Prescription duration and treatment episodes in oral glucocorticoid users: application of the parametric waiting time distribution

Purpose: Glucocorticoids are widely used medications. In many pharmacoepidemiological studies, duration of individual prescriptions and definition of treatment episodes are important issues. However, many data sources lack this information. We aimed to estimate duration of individual prescriptions for oral glucocorticoids and to describe continuous treatment episodes using the parametric waiting time distribution. Methods: We used Danish nationwide registries to identify all prescriptions for oral glucocorticoids during 1996-2014. We applied the parametric waiting time distribution to estimate duration of individual prescriptions each year by estimating the 80th, 90th, 95th and 99th percentiles for the interarrival distribution. These corresponded to the time since last prescription during which 80%, 90%, 95% and 99% of users presented a new prescription for redemption. We used the Kaplan-Meier survival function to estimate length of first continuous treatment episodes by assigning estimated prescription duration to each prescription and thereby create treatment episodes from overlapping prescriptions. Results: We identified 5,691,985 prescriptions issued to 854,429 individuals of whom 351,202 (41%) only redeemed 1 prescription in the whole study period. The 80th percentile for prescription duration ranged from 87 to 120 days, the 90th percentile from 116 to 150 days, the 95th percentile from 147 to 181 days, and the 99th percentile from 228 to 259 days during 1996-2014. Based on the 80th, 90th, 95th and 99th percentiles of prescription duration, the median length of continuous treatment was 113, 141, 170 and 243 days, respectively. Conclusion: Our method and results may provide an important framework for future pharmacoepidemiological studies. The choice of which percentile of the interarrival distribution to apply as prescription duration has an impact on the level of misclassification. Use of the 80th percentile provides a measure of drug exposure that is specific, while the 99th percentile provides a sensitive measure.

Maternal Smoking During Pregnancy and Risk of Appendicitis in the Offspring

Summary: This population-based cohort study in Denmark investigated the association between smoking during pregnancy and the risk of appendicitis in offspring. The Danish-American Research Exchange (DARE): a cross-sectional study of a binational research education program

Analysis of the research outcomes and intentions of DARE alumni suggests that this medical student-initiated research model has positive effects on promoting binational collaboration in medicine.

Root-cause Analysis of 762 Danish Post-colonoscopy Colorectal Cancer Patients

Summary: Earlier studies on lymphoma risk in patients with inflammatory bowel disease (IBD) have provided varying estimates, but have been limited by detection biases, misclassification, and small sample sizes. Our binational cohort study found that the risk of lymphoma has increased in Crohn's disease (CD) patients over the past 20 years, but not in ulcerative colitis (UC) patients. The increase in risk was mainly driven by T-cell lymphomas and aggressive B-cell lymphomas.

Summary: We investigated the relationship between hs-CRP and C-peptide in patients with recently diagnosed T2D and found that high levels of hs-CRP were associated with increased risk of CVEs and all-cause mortality. Patients with high levels of both biomarkers had the highest risk. These findings have important implications for early detection and prevention of deadly diseases beyond CVEs.

Comorbidity and risk of infection among patients with hip fracture: a Danish population-based cohort study

Summary: The impact of comorbidity on infection risk among hip fracture patients is not clear, but a high incidence of infection was found. Comorbidity was identified as an important risk factor for infection up to 1 year after surgery. Prenatal antidepressant exposure and emotional disorders until age 22: a danish register study

Summary: This study used Danish population data to examine the association between maternal use of selective serotonin reuptake inhibitors (SSRIs) during pregnancy and children outcomes. The results showed that children of mothers who used SSRIs during pregnancy had higher risk of depressive, anxiety, and adjustment disorders, and earlier age of onset of these disorders compared to children of mothers who did not use SSRIs or discontinued SSRIs use before conception. Paternal use of SSRIs in the absence of maternal use during pregnancy and maternal use only after pregnancy were also associated with these outcomes.

High- vs. low-dose diclofenac and cardiovascular risks: a target trial emulation

Summary: This study investigated the dose dependency of diclofenac's cardiovascular risks by using Danish health registries and a target trial emulation design. The results showed that both high-dose and low-dose diclofenac users had a similar increased risk of cardiovascular events compared to non-users. This finding challenges the assumption that low-dose diclofenac is risk-neutral.

Attitudes towards deprescribing in geriatric psychiatry: A survey among older psychiatric outpatients

Concerns about missing out on future benefits and previous negative experiences with deprescribing were also significant factors influencing patient attitudes.
