Summary: This study describes two cases of AIDS-related secondary HLH treated with ruxolitinib. However, the safety and efficacy of this novel treatment need to be evaluated in large clinical trials in the future.

Summary: This case study reports the diagnosis and successful treatment of an older male patient with HLH related to previous SARS-CoV-2 infection. The patient responded poorly to classical therapy, but showed improvement after treatment with ruxolitinib.

Summary: Hemophagocytic lymphohistiocytosis (HLH) is a rare hyperinflammatory syndrome driven by overactive T cells and macrophages that abundantly secrete pro-inflammatory cytokines. Etoposide-based regimens have reduced mortality associated with HLH, but the 5-year survival rate remains low. Novel cytokine-directed therapies, such as ruxolitinib, are being studied to improve outcomes in HLH.

Case report: Ruxolitinib plus dexamethasone as first-line therapy in haemophagocytic lymphohistiocytosis

Summary: Haemophagocytic lymphohistiocytosis (HLH) is a cytokine-driven inflammatory syndrome caused by uncontrolled hypersecretion of inflammatory cytokines. Conventional first-line treatment for HLH included HLH-94 and HLH-2004 regimens. We reported two cases of HLH, one caused by natural killer (NK)/T-cell lymphoma and another associated with missense variants in the perforin 1 gene. They both received the ruxolitinib plus dexamethasone protocol and had a rapid response to treatment without obvious adverse effects. Our report indicates that treatment with ruxolitinib plus dexamethasone might be a potential option for HLH, and clinical trials warrant further investigation. In addition, the detection of HLH-related genes is necessary for the identification of late-onset familial HLH in certain settings.

A predictive model for identifying secondary underlying diseases of hemophagocytic lymphohistiocytosis

Summary: This study aimed to establish a predictive model for the timely diagnosis of the original disease resulting in secondary HLH by validating clinical and laboratory findings. The results showed that lower levels of hemoglobin and platelets, lower levels of ferritin, splenomegaly, and Epstein-Barr virus positivity were associated with hematologic disease, while young age and female sex were associated with rheumatic disease. The established predictive model can assist clinicians in diagnosing the underlying disease leading to secondary HLH during routine practice.

Summary: Research has shown that RSPO2 and RSPO3 can serve as potential therapeutic targets as they modulate Wnt signaling and promote epithelial cell proliferation and survival. High levels of RSPO2 and RSPO3 expression are characteristic of a subset of colorectal and breast cancers, and are associated with an activated Wnt signaling gene expression profile. Knockdown of RSPO2 reduces Wnt signaling and proliferation in breast cancer cells.

Summary: A novel LCK inhibitor, NTRC 0652-0, was found to inhibit YAP signaling in CCA cells. It decreased tyrosine phosphorylation, nuclear localization, and co-transcriptional activity of YAP, leading to apoptotic cell death in CCA cell lines. FGFR2 fusion-positive CCA was identified as a susceptible subset, and NTRC 0652-0 showed preclinical efficacy in patient-derived organoid and xenograft models.

Physical activity is associated with reduced risk of liver disease in the prospective UK Biobank cohort

Summary: This study aimed to clarify the association between accelerometer-measured physical activity and chronic liver disease by examining risk of overall and specific liver diseases and their progression in relation to accelerometer-based physical activity in 96,688 participants in the UK Biobank. The results showed a clear, dose-dependent protective association between accelerometer-measured physical activity and liver disease development and progression, suggesting the potential use of wearables for personalized prevention of liver diseases.
