Summary: This study found that deletion of the PITX2 gene in human induced pluripotent stem cell-derived atrial cardiomyocytes results in some abnormalities similar to electrical remodeling in atrial fibrillation. These findings suggest that the PITX2 gene plays an important role in the occurrence of atrial fibrillation.

Summary: The EAST-AFNET 4 trial found that early rhythm control (ERC) reduces the rate of a composite primary outcome by around 20%. Summary: This study aims to investigate whether prophylactic administration of atorvastatin reduces postoperative atrial fibrillation (POAF) in patients undergoing surgical aortic valve replacement (SAVR). The study is a randomized, double-blind, placebo-controlled trial involving 266 patients without a history of AF or statin use. The primary endpoint is the occurrence of POAF, documented by rhythm strip or 12-lead ECG.

Prevalence and Prognostic Significance of Bradyarrhythmias in Patients Screened for Atrial Fibrillation vs Usual Care Post Hoc Analysis of the LOOP Randomized Clinical Trial

Summary: There is a growing interest in heart rhythm monitoring and the detection of subclinical atrial fibrillation (AF) which may identify cases of bradyarrhythmias. This study aimed to assess the prevalence and prognostic significance of bradyarrhythmias in individuals screened for AF using an implantable loop recorder (ILR). The results showed that more than 1 in 5 individuals over the age of 70 with cardiovascular risk factors can be diagnosed with bradyarrhythmias when long-term continuous monitoring for AF is applied. ILR screening led to a significant increase in the diagnosis of bradyarrhythmias and pacemaker implantations compared with usual care, but there was no change in the risk of syncope or sudden death.

Summary: This study compared the acute lesion extent after pulmonary vein isolation (PVI) induced by pulsed-field ablation (PFA) and multielectrode radiofrequency balloon (RFB). The results showed that PFA resulted in larger ablation areas and higher release of cardiac injury biomarkers compared to RFB.

Localized cardiomyocyte lipid accumulation is associated with slowed epicardial conduction in rats

Summary: This study demonstrates that the transmural conduction gradient in rat ventricular slices is abolished when there is increased pericardial adiposity, with slowed epicardial conduction correlating with an increase in cardiomyocyte lipid content. This gradient is important in reducing vulnerability to arrhythmias, and its absence due to conduction slowing and cardiomyocyte lipid accumulation suggests a novel mechanism by which pericardial adiposity may exacerbate ventricular arrhythmias.

Summary: Long-chain acyl-carnitines, specifically C18:1AC, have been found to be potential arrhythmogenic metabolites. This metabolite has biphasic effects on cardiac electrophysiology and metabolism, and has the potential to serve as a circulating biomarker for predicting risk of atrial fibrillation.

Summary: This study externally validated the ESUS recurrence score and supported a threshold of >= 7 for identifying ESUS patients at high-risk of stroke recurrence. However, further adjustments may be needed to improve the model's performance in independent cohorts. The use of risk scores may be helpful in guiding extended diagnostics and further trials on secondary prevention in patients with ESUS.
