Diagnosis and surveillance of late-onset fetal growth restriction

By consensus, late fetal growth restriction is that diagnosed >32 weeks. This condition is mildly associated with a higher risk of perinatal hypoxic events and suboptimal neurodevelopment. Histologically, it is characterized by the presence of uteroplacental vascular lesions (especially infarcts), although the incidence of such lesions is lower than in preterm fetal growth restriction. Screening procedures for fetal growth restriction need to identify small babies and then differentiate between those who are healthy and those who are pathologically small. First- or second-trimester screening strategies provide detection rates for late smallness for gestational age <50% for 10% of false positives. Compared to clinically indicated ultrasonography in the third trimester, universal screening triples the detection rate of late smallness for gestational age. As opposed to early third-trimester ultrasound, scanning late in pregnancy (around 37 weeks) increases the detection rate for birthweight <3rd centile. Contrary to early fetal growth restriction, umbilical artery Doppler velocimetry alone does not provide good differentiation between late smallness for gestational age and fetal growth restriction. A combination of biometric parameters (with severe smallness usually defined as estimated fetal weight or abdominal circumference <3rd centile) with Doppler criteria of placental insufficiency (either in the maternal [uterine Doppler] or fetal [cerebroplacental ratio] compartments) offers a classification tool that correlates with the risk for adverse perinatal outcome. There is no evidence that induction of late fetal growth restriction at term improves perinatal outcomes nor is it a cost-effective strategy, and it may increase neonatal admission when performed <38 weeks.

Summary: This study aims to explore the prevalence of brain anomalies and their relationship with spinal defects in fetuses eligible for prenatal open spina bifida repair. The results showed that more than half of the fetuses had abnormal corpus callosum and/or abnormal ventricular wall before the prenatal repair. Furthermore, non-sacral lesions and wider lateral ventricles were associated with an increased risk of brain abnormalities. Therefore, a detailed preoperative evaluation of the central nervous system is crucial in fetuses with open spina bifida.

Summary: This analysis suggests that fetoscopic endoluminal tracheal occlusion improves survival in cases of moderate and severe lung hypoplasia. The timing of balloon insertion may impact the treatment effect, but further research is needed to determine its precise influence due to the limited sample size and confounding factors. In particular, early insertion of the balloon greatly increases the risk of preterm delivery.

Summary: COVID-19 has a lesser impact on children compared to adults, but they can still get infected and transmit the virus. A study evaluated the use of high-throughput Luminex assays to measure SARS-CoV-2 antibodies in saliva and compared them to serum/plasma samples in infected children and adults. The results showed a correlation between saliva and serum/plasma antibody levels, and infected children had higher antibody levels, suggesting stronger clinical immunity. Therefore, measuring saliva antibodies could be a useful tool to determine COVID-19 prevalence and transmission in pediatric populations.

Summary: The study aimed to assess the cortical maturation pattern in fetuses with CMV infection using MRI, and compare it with healthy controls. Results showed that even in non-severely affected fetuses, CMV infection could be associated with altered brain cortical structure.

Summary: This study aimed to investigate the measurements of midbrain morphology on neurosonography in late-onset small fetuses classified as small-for-gestational-age (SGA) or growth-restricted (FGR). The results showed that small fetuses had significantly shorter corpus callosum-fastigium length and tectal length compared to controls, and these differences were more pronounced in fetuses with more severe smallness. These findings suggest the potential value of midbrain measurements as biomarkers for brain development in a high-risk population.

Halo sign in fetal cytomegalovirus infection: cerebral imaging abnormalities and postmortem histopathology in 35 infected fetuses

Summary: The study aimed to evaluate the correlation between the periventricular echogenic halo (halo sign) and histopathological findings, as well as its association with other brain imaging abnormalities in fetuses with cytomegalovirus (CMV) infection. The study included CMV-infected fetuses diagnosed with severe CNS abnormalities seen on ultrasound. The halo sign was observed in 91% of the fetuses, with ventriculitis being more frequent in cases with a non-isolated halo sign. Histopathological analysis showed that fetuses with an isolated halo sign had mild ventriculitis without severe brain lesions. The study suggests that detailed neurosonographic follow-up and MRI examination should be considered in the early third trimester for prognostic evaluation of CMV-infected fetuses with the halo sign as an isolated finding. An automatic pipeline for atlas-based fetal and neonatal brain segmentation and analysis

Acute fetal cardiovascular adaptation to artificial placenta in sheep model

Summary: This study aimed to describe the acute cardiovascular adaptation of the fetus after connection to an artificial placenta (AP) using ultrasound and invasive and non-invasive hemodynamic assessment. The results showed that the connection to the AP system resulted in a reduction in the pulsatility index and increased umbilical venous peak velocity. While cardiac structure and function were preserved in the short term, the system led to non-physiologically elevated venous pressure and pulsatile flow.

Summary: This study describes the development of an artificial placenta (AP) system in sheep, facing challenges and bottlenecks but ultimately achieving survival up to one week. A total of 28 fetal sheep were transferred to the AP system, with progressively increased survival goals. Results showed improved success rates over the course of the study, but common issues still hindered the system in the first two groups. The study concludes that developing a reproducible AP system requires significant time, effort, and a highly multidisciplinary team.

Longitudinal evolution of central nervous system anomalies in fetuses with open spina bifida fetoscopic repair and correlation with neurologic outcome

Summary: This study investigated the changes in central nervous system anomalies before and after prenatal open spina bifida repair and their relationship with postnatal neurologic outcomes. The results showed a reduction in hindbrain herniation and a normalization of the clivus supraocciput angle after surgery, but no significant increase in abnormal corpus callosum or heterotopia. However, the presence of abnormal corpus callosum and severe ventriculomegaly was associated with an increased risk of suboptimal neurodevelopment.

Fetal growth velocity according to the mode of assisted conception

Summary: This study found that pregnancies conceived through assisted reproductive techniques (ART) have a higher risk of placental related disease in the third trimester. Comparing pregnancies conceived with autologous oocytes to those conceived with donated oocytes, it was observed that ART pregnancies have a lower fetal growth velocity from the second trimester to delivery, particularly in pregnancies with donated oocytes. These findings suggest that pregnancies achieved through ART, especially with donated oocytes, may be at a higher risk of placental dysfunction and require closer follow-up.
