A fatal case of thiacloprid poisoning

Neonicotinoid insecticides are considered to be less toxic to humans compared to older insecticides such as organophosphates, carbamates, pyrethroids, and organochlorine compounds. However, reports of severe human toxicity with neonicotinoids are emerging. Acute human thiacloprid poisoning and death as a result have not been reported in the literature so far. Here we report a case of thiacloprid poisoning resulting from deliberate ingestion in a 23-year-old man, manifesting with status epilepticus, respiratory paralysis, rhabdomyolysis, metabolic acidosis, and acute kidney injury (AKI), and ultimately giving rise to refractory shock and death. Thiacloprid can cause fatal human toxicity when ingested heavily, and absence of an effective antidote raises concern in this regard.

Assessment of vascular function in complete glycaemic spectrum

Clostridioides difficile Diarrhea: An Emerging Problem in a South Indian Tertiary Care Hospital

Summary: This study aimed to determine the burden and risk factors of CDI among patients with hospital-acquired diarrhea in India. The prevalence of CDI was found to be 18.67%, with prolonged hospital stay being the major risk factor. Addition of GeneXpert for toxin gene detection increased the yield from 12% to 18.68%.

Summary: A 27-year-old woman presented with a variety of symptoms, and was diagnosed with systemic lupus erythematosus and immune-mediated intravascular hemolysis. Symptoms resolved following treatment and there were no further episodes of hemolysis during follow-up.

Association between serum netrin-1, netrin-4 and risk of the acute coronary syndrome in patients with type 2 diabetes mellitus-A pilot study

Summary: The present study aimed to assess the diagnostic value of serum netrin-1 and netrin-4 in recognizing acute coronary syndrome (ACS) in patients with type 2 diabetes mellitus (T2DM). It was found that T2DM patients with ACS had lower levels of serum netrin-1 and netrin-4, which were negatively associated with other biochemical markers. ROC analysis showed that netrin-1 and netrin-4 had good sensitivity and specificity for predicting ACS in T2DM patients. Therefore, serum netrin-1 and netrin-4 levels could be considered as complementary markers for ACS diagnosis in T2DM patients.

Venom induced consumption coagulopathy and performance of 20-min whole blood clotting test for its detection in viperid envenomation

Summary: This study evaluated venom induced consumption coagulopathy (VICC) in viperid envenomation (VE) patients and found that the majority of patients had VICC, with 68% of them experiencing overt bleeding. The 20-min whole blood clotting test (20WBCT) showed good performance in detecting VICC, but false positive results should be considered before deciding on snake antivenom treatment.

Body Iron Store and its Association with Risk of First Episode of Spontaneous Lower Extremity Deep Vein Thrombosis/Pulmonary Embolism: A Case-Control Study

Summary: The association between body iron stores and the risk of deep vein thrombosis/pulmonary embolism (DVT/PE) among Indian subjects was studied. Anemia and elevated red cell distribution width (RDW) were significantly associated with increased risk of DVT/PE, while iron deficiency was not. These findings suggest that higher iron stores may be a risk factor for DVT/PE.

Summary: This study found that the expression of ADAM10 in PBMC and serum sRAGE levels were lower in T2DM patients with ACS compared to those without ACS. Serum sRAGE levels and ADAM10 expression were correlated with markers of cardiac injury and glycaemic status. ADAM10 expression and serum sRAGE levels could be used to predict ACS risk and diagnose ACS in T2DM patients.

Do young children with known cannabis intoxication benefit from further neurological-based testing or imaging?

Summary: Recent research indicates that children who unintentionally ingest cannabis often undergo extensive additional testing, such as head imaging or lumbar puncture. However, the yield of these tests is limited. While head imaging and lumbar puncture are frequently performed, diagnoses such as skull fracture, intracranial hemorrhage, intracranial neoplasm, meningitis, or intracranial abscess are rare. Additionally, discharge diagnoses related to other drugs are infrequent. The most common co-diagnoses are cocaine and opioids. Therefore, prompt consideration of cannabis ingestion and quick identification through testing may result in more effective neuroimaging outcomes.

Performance of Google bard and ChatGPT in mass casualty incidents triage

Association between positive blood culture and clinical outcomes among children treated for sepsis in the emergency department
