Air Pollution and Asthma Mechanisms of Harm and Considerations for Clinical Interventions

Paul E. Pfeffer, Ian S. Mudway, Jonathan Grigg

Summary: There is global concern about the harmful impact of polluted air on respiratory health of asthma patients. Despite multiple epidemiologic studies showing ongoing associations, inconsistent findings suggest a need for experimental studies to clarify the mechanistic basis of the effects of ambient pollution on the lungs.

Asthma prescribing according to Arg16Gly beta-2 genotype: a randomised trial in adolescents

Tom Ruffles, Christina J. Jones, Colin Palmer, Steve Turner, Jonathan Grigg, Roger Tavendale, Fiona Hogarth, Petra Rauchhaus, Kristina Pilvinyte, Romanie Hannah, Helen Smith, Roberta Littleford, Brian Lipworth, Somnath Mukhopadhyay

Summary: This study demonstrates that genotype-driven asthma prescribing is associated with a significant improvement in a clinical outcome compared to standard care, especially benefiting adolescents with the AA homozygous genotype. The potential role of such beta(2)-adrenoceptor genotype directed therapy in younger and more severe childhood asthma warrants further exploration.

Evidence for the presence of air pollution nanoparticles in placental tissue cells

Norrice M. Liu, Lisa Miyashita, Barbara A. Maher, Graham McPhail, Carolyn J. P. Jones, Benjamin Barratt, Shakila Thangaratinam, Vassil Karloukovski, Imada A. Ahmed, Zabeada Aslam, Jonathan Grigg

Summary: The study found that inhaled metal-bearing air pollution-derived PM can translocate to distant organs and be taken up by tissue resident phagocytes in vivo, including potentially affecting the fetus in the uterus. This suggests that the human placenta may be a primary target for these particles.

An enhanced care package to improve asthma management in Malawian children: a randomised controlled trial

Sarah Rylance, Beatrice Chinoko, Bright Mnesa, Chris Jewell, Jonathan Grigg, Kevin Mortimer

Summary: The study showed that an outpatient asthma care package, including task-shifting of asthma management roles, significantly improved asthma symptoms and reduced exacerbations for children aged 6-15 diagnosed with asthma. This intervention also led to fewer emergency healthcare visits and school absences compared to standard care, indicating potential benefits for asthma patients in resource-limited settings.

Prostaglandin E2 and phagocytosis of inhaled particulate matter by airway macrophages in cystic fibrosis

Norrice M. Liu, Lisa Miyashita, Marek Sanak, Benjamin Barratt, Jonathan Grigg

Summary: The study reveals that increased PGE(2) in the CF airway suppresses phagocytosis of inhaled PM by AM. Despite no difference in PM exposure between groups, the area of phagocytosed carbon by AM was decreased in children with CF.
