Summary: Fournier gangrene is a rapidly progressing necrotizing fasciitis with a high mortality rate, commonly affecting the perineal, perianal, and genital regions. While clinical diagnosis is usually sufficient, radiological diagnostics can help determine the extent of the disease. This article presents a case study of Fournier gangrene diagnosed by MRI, demonstrating successful treatment outcome through early intervention and surgical treatment.

Summary: The purpose of this study was to investigate health care utilization patterns and identify risk factors associated with potential diagnostic delay leading up to the diagnosis of necrotizing soft tissue infections of the genitalia. The study found that nearly 50% of insured patients who undergo surgery for or die from this condition will present to a medical provider with symptomatically similar diagnoses suggestive of early disease development, and many of these visits likely represent diagnostic delay.

Summary: In this retrospective study, we analyzed the clinical data of 36 patients with FG and evaluated the effects of NPWT. Perianal and external genital infections were the most common types of FG, with Escherichia coli, Enterococcus, and Staphylococcus haemolyticus being the most common pathogens. Patients treated with NPWT had shorter hospital stays, fewer operations, and faster wound healing compared to those treated with conventional dressings. Therefore, clinicians should consider the possibility of FG in patients with diabetes and NPWT can be an effective adjuvant therapy for managing FG wounds.

Summary: In a prospective study of patients with Fournier's gangrene, high-risk subgroups were identified, with those experiencing adjacent organ involvement more likely to have gastrointestinal sources of infection, require mechanical ventilation, and have longer hospital stays. Overall, FG was associated with significant perioperative morbidity.

Summary: Patients with hematological malignancies are at higher risk of Fournier's gangrene (FG) due to immunosuppression, and surgical treatment, vacuum-assisted closure, and hyperbaric oxygen therapy can contribute to recovery in these patients.

Summary: The study suggests that in the treatment of Fournier's gangrene, skin parts without necrosis can be spared during debridement to facilitate easier wound closure without compromising surgical safety. This approach may help maintain the safety of the procedure while allowing for easier wound closure.

Summary: The study found that the use of diversional stoma in Fournier's gangrene (FG) is associated with poorer outcomes, with significantly higher mortality rate and more surgical procedures performed in patients who required a stoma.

Summary: We report a case of Fournier's gangrene caused by Actinomyces europaeus in a 61-year-old female with type 2 diabetes. A. europaeus is associated with abscesses, decubitus ulcers, and purulent urethritis. This is the first case report of FG caused by A. europaeus mono-infection, suggesting its potential to cause necrotizing fasciitis.

Summary: This prospective study aimed to evaluate the role of colostomy in managing Fournier gangrene affecting the perianal region. The results showed that patients in the colostomy group had fewer debridements, shorter hospital stay, and faster wound healing compared to the non-colostomy group. These patients also had lower rates of wound dehiscence, infection, and flap ischemia, although the difference was not statistically significant.

Summary: This study retrospectively analyzed the effect of early surgery combined with empiric antimicrobials on mortality in necrotizing soft tissue infections (NSTIs). The results showed that early surgery independently reduced 30-day mortality, and the choice of empiric antimicrobials did not further enhance this effect.

Summary: This study aimed to understand the current practice patterns of Fournier's gangrene (FG) in Europe and identify the implications for planning a prospective FG registry. The results showed that there is no standard of care for FG throughout Europe, highlighting the need for guidelines and increased disease awareness. Participants emphasized the importance of establishing an international registry and developing predictive models.

Summary: Fournier's gangrene, a necrotizing fasciitis affecting the scrotal and inguinal region, presents high mortality and complication rates. Early diagnosis, prompt surgical intervention and broad-spectrum antibiotic therapy are essential for preventing complications in these cases.

Summary: Fournier's gangrene is a rare and potentially fatal condition characterized by a necrotizing soft-tissue infection affecting the external genitalia and perineum. Effective management is crucial due to the possibility of life-threatening complications. This retrospective study aimed to fill knowledge gaps, compare reconstructive options, and encourage reflection on current management. The findings suggest that most defects can be repaired using split skin grafts, while full-thickness skin grafts should be used whenever possible to prevent secondary contracture and the need for revision surgery.
