Cryptococcus gattii infections, especially including those with severe clinical manifestations, may be related to underlying host immunologic factors. We present 2 cases with autoantibodies to granulocyte-macrophage colony-stimulating factor (GM-CSF), a key cytokine in macrophage function. Immunologic evaluation for anti-GM-CSF antibodies may be important to inform management and counseling.

Host genetics of innate immune system in infection

Amy P. Hsu, Steven M. Holland

Summary: This article highlights the critical pathways of innate immunity, focusing on the latest genetic findings. It discusses the dominant and recessive mutations in known genes, expanded phenotypes associated with each gene, population-based variants in recognition and signaling, and the role of STAT3 signaling in specific phenotypes. The article also introduces novel roles for autophagy, actin cytoskeletal remodeling, and DNA replication. Overall, these findings demonstrate the rapid advancement of our knowledge in innate immunity and the need for further research.

Anticytokine autoantibodies: Autoimmunity trespassing on antimicrobial immunity

Aristine Cheng, Steven M. Holland

Summary: This article reviews the role of anticytokine autoantibodies in immunodeficiency and immune dysregulation, including HLA associations, disease associations, and treatment options. Suspecting the presence of these antibodies in patients and identifying them at the onset of symptoms can have a beneficial impact on disease improvement and outcomes.

Febrile neutropenia management and outcomes in hematopoietic cell transplantation for chronic granulomatous disease

Mark Parta, Jennifer Cuellar-Rodriguez, Juan Gea-Banacloche, Jing Qin, Corin Kelly, Christa S. Zerbe, Steven M. Holland, Harry L. Malech, Elizabeth M. Kang

Summary: We analyzed events and therapies related to febrile neutropenia in patients receiving hematopoietic cell transplantation (HCT) for chronic granulomatous disease (CGD). The study found that fever prior to neutropenia was common and some patients experienced Gram-negative bacterial infection before or during neutropenia. Empiric therapy with noncarbapenem antibiotics was commonly used, but carbapenems were also given in some cases. No deaths related to infection associated with neutropenia occurred.

Summary: Mycobacterium abscessus (Mab) infections pose a growing threat to patients, especially those with lung diseases and cystic fibrosis. Finding effective combinations of beta-lactam antibiotics and beta-lactamase inhibitors to disrupt cell wall synthesis in Mab is beneficial. The novel diazobicyclooctane beta-lactamase inhibitor durlobactam (DUR) enhances the susceptibilities of Mab isolates to beta-lactams in cell-based assays. DUR potently inhibits the beta-lactamase Bla(Mab) and forms stable acyl-enzyme complexes, providing a biochemical rationale for its activity.

Vaccine Protection of Mice With Primary Immunodeficiencies Against Disseminated Coccidioidomycosis

Daniel A. Powell, Amy P. Hsu, Christine D. Butkiewicz, Hien T. Trinh, Jeffrey A. Frelinger, Steven M. Holland, John N. Galgiani, Lisa F. Shubitz

Summary: This study investigates the use of vaccination in mice with immune deficiencies to mitigate disseminated coccidioidomycosis (DCM). The results show that vaccinated mice had significantly reduced fungal burdens in the lungs and spleen, and controlled spleen dissemination. These findings suggest that the Delta cps1 vaccine could be beneficial for individuals at high risk of immune deficiencies.

Summary: This study focuses on the association between the genetic locus encoding OAS1-OAS3 antiviral proteins and the severity of COVID-19. The analysis reveals that the risk of hospitalized COVID-19 is associated with a common OAS1 haplotype, which is also linked to reduced clearance of SARS-CoV-2. Further investigations indicate the functional contribution of specific OAS1 gene variants associated with the risk haplotype in protein abundance.
