Introduction: The interplay between midlife vascular risk factors and midlife cognitive function with later life mild cognitive impairment (MCI) and dementia (DEM) is not well understood. Methods: In the Atherosclerosis Risk in Communities Study, cardiovascular risk factors and cognition were assessed in midlife, ages 45-64 years. In 2011-2013, 20-25 years later, all consenting Atherosclerosis Risk in Communities participants underwent a cognitive and neurological evaluation and were given adjudicated diagnoses of cognitively normal, MCI, or DEM. Results: In 5995 participants with complete covariate data, midlife diabetes, hypertension, obesity, and hypercholesterolemia were associated with late-life MCI and DEM. Low midlife cognition function was also associated with greater likelihood of late-life MCI or DEM. Both midlife vascular risk factors and midlife cognitive function remained associated with later life MCI or DEM when both were in the model. Discussion: Later life MCI and DEM were independently associated with midlife vascular risk factors and midlife cognition. (C) 2018 the Alzheimer's Association. Published by Elsevier Inc. Summary: Among women with symptomatic uterovaginal prolapse, sacrospinous hysteropexy with graft resulted in a lower surgical failure rate compared to vaginal hysterectomy with uterosacral suspension. Obesity and duration of prolapse symptoms were identified as risk factors associated with surgical failure, and should be considered when determining treatment plans.

Summary: This study investigated the incidence and risk factors for pulmonary hypertension (PH) in patients with venous thromboembolism (VTE). The results showed that 3.5% of patients developed PH within 2 years after the initial VTE event. The risk was higher in women, older individuals, and those with additional comorbidities.

Racial Disparities in Hospitalization Among Patients Who Receive a Diagnosis of Acute Coronary Syndrome in the Emergency Department

Summary: This study investigated whether there are racial and ethnic differences in the risk of being discharged home among patients who received a diagnostic code of acute coronary syndrome (ACS) in the emergency department (ED). The results showed that Black and Hispanic patients had a higher risk of being discharged home compared to White patients, while Asian/Pacific Islander patients had a relatively lower risk. The association between race and ethnicity and discharge home was marginally mediated by health insurance status.

The results showed that there were differences in the risk of MCI/PD depending on the method used to ascertain AF. Therefore, the case definition of AF and quantification of AF burden are important factors in studying the link between AF and cognitive dysfunction.

Left Atrial Mechanical Dysfunction and the Risk for Ischemic Stroke in People Without Prevalent Atrial Fibrillation or Stroke: A Prospective Cohort Study

Summary: This study evaluated the association between left atrial function and size and ischemic stroke, and determined whether these measures could improve the accuracy of stroke prediction using CHA(2)DS(2)-VASc score variables. The results showed that left atrial size and function were associated with ischemic stroke, and the cumulative incidence rates of severe left atrial enlargement were 2.99%, 3.18%, and 1.99% for left atrial reservoir, conduit, and contractile strain, respectively. Adding left atrial function measures to CHA(2)DS(2)-VASc score variables improved stroke prediction accuracy.

Summary: The study aimed to determine the role of intraoperative data in predicting the outcomes of sacral neuromodulation for urgency urinary incontinence. The analysis of intraoperative details from the ROSETTA trial showed that intraoperative data had limited utility in predicting both acute and long-term outcomes. However, the position of the implant lead in relation to the trajectory of the sacral nerve root appeared to be important.

SGLT2 Inhibitors Are Associated With Reduced Cardiovascular Disease in Patients With Type 2 Diabetes: An Analysis of Real-World Data

Summary: The objective of this study was to assess the association between SGLT2 inhibitors and other second-line diabetes therapies with the risk of cardiovascular disease (CVD) and conduct head-to-head comparisons between SGLT2 inhibitors. Using data from the MarketScan databases, SGLT2 inhibitor users were matched with other second-line therapy users. The results showed that SGLT2 inhibitors were associated with a lower risk of CVD compared to other second-line therapies. No significant differences were found when comparing individual SGLT2 inhibitors.
