Objective: The role of testosterone (T) in regulating body composition is conflicting. Thus, our goal is to meta-analyse the effects of T supplementation (TS) on body composition and metabolic outcomes. Methods: All randomized controlled trials (RCTs) comparing the effect of TS on different endpoints were considered. Results: Overall, 59 trials were included in the study enrolling 3029 and 2049 patients in TS and control groups respectively. TS was associated with any significant modification in body weight, waist circumference and BMI. Conversely, TS was associated with a significant reduction in fat and with an increase in lean mass as well as with a reduction of fasting glycaemia and insulin resistance. The effect on fasting glycaemia was even higher in younger individuals and in those with metabolic diseases. When only RCTs enrolling hypogonadal (total T < 12 mol/l) subjects were considered, a reduction of total cholesterol as well as triglyceride (TGs) levels were also detected. Conversely, an improvement in HDL cholesterol levels as well as in both systolic and diastolic blood pressure was not observed. Conclusion: Our data suggest that TS is able to improve body composition and glycometabolic profile particularly in younger subjects and in those with metabolic disturbances. Specifically designed studies are urgently needed to confirm this point.

Cluster Analysis Method Reveals Gender Attitudes in Sociosexual Orientation of a Southern Italy Population During the COVID-19 Lockdown

Maria Grazia Vaccaro, Giulia Izzo, Alessia Sarica, Sandro La Vignera, Antonio Aversa

Summary: This study aims to analyze the different perceptions of sociosexual experiences among the southern population during the COVID-19 pandemic. Through cluster analysis, we identified eleven clusters and found intra- and inter-diagnostic heterogeneity in the sexual profile of participants. We also discovered significant gender differences, with women showing a greater propensity for positive sexual attitudes and desires compared to men.

Effects of benzo[a]pyrene on the reproductive axis: Impairment of kisspeptin signaling in human gonadotropin-releasing hormone primary neurons

Summary: The neuroendocrine control of reproduction is regulated by the pulsatile release of GnRH by hypothalamic neurons. BaP, a pollutant, can disrupt endocrine function and affect gonadal function and gamete maturation. This study found that BaP alters the phenotype and function of GnRH neurons by interfering with KISS1R signaling and GnRH secretion, highlighting its importance in the central neuroendocrine control of reproduction.

Summary: A study found that male cannabis users are younger, have fewer comorbidities, and have better sexual performance compared to non-users. Cannabis use is associated with a more unstable relationship and a higher frequency of masturbation. Cannabis use may improve anthropometric and lipid profiles as well as penile arterial response in patients with sexual dysfunction.

The Variant p.Ala84Pro Is Causative of X-Linked Hypophosphatemic Rickets: Possible Relationship with Burosumab Swinging Response in Adults

Summary: Loss of function mutations in the PHEX gene can lead to X-linked dominant hypophosphatemia, the most common form of genetic rickets. This condition is characterized by renal phosphate wasting, increased fibroblast growth factor 23 (FGF-23), growth retardation, bone deformities, and musculoskeletal manifestations. Despite the discovery of numerous mutations in the PHEX gene, there are currently no clear genotype-phenotype correlations in patients with hypophosphatemic rickets (XLH).

Hyperprolactinemia and male sexual function: focus on erectile dysfunction and sexual desire

G. Corona, G. Rastrelli, N. Bianchi, C. Sparano, A. Sforza, L. Vignozzi, M. Maggi

Summary: The present paper aims to analyze and discuss the relationship between male sexual function and elevated prolactin (PRL) levels (HPRL) using clinical data and meta-analytic approach. 4.2% of patients seeking medical care for sexual dysfunction showed PRL levels above the normal range. HPRL was found to have a negative influence on male sexual desire. Treating HPRL can improve libido, but its effect on erection is limited. The role of HPRL in erectile dysfunction remains inconclusive. Overall, the paper provides important evidence on the relationship between PRL levels and male sexual function.

Summary: This study collected evidence on the risk of influenza complications in people with diabetes and the effectiveness of influenza vaccines in these individuals. The results showed that diabetes increases the risk of severe complications from influenza, and influenza vaccination has a significant protective effect for patients with diabetes.

Livio Tarchi, Giuseppe Pierpaolo Merola, Ottone Baccaredda-Boy, Francesca Arganini, Emanuele Cassioli, Eleonora Rossi, Mario Maggi, David S. Baldwin, Valdo Ricca, Giovanni Castellini

Summary: The purpose of this systematic review was to investigate the adverse effects of SSRIs on sexual function and determine if these effects persist after discontinuation. The review found limited evidence of sexual dysfunction following SSRI discontinuation and was unable to establish a cause-effect relationship. However, it highlighted the need for further research to explore the relationship between SSRI exposure and persistent sexual dysfunction and to develop new treatment options.

Impact of Preoperative Immunonutrition on Oxidative Stress and Gut Barrier Function in Surgical Patients with Crohn's Disease

Summary: International guidelines recommend peri-operative immunonutrition (IN) support for colorectal surgery patients to reduce postoperative complications. This pilot study on Crohn's patients undergoing intestinal surgery found that IN patients had lower plasma levels of AGEs and higher mucosal TJP1 expression, suggesting that IN may improve intestinal mucosa barrier function through the regulation of AGEs/RAGE signaling. The study also found positive correlations between plasma and tissue levels of AGEs and AOPPs, as well as negative correlations between plasma AGEs and TJP1 expression, and between RAGE expression and TJP1 gene expression.

Summary: Red wine is rich in nutrients and has been the focus of numerous scientific studies. It has been reported that moderate consumption of red wine, with its phenolic and antioxidant properties, can have positive effects on various diseases. Additionally, the ethanol content in red wine has shown various biological properties. However, little is known about the correlation between red wine consumption and male sexual function. This review aimed to evaluate the effects of moderate red wine consumption on erectile function and found potential benefits in patients with erectile dysfunction as well as positive influences on reproductive function through vasorelaxant and antioxidant mechanisms.

Summary: Sperm cryopreservation is recommended for male fertility preservation in cancer patients or other medical conditions. The recovery rates of motility and viability vary depending on the medical condition. Our study showed that lower sperm quality led to lower recovery rates, and donor samples had the highest recovery rates. Cut-off values for motility recovery were established based on initial semen quality. Overall, our results emphasize the importance of considering intrinsic sperm characteristics in cryopreservation.
