The rise and fall of pneumococcal serotypes carried in the PCV era

Streptococcus pneumoniae is a major cause of meningitis, sepsis and pneumonia worldwide. Vaccination using pneumococcal conjugate vaccines (PCV) has therefore been part of the UK's childhood immunisation programme since 2006. Here we describe pneumococcal carriage rates in children under five years of age attending the paediatric department of a large UK hospital in response to vaccine implementation over seven winter seasons from 2006 to 2013. S. pneumoniae (n = 696) were isolated from nasopharyngeal swabs (n = 2267) collected during seven consecutive winters, October to March, 2006/7 to 2012/13. This includes the period immediately following the introduction of the seven-valent pneumococcal conjugate vaccine (PCV7) in 2006 in addition to pre- and post-PCV13 introduction in 2010. We show a decrease in PCV13 vaccine serotypes (VT) in the three years following PCV13 vaccine implementation (2010/11 to 2012/13). Serotype 6A represented the only observed VT following PCV13 implementation with all others (including PCV7 serotypes) absent from carriage. Overall pneumococcal carriage, attributable to non-VT (NVT), was consistent across all sampling years with a mean of 314%. The ten most frequently isolated NVTs were 6C, 11A, 15B, 23B, 15A, 21, 22F, 35F, 23A and 15C. Fluctuations in the prevalence of each were however noted. Comparing prevalence at 2006/07 with 2012/13 only 15A was shown to have increased significantly (p value of 0.003) during the course of PCV implementation. These data support the increasing evidence that the primary effect of PCVs is due to population immunity by reducing or eliminating the carriage of invasive VT serotypes. With IPD being increasingly attributed to non-vaccine serotypes, surveillance of carriage data continues to act as an early warning system for vaccine design and public health policy that require continual data of both carried pneumococcal serotypes and IPD attributed serotype data. Psychological distress experienced by parents caring for an immunosuppressed child during the COVID-19 pandemic

Summary: This longitudinal study found that 12% of British parents caring for an immunosuppressed child experienced high levels of concern throughout the first 2 years of the COVID-19 pandemic, which was related to their mental health problems. Single parenting, low household income, difficult access to greenspace, and higher exposure to COVID positive cases and restrictions in the North of England were associated with emotional mental health problems among these parents. Optimism, reduction of isolation, and support promoted coping and management of the challenges posed by the pandemic.

Summary: Over 1,000 cases of unexplained paediatric hepatitis have been reported worldwide, with 278 cases in the UK. A study investigated 38 cases and found high levels of adeno-associated virus 2 (AAV2) in the liver, blood, plasma or stool of 27 out of 28 cases. The study suggests that abnormal AAV2 replication aided by other viruses may trigger immune-mediated liver disease in predisposed children.

Comparative genomic epidemiology of serotype 3 IPD and carriage isolates from Southampton, UK between 2005 and 2017

Summary: Despite its inclusion in PCV13, serotype 3 pneumococci remains a significant cause of disease. Recent studies have identified three clades within the population structure, with Clade II being more antibiotic-resistant. A genomic analysis of serotype 3 isolates in Southampton, UK showed that Clade I alpha CC180 GPSC12 is the dominant cause of both carriage and invasive disease.

Oral and gut microbial biomarkers of susceptibility to respiratory tract infection in adults: A feasibility study

Summary: We conducted a feasibility cohort study to collect oral and gut samples from adult participants at three time points - baseline, during a respiratory infection, and post-infection. Our aim was to identify microbial biomarkers of susceptibility to respiratory infections in healthy participants. The analysis showed that participants with respiratory infection symptoms had higher levels of Streptococcus and Megamonas in their oral samples, while Lactobacillus, Synergistetes, Verrucomicrobia, and Dethiosulfovibrio were depleted. In the gut samples, Veillonella, Rikenellaceae, Enhydobacteria, Eggerthella, and Xanthomonsdales were enhanced, while Desulfobulbus and Coprobacillus were depleted. This study demonstrated the feasibility of collecting multiple biological samples for microbial profiling and identified potential microbial biomarkers of respiratory infection susceptibility.

Diagnosing acute bone and joint infection in children

Summary: Acute bone and joint infections in children are serious and often misdiagnosed. Clinicians face difficulty in differentiating between transient synovitis and bone/joint infection. Existing decision support tools lack methodological expertise and do not consider the importance of imaging. A UK multicentre study aims to integrate imaging into a new decision support tool with the help of experts, addressing the need for evidence-based guidelines.

Lactoferrin modulates the biofilm formation and bap gene expression of methicillin-resistant Staphylococcus epidermidis

Summary: Lactoferrin, an innate glycoprotein, has broad antibacterial and antibiofilm properties. This study investigated the ability of lactoferrin to prevent biomass formation and eradicate preformed biofilms of a methicillin-resistant Staphylococcus epidermidis (MRSE) strain. It also examined lactoferrin's modulation of the expression of the biofilm-associated protein gene (bap), which is responsible for bacterial adhesion and aggregation. In vitro biofilm assays showed that lactoferrin prevented biofilm formation and eradicated established biofilms for up to 24 and 72 hours, respectively. Extensive eradication of MRSE biofilm biomass was accompanied by significant upregulation of bap gene expression.
