Oocyte donation outcome after oncological treatment in cancer survivors

Objective: To study reproductive outcome in patients cured of cancer who required oocyte donation (OD) owing to iatrogenic ovarian dysfunction. Design: Multicenter, unmatched, retrospective cohorts study. Setting: Private, university-affiliated group of clinics. Patient(s): Women treated and cured of cancer (n = 142) who underwent 333 cycles of OD (exposed group) and women without a previous cancer diagnosis (n = 17,844) who underwent 29,778 cycles of OD (unexposed cohort) between January 2000 and January 2012. Intervention(s): Retrospective chart review. Main Outcome Measure(s): Pregnancy, implantation, miscarriage, and ongoing pregnancy rates. Result(s): There were no differences in terms of pregnancy (55.7% vs. 54.7%), implantation (39.8% vs. 38.2%), miscarriage (29.5% vs. 26.9%), or delivery rates (39.3% vs. 39.9%) between the unexposed group and the patients previously diagnosed and cured of cancer, respectively. There was no correlation between OD outcome and cancer type. Conclusion(s): Endometrial receptivity in women treated and cured of cancer was comparable to that of general patients without previous malignancies who had received OD, based on the largest series available in the literature. Influence of telomerase activity and initial distribution on human follicular aging: Moving from a discrete to a continuum model

Summary: This paper proposes a discrete model for the temporal evolution of a population of cells sorted according to their telomeric length. The model assumes asymmetric distribution of genetic material during cell division, with one daughter cell having the same telomere length as the mother and the other having shorter telomeres. The model takes into account telomerase activity, cell death, and the Hayflick limit. A partial differential equation with boundary conditions is derived to represent the continuous model. The model is applied to study the aging rate of human follicular growth.

Summary: Currently, there is a lack of understanding and awareness about the causes of male infertility and the importance of male reproductive health. This leads to standardized treatment strategies using medically assisted reproduction techniques, which overlook the potential co-morbidities and shift the burden to women. To improve male reproductive health, it is essential to establish centers for data and sample collection and study the effects of genetics, epigenetics, and environment on fertility and offspring health. Personalized treatment protocols should be developed and education about infertility should be increased.
