Near-Infrared Lymphatic Mapping of the Recurrent Laryngeal Nerve Nodes in T1 Esophageal Cancer

Background. It is still unclear that dissection of recurrent laryngeal nerve nodes is mandatory in patients with cTl middle or lower thoracic esophageal squamous cell carcinoma when the nodes are negative in preoperative staging workup. We aimed to evaluate the feasibility of near-infrared image-guided lymphatic mapping of bilateral recurrent laryngeal nerve nodes. Methods. The day before operation, we injected indocyanine green (ICG) into the submucosal layer by endoscopy. At the time of upper mediastinal dissection, ICG-stained basins were identified along the bilateral recurrent laryngeal nerves and retrieved under guidance of the Firefly system. After the operation, remnant ICG-unstained basins were dissected from the specimen to assess the presence of metastasis. Results. Of 29 patients enrolled, ICG-stained basins could be identified in 25 patients (86.2%), and 6 of them (24.0%) had nodal metastasis; 4 in the right recurrent laryngeal nerve chain, 1 in the left recurrent laryngeal nerve chain, and 1 in both recurrent laryngeal nerve chains. On pathologic examination of 345 recurrent laryngeal nerve nodes, two metastatic nodes were identified in ICG-unstained basins along the left recurrent laryngeal nerve in a patient who had lymph node metastases in ICG-stained basins along both recurrent laryngeal nerves. Negative predictive value in detection of nodal metastasis was 100% for the right recurrent laryngeal nerve chain and 98.2% for the left recurrent laryngeal nerve chain. Conclusions. Real-time assessment of recurrent laryngeal nerve nodes with near-infrared image was technically feasible, and we could detect lymphatic basins that most likely have nodal metastasis. Clinical Outcomes and Prognosis of Patients With Interstitial Lung Disease Undergoing Lung Cancer Surgery: A Propensity Score Matching Study

Summary: We compared the prognosis after lung cancer surgery between 104 patients with ILD and 104 patients without ILD who were matched by key variables. The findings showed that patients with ILD had worse survival rates, especially those with concomitant IPF or high physiological severity of ILD. Evaluating the underlying ILD before surgery is considered helpful in predicting the prognosis after lung cancer surgery.

Summary: The study aimed to validate the effectiveness of the IASLC grading system in Korean patients and propose a modification to improve its predictive ability, especially for grade 2 patients. By reviewing the histopathologic characteristics of 1358 patients with invasive pulmonary adenocarcinoma, the validity of the revised model (Lepidic-10) was evaluated and compared to the IASLC system. The results showed that the Lepidic-10 model had better performance in predicting survival and recurrence time in patients from two hospitals compared to the IASLC grading.

Relieving Anxiety Through Virtual Reality Prior to Endoscopic Procedures

Summary: This study found that exposing patients to virtual reality (VR) prior to endoscopic procedures can reduce their anxiety levels, leading to less discomfort and incomplete procedures, as well as decreased use of sedative medication. The proportion of patients with high anxiety significantly decreased in the VR group compared to the control group, and patients in the VR group reported higher satisfaction with sedation.

Nomogram for pre-procedural prediction of non-curative endoscopic resection in patients with early gastric cancer

Intravascular Ultrasound Guided Intervention in Calcified Coronary Lesions Showed Good Clinical Outcomes during One Year Follow-Up

Mobile Apps to Improve Brace-Wearing Compliance in Patients with Idiopathic Scoliosis: A Quality Analysis, Functionality Review and Future Directions

Summary: This study aimed to evaluate the current mHealth apps that improve brace-wearing compliance by assessing their quality and functionalities. Ten apps were identified from literature review and commercial app markets. The apps were evaluated based on transparency, health content, technical content, security/privacy, usability issues, and subjective ratings. The overall quality of the apps was moderate, with none reaching a high or excellent quality. The transparency section had the highest rating, while the security/privacy section had the lowest rating. There is a need for developing high-quality apps with appropriate functionalities to support brace treatment for patients with idiopathic scoliosis.

Association of immune-mediated inflammatory diseases with depression and anxiety in patients with type 2 diabetes: A nationwide population-based study

Summary: This study aimed to assess whether the presence of immune-mediated inflammatory diseases (IMIDs) confers a higher risk of depression and anxiety in patients with type 2 diabetes (T2DM). The results showed that the existence of gut, joint, and skin IMIDs was associated with a higher risk of depression and anxiety. Patients with multiple IMIDs had a higher risk than those with only one.

Sample Collection Methods in Upper Gastrointestinal Research

Summary: Significant translational research advances have been made in the upper gastrointestinal (GI) research field in recent years. Endoscopic evaluation is a reasonable option for acquiring upper GI tissue for research purposes with minimal risk. The optimal number of biopsy samples and sample storage is crucial and might influence results. Few reports have been published on methods for sample collection from endoscopic biopsies. This review provides a protocol for collecting study samples for upper GI research, including microbiome, DNA, RNA, protein, single-cell RNA sequencing, and organoid culture, through a comprehensive literature review. Different methods and sample sizes are recommended for different types of analysis.

Effectiveness of C1 Laminectomy for Chiari Malformation Type 1: Posterior Fossa Volume Expansion and Syrinx-Volume Decrease Rate

Summary: This study aimed to analyze the effect of foramen magnum decompression with C1 laminectomy (C1L) for Chiari malformation type 1 (CM-1) in terms of improving clinical symptoms, expanding posterior fossa volume, and decreasing syrinx volume. The results showed that C1L can effectively increase posterior fossa volume, and combining C1L with duraplasty can result in a faster decrease in syrinx volume.
