summary: Hyperbilirubinemia is common in trauma patients, but little is known about its effects on clinical outcomes. This study aimed to assess the relationship between hyperbilirubinemia and patient outcomes following trauma. The study included 387 severely injured patients admitted to the trauma bay. Patients were categorized into low-bilirubin (LB) and high-bilirubin (HB) groups based on peak bilirubin levels. The HB group had significantly higher rates of complications and mortality, including pneumonia, acute kidney injury, sepsis, and wound infections. Multivariate analysis showed that higher bilirubin levels were associated with increased risk of complications. In conclusion, hyperbilirubinemia was associated with poorer outcomes in trauma patients.

Summary: This retrospective analysis studied risk factors for postendoscopic bacteremia in a tertiary center in Israel. Neutropenia, low albumin level, male gender, older age, preprocedure fever, and admitting department were associated with increased risk for bacteremia. However, the risk of postendoscopic bacteremia in neutropenic patients was not significantly different compared to other procedures or the background risk among neutropenic patients.

Summary: In patients with medically refractory inflammatory bowel disease who undergo surgery preserving the rectum, the risk of rectal cancer is low and may be further reduced by surveillance endoscopy. Expectant management with surveillance endoscopy could be a reasonable alternative to completion proctectomy in selected patients.

Summary: This study evaluated the efficacy and safety of a novel homemade endoscopic vacuum therapy for the treatment of transmural gastrointestinal defects. A total of 144 patients were included, with a technical success rate of 100% and a clinical success rate of 88.89%. The use of this homemade EVT was found to be feasible, safe, and effective for the management of TGID.

Summary: This study compared patients with and without sedation during emergency endoscopy for upper gastrointestinal bleeding (UGIB) and clarified the safety and efficacy of sedation in emergency endoscopy. The sedation group had a shorter procedure time and similar success rate of endoscopic hemostasis compared to the nonsedation group. There were no significant differences in adverse events between the two groups. Factors such as red blood cell transfusion and rebleeding were associated with increased risk of 30-day mortality from UGIB.
