Summary: The study aimed to characterize the long-term risk of first recurrence of ACS among survivors of an incident ACS, based on the STEMI/NSTEMI/UA diagnosis. The risk of recurrence did not depend on the type of the incident ACS, but major complications and impaired LVEF increased the risk. The most frequent forms of recurrence were NSTEMI and UA.

Summary: This study aimed to assess lifetime morbidity and long-term survival in patients with bicuspid aortic valve (BAV) in the community. The results showed that BAV patients have a high lifetime morbidity burden, with a high incidence of complications such as aortic stenosis or regurgitation, aortic surgery, aortic aneurysm, infective endocarditis, and aortic dissection. Compared to patients with typical valvulo-aortopathy, patients with complex valvulo-aortopathy have lower survival rates.

Summary: This study investigated the clinical course and effectiveness of steroid-based intervention in patients with HHV6-positive lymphocytic myocarditis. It found that low levels of HHV6 DNA were frequently detected in the myocardium, and patients with low levels of HHV6 DNA showed spontaneous clinical improvement. In selected symptomatic patients with cardiac HHV6 DNA, steroid-based therapy was found to be effective and safe.

Summary: The National Heart, Lung, and Blood Institute (NHLBI) organized a workshop in May 2021 to discuss new research opportunities in the field of myocarditis. International experts reviewed the current scientific knowledge and identified gaps and opportunities in basic research, diagnostics, translational research, and therapeutic approaches for future studies on myocarditis. The workshop focused not only on community-acquired myocarditis but also on emerging causes such as immune checkpoint inhibitors and SARS-CoV-2-related myocardial injuries. The potential use of systems biology and artificial intelligence methods to discover new disease mechanisms and therapeutic targets was also considered. Another important aspect discussed was the investigation of the relationship between social determinants of health (SDoH), including race and economic status, and inflammatory response and outcomes in myocarditis. The result of the workshop was a proposal for the reclassification of myocarditis based on the latest knowledge of immunological pathogenesis to improve prognosis estimation and target-specific treatments.

Summary: In recent years, research has shown that myocardial inflammation (M-Infl) is involved in the pathophysiology and phenotypes of genetically inherited cardiomyopathies, such as dilated and arrhythmogenic cardiomyopathy. M-Infl, which resembles myocarditis, frequently occurs as a clinical manifestation of these cardiac diseases. The emerging role of M-Infl is leading to the discovery of potential targets for treating inflammation and paving the way for new approaches in cardiomyopathies.

Summary: This study aimed to evaluate the predictive value of the GRACE risk score in acute pulmonary embolism (PE) patients for 30-day mortality and the need for thrombolytic treatment. The study found that the GRACE risk score, along with the PESI score, were independent risk factors associated with 30-day mortality and the need for thrombolytic treatment. The GRACE risk score showed high sensitivity and specificity in predicting mortality, and its combination with the PESI score helped define high-risk PE patients and predict poor prognosis.

Summary: Transthoracic echocardiography (TTE) is commonly used to evaluate isolated tricuspid regurgitation (TR), but there is limited research comparing its quantification with magnetic resonance imaging (MRI). In this study, a novel TTE algorithm was developed to identify severe TR based on TTE parameters. The algorithm had a higher accuracy than the current guidelines' criteria in detecting severe TR by MRI.

Summary: This study evaluated the angiogenic capacity of adipose tissue in patients undergoing cardiac surgeries and found that patients with coronary artery disease (CAD) had reduced angiogenic capacity in their pericardial adipose tissue. Gene expression analysis revealed upregulated expression of the anti-angiogenic gene TSP-1 in CAD patients, with no significant differences in other angiogenic factors. Additionally, inhibiting TSP-1 expression significantly improved angiogenic deficiency in CAD patients.

Summary: The current guidelines recommend prophylactic implantable cardioverter-defibrillator (ICD) for patients with symptomatic heart failure (HF) and low left ventricular ejection fraction. However, not all patients will benefit from ICD treatment. This study evaluated the feasibility of using the MADIT-II-based Risk Stratification Score (MRSS) to predict the survival benefit of prophylactic ICDs in patients with HF. The results showed that different risk subgroups had varying levels of ICD survival benefit.

Summary: This study evaluated the rates and causes of 30-day readmissions after discharge for heart failure (HF) as a primary and secondary diagnosis. The findings indicate that efforts to reduce readmission rates should include patients with secondary HF diagnosis, and surveillance should extend to 2 weeks postdischarge to identify at-risk patients.

Summary: This study aimed to examine the relation between reverse septal curvature (RSC) and nonsustained ventricular tachycardia (NSVT) in patients with hypertrophic cardiomyopathy (HCM). The study found that patients with RSC were younger and had a higher incidence of NSVT, and RSC was identified as a strong independent risk factor for NSVT.
