Summary: The study aimed to investigate the predictive value of synovitis and tenosynovitis detected by grayscale and power Doppler ultrasound in relation to failure of tapering disease-modifying antirheumatic drugs in rheumatoid arthritis patients. Results suggest that PD synovitis and GS tenosynovitis may be useful in identifying RA patients at risk of relapse after DMARD tapering.

Summary: This study found that unilateral SASD bursitis was more common in patients with PMR compared to those with bilateral FS, while unilateral inferior GH synovitis was more frequent in bilateral FS patients. Patients with bilateral inferior GH synovitis without bilateral SASD bursitis were more likely to have bilateral FS.

Summary: Musculoskeletal ultrasound (MSUS) is an important tool in pediatric rheumatology for detecting subclinical disease activity and treating patients during the window of opportunity. However, its role in assessing remission in JIA patients is not well-defined. This systematic review analyzes published literature on MSUS in JIA patients in remission, finding that abnormal MSUS findings, such as subclinical synovitis and Power Doppler signal, are common even in remission. Further studies are needed to better understand the implications and develop a standardized scoring system.

Summary: Subclinical synovitis is linked to structural damage progression and flare risk, indicating clinical composite scores may not fully suppress inflammation; Sustained clinical remission impacts long-term joint damage progression, physical function, and quality of life; Treating subclinical inflammation leads to more frequent strict clinical remission and better physical function, with potential for more adverse events.

Summary: Treatment with Baricitinib for 24 weeks significantly decreased disease activity composites such as DAS28-ESR, SDAI, and CDAI in patients with RA. Improvement in ultrasound-detected gray-scale and power Doppler synovitis and joint effusion was observed after 24 weeks of Baricitinib treatment. Changes in power Doppler score were significantly associated with changes in DAS28-ESR, but not SDAI and CDAI.

Summary: Most patients with rheumatoid arthritis (RA) in sustained remission will experience flares during tapering of medication, but the majority will regain remission after gradually increasing the dose. MRI and ultrasound can be used as predictors of flares.

Summary: This study focused on the disease-flare process in rheumatoid arthritis (RA) after discontinuing biological disease-modifying antirheumatic drugs (bDMARDs). The baseline level of soluble tumor necrosis factor receptor 1 (sTNFR1) was found to impact subsequent disease flare, while a higher baseline interleukin (IL)-2 concentration was only beneficial to patients with lower sTNFR1 levels, leading to maintained remission in a significant percentage of patients.

Summary: This study aimed to standardize and evaluate the reliability of ultrasonographic assessment of inflammatory and structural lesions in patients with hand osteoarthritis. The results showed that ultrasound had substantial to excellent intra-reader reliability for inflammatory features of hand OA, but the inter-reader reliability varied and the reliability of structural features was relatively low.

Summary: Early diagnosis and tight control improve outcomes of rheumatoid arthritis. This study compared the effectiveness and costs of an early arthritis clinic (EAC) with the current standard of care. The results showed that despite higher costs, the EAC reduced the number and length of hospitalizations and had higher adherence to quality-of-care guidelines.

Summary: Cryoglobulinemic vasculitis is a small-vessel vasculitis characterized by the proliferation of B-cell clones producing pathogenic immune complexes. It is often secondary to hepatitis C virus, autoimmune diseases, and hematological malignancies. The clinical course of cryoglobulinemic vasculitis is usually mild, but severe organ damage and life-threatening manifestations can occur.

Summary: This study assessed the reliability and diagnostic accuracy of new radiographic imaging definitions developed by an international multidisciplinary working group for identification of calcium pyrophosphate deposition (CPPD). The results showed that the new radiographic definitions have high specificity for the diagnosis of CPPD, but the sensitivity remains low for all sites and the overall diagnosis.

Summary: Several studies show that age at onset has an impact on the clinical-serological presentation, comorbidities and disease course of patients with systemic lupus erythematosus (SLE). This study evaluated the correlation between age at onset and clinical-serological manifestations and comorbidities in patients with recent onset SLE. The results showed that early onset SLE had a more active immunological profile, while late onset SLE had a higher incidence of comorbidities.

Summary: This study aimed to evaluate the effect of adherence to conventional synthetic disease-modifying anti-rheumatic drugs (csDMARDs) on the achievement of clinical remission in patients with new-onset inflammatory arthritis. The results showed that patients with higher adherence to csDMARDs were more likely to achieve clinical remission.
