Bone markers and cardiovascular risk in type 2 diabetes patients

Background: Vascular calcifications are associated with a three- to fourfold increased risk of cardiovascular disease (CVD) and are highly prevalent in type 2 diabetes patients. Emerging evidence indicates that vascular calcification is a process of active bone formation regulated by stimulators and inhibitors of calcification. Therefore, this study aimed to evaluate whether six bone markers are associated with CVD risk in patients with type 2 diabetes. Methods: We used data of a case-cohort study, nested in the EPIC-NL cohort, comprising 134 CVD cases and a random subcohort of 218 participants, all with type 2 diabetes at baseline. Six bone markers (osteocalcin, osteopontin, osteonectin, osteoprotegerin, alkaline phosphatase and sclerostin) were measured in baseline plasma samples with multiplex assays and information on CVD events was obtained. The association of bone makers with CVD risk was evaluated using Cox proportional hazard analyses. Results: Higher concentrations of plasma osteopontin were associated (p(trend) < 0.01) with an increased CVD risk with a hazard ratio of 2.00 (95%-CI 1.20-3.35) for the highest versus the lowest quartile in a multivariable adjusted model. The other bone markers were not associated with CVD risk. Conclusions: Higher osteopontin concentrations were associated with an increased CVD risk in type 2 diabetes patients. No consistent associations were found for the other five bone markers and risk of CVD in type 2 diabetes patients.

Summary: This study found that pyridoxamine (PM), a B6 vitamer and dicarbonyl scavenger, can reduce methylglyoxal (MGO), protein-bound N delta-(5-hydro-5-methyl-4-imidazolon-2-yl)-ornithine (MG-H1), soluble vascular cell adhesion molecule-1 (sVCAM-1) and soluble intercellular adhesion molecule-1 (sICAM-1). However, PM does not affect insulin sensitivity and vascular function.

Advanced glycation endproducts in diabetes-related macrovascular complications: focus on methylglyoxal

Summary: Diabetes is linked to vascular injury and the development of macrovascular complications. Advanced glycation endproducts (AGEs) and the precursor methylglyoxal (MGO) play important roles in connecting diabetes and vascular injury. While most studies have focused on the relationship between AGEs and vascular injury, the effects of MGO on vasculature are less understood. This review explores the mechanisms linking AGEs and MGO to the progression of atherosclerosis. AGEs and MGO are involved in various stages of atherosclerosis, but further research is needed to understand the exact underlying mechanisms. Nonetheless, targeting AGEs and MGO could provide therapeutic strategies for the macrovascular complications of diabetes.

The Intake of Dicarbonyls and Advanced Glycation Endproducts as Part of the Habitual Diet Is Not Associated with Intestinal Inflammation in Inflammatory Bowel Disease and Irritable Bowel Syndrome Patients

Summary: A Western diet high in dicarbonyls and advanced glycation endproducts (AGEs) may worsen inflammation in inflammatory bowel disease (IBD) and irritable bowel syndrome (IBS). This study examined the intake of dicarbonyls and AGEs in the habitual diet of IBD and IBS patients and their association with intestinal inflammation. The results showed that the absolute intake of dicarbonyls and AGEs was higher in IBD and healthy control subjects compared to IBS. However, after adjusting for energy intake, only glyoxal was lower in IBD compared to IBS and healthy controls. No significant association was found between dietary dicarbonyls and AGEs and intestinal inflammation in either patient group. The findings suggest that potential harmful effects of these compounds in the diet of Dutch patients with IBD or IBS may be counteracted by anti-inflammatory components in the food matrix.

Summary: This study used population-based observational cross-sectional data to assess the associations between glucose metabolism status and measures of glycaemia with corneal nerve fibre measures. The results showed a linear association between adverse glucose metabolism status, higher levels of glycaemic measures, and corneal neurodegeneration. This study provides evidence that hyperglycaemia-associated corneal neurodegeneration is a continuous process that occurs before the onset of type 2 diabetes.
