The effectiveness of conservative and surgical treatment for shoulder stiffness: a systematic review of current literature

Introduction: Currently, no therapeutic intervention is universally accepted, and the most effective management for restoring motion and diminishing pain in patients with shoulder stiffness has yet to be defined. This systematic review analyses outcomes of conservative and surgical interventions to treat shoulder stiffness. Source of data: A systematic review of literature according to the PRISMA guidelines was performed. A comprehensive search of PubMed, Medline, CINAHL, Cochrane, Embase, Ovid and Google Scholar databases using various combinations of the keywords 'shoulder', 'shoulder stiffness', 'stiff shoulder', 'conservative', since inception of databases to June 2018 was performed. Areas of agreement: Shoulder stiffness could be treated with conservative means including nonsteroidal anti-inflammatory medications, corticosteroid injections, or transcutaneous electrical nerve stimulation, manipulation under anaesthesia, and arthroscopic capsular release. Areas of controversy: No therapeutic intervention is universally accepted, and the most effective management to restore motion and diminish pain in patients with shoulder stiffness has yet to be defined. Growing points: The rate of failure after treatment for stiff shoulder is higher in the surgical group than in the conservative group. Areas timely for developing research: There is insufficient evidence to establish whether surgical or conservative management is the best choice to manage shoulder stiffness. Prospective, randomized studies are needed to establish whether surgical or conservative management produce a clinically relevant difference in functional outcome.

Open anatomical glenoid reconstruction with an iliac crest bone autograft effectively resolves off-track Hill-Sachs lesions to on-track lesions

Summary: This study aimed to investigate whether iliac bone autografts with screw fixation could transform off-track Hill-Sachs lesions into on-track lesions in patients with dynamic anteroinferior instability. The results showed that the off-track lesions were successfully transformed into on-track lesions after 6 weeks of surgery and remained stable during the final follow-up. The study also observed bony remodeling processes over time, indicating a slight decrease in glenoid surface area.

Micro-fragmented and nanofat adipose tissue derivatives: In vitro qualitative and quantitative analysis

Summary: This study aimed to compare the biological properties of micro-fragmented adipose tissue (MAT) and nanofat adipose tissue (NAT) obtained with different techniques. The results showed that NAT had the highest content of extracellular matrix, cell replication, and stemness gene expression, providing valuable clues for the clinical use of adipose tissue derivatives.

Advances in surgery and understanding of biological concepts have influenced ACL postoperative rehabilitation protocols, but new technologies may introduce inconsistency. Rehabilitation protocols should consider accurate diagnosis, patient expectations, and geographical differences in ACL rehabilitation.

Summary: Background: Infections after unicompartmental knee arthroplasty (UKA) surgery are rare and poorly studied. Optimal management of these infections is unclear. This study presents the results of a large multicentre clinical study of UKA infections treated with the Debridement, Antibiotics and Implant Retention (DAIR) procedure. Results showed high success rates and implant survivorship with the DAIR procedure.

Induced Models of Osteoarthritis in Animal Models: A Systematic Review

Summary: This study aimed to determine the most appropriate animal model for the study of osteoarthritis (OA) by conducting a literature review and analyzing 36 selected studies. The results showed that various induction models, including mechanical, surgical, and chemical methods, have been proven suitable for inducing OA in animals. However, there is still no clear gold standard for choosing the best animal model for OA. This study provides valuable information for researchers and contributes to our understanding and treatment of OA.

Allografts for Medial Patellofemoral Ligament (MPFL) Reconstruction in Adolescent Patients with Recurrent Patellofemoral Instability: A Systematic Review

Summary: This systematic review provides updated evidence on the use of allografts for medial patella-femoral ligament (MPFL) reconstruction. Patient-reported outcome measures, redislocation rates, and complication rates were examined as outcomes. The review followed the PRISMA guidelines and included studies on adolescents and children with recurrent patellofemoral instability (PFI). The results showed improved Kujala and IKDC scores, low rates of dislocation, reoperation, and subluxation. The findings support the use of allografts for MPFL reconstruction in adolescent patients with recurrent patellofemoral instability, but more high-quality studies are needed to fully establish the long-term benefits and complication rates of allograft MPFL.

Summary: The role of chloroquine (CQ) and hydroxychloroquine (HCQ) in the management of dry eye disease is still uncertain. This systematic review and meta-analysis found that CQ and HCQ improved symptoms and signs of dry eye disease, with increased tear breakup time and Schirmer I test, and decreased Ocular Surface Disease Index (OSDI) and corneal staining.

Summary: A novel machine learning algorithm was proposed to predict the outcomes of arthroscopic rotator cuff repair. A machine learning model was developed using pre-operative factors and the American Shoulder and Elbow Surgeons scores. The algorithm accurately predicted post-operative scores, which can be used for pre-operative counseling, planning, and resource allocation.
