Summary: This study aimed to investigate the short-, medium-, and long-term outcomes of intravenous thrombolysis in minor stroke patients. The results suggest that intravenous thrombolysis may be safe and effective in improving functional outcomes in minor stroke patients within a 4.5-h window. However, further randomized controlled trials are needed for validation. The functional recovery rate in the overall study population ranged from 12.8% to 18.85%.

Summary: There are controversies regarding the role of sex and gender in acute stroke, as assessing their individual roles is difficult due to their intertwined nature. Furthermore, differences in baseline function between women and men contribute to disparities in post-stroke outcomes. Understanding these sex and gender differences is crucial to provide optimal care for acute stroke patients. Ignoring these factors may lead to erroneous conclusions and suboptimal care.

Summary: This study aimed to determine the risks conferred by metabolic syndrome (MetS) and hyperglycemia to clinical outcomes in acute ischemic stroke patients treated with intravenous thrombolysis (IVT). The presence of MetS was found to be an independent risk factor for poor functional outcome (PFO) at 3 months in these patients. Moreover, the presence of hyperglycemia alone or concurrent with MetS was associated with an elevated risk of PFO after receiving IVT.

Summary: Patients treated with urokinase had similar outcomes but a higher risk of extracranial bleeding compared to patients treated with alteplase. The risk of extracranial bleeding was higher in the patients treated with high-dose urokinase than in the patients treated with low-dose urokinase. Patients who received low-dose urokinase had similar outcomes and complications compared to patients treated with alteplase.

Summary: In acute ischemic stroke patients treated with intravenous thrombolysis, those with prestroke disability did not have a significantly increased risk of symptomatic intracerebral hemorrhage or parenchymal hemorrhage. However, they were associated with a higher risk of early mortality compared to patients without prestroke disability.

Summary: A retrospective cohort study investigated the effect of acute alcohol consumption on outcomes in patients with acute ischemic stroke treated with intravenous thrombolysis (IVT). Elevated blood alcohol concentration was found to be a significant protective factor against unfavorable short-term and long-term outcomes. Patients with higher blood alcohol concentration showed lower NIHSS scores at 3 and 7 days after IVT.

Summary: This study aimed to develop a convenient nomogram for the bedside evaluation of stroke patients with intravenous thrombolysis, predicting neurological outcomes. Through retrospective evaluation of clinical characteristics of stroke patients and comparing predictive abilities using logistic regression models, a nomogram based on baseline NIHSS, blood sugar, blood cholesterol level, and part-and full anterior circulation infarction (OCSP type) was developed.

Summary: This study analyzed the impact of intravenous thrombolytic therapy (IVT) on functional outcome and secondary injury volumes in patients with complete reperfusion after thrombectomy. The results showed that bridging IVT was associated with a lower proportion of functional independence and a higher rate of symptomatic intracerebral hemorrhage (sICH). Multivariable analysis confirmed the independent association of bridging IVT with sICH and edema volume.

Summary: This study assessed the efficacy and adverse events of intravenous tirofiban before endovascular thrombectomy for acute ischemic stroke secondary to large vessel occlusion. The findings showed no significant difference in disability severity at 90 days between the tirofiban and placebo groups, suggesting that intravenous tirofiban may not be beneficial for acute ischemic stroke patients undergoing endovascular therapy.

Summary: This systematic review found that pre-hospital factors and in-hospital factors contribute to delays in thrombolysis administration, with long onset-to-needle times more common in certain patient groups. Various strategies exist to reduce delays, with processes integrating out-of-hospital and in-hospital processes showing the greatest improvements in time to thrombolysis.

Summary: This study evaluated the safety and short-term efficacy of intravenous thrombolysis (IVT) in acute ischemic stroke within an expanded time window delivered through a hub-and-spoke telestroke network. The results showed that IVT delivered through the telestroke network in the expanded time window was safe with equivalent short-term functional outcomes for patients admitted to spoke and hub centers.

Summary: This study aimed to evaluate the predictive value of V2 Doppler imaging for the detection of vertebrobasilar stenosis or occlusion. The results showed that V2 Doppler abnormalities predicted stenosis or occlusion in the vertebrobasilar artery with a sensitivity of 56.2% and specificity of 96.4%. Furthermore, abnormal Doppler results were associated with hypoplastic vertebral arteries and high-resistance flow.

Summary: Despite the use of antiplatelet agents, high-on-aspirin platelet reactivity (HAPR) can lead to recurrent ischemic stroke, especially in cases of large-artery atherosclerosis. HAPR is more commonly associated with stroke due to extracranial atherosclerosis and inadequate antiplatelet inhibition. Artery-to-artery embolism is independently associated with HAPR in both types of stroke.
