Characteristics of adult intestinal failure centers: An international multicenter survey

Narisorn Lakananurak, Lisa Moccia, Elizabeth Wall, Jean Herlitz, Hilary Catron, Edward Lozano, Adela Delgado, Tim Vanuytsel, David Mercer, Sophie Pevny, Mark Berner-Hansen, Leah Gramlich

Summary: This study examined the characteristics of adult chronic intestinal failure (CIF) centers and their patients globally. The results showed that most centers manage around 100 CIF patients, and there were variations in the composition of multidisciplinary teams (MDT).

Summary: This study retrospectively analyzed single-center bilateral lung transplantations performed between 2010 and 2020. The results showed that lung transplantation from donors aged ≥70 years is feasible and safe, with comparable short and long-term outcomes to transplantation from younger donors.

Protective Effect of Oxygen and Isoflurane in Rodent Model of Intestinal Ischemia-Reperfusion Injury

Mathias Clarysse, Alison Accarie, Ricard Farre, Emilio Canovai, Diethard Monbaliu, Jan Gunst, Gert De Hertogh, Tim Vanuytsel, Jacques Pirenne, Laurens J. J. Ceulemans

Summary: Animal research on intestinal ischemia-reperfusion injury (IRI) is mainly done using rodent models. Previously, intraperitoneal injections with ketamine-xylazine mixtures were common, but now volatile anesthetics like isoflurane are more commonly used. However, the impact of the anesthetic method on intestinal IRI has not been studied. This study aims to analyze the different anesthetic methods and their influence on the extent of intestinal IRI in a rat model.

The Role of a Colon-in-Continuity in Short Bowel Syndrome

Astrid Verbiest, Palle Bekker Jeppesen, Francisca Joly, Tim Vanuytsel

Summary: Short bowel syndrome (SBS) is a rare gastrointestinal condition caused by extensive surgical resection, resulting in less than 200 cm of remaining small intestine. This condition is associated with a high risk for intestinal failure (IF) and requires parenteral support (PS). This review focuses on the role of the colon in SBS-IF, including intestinal adaptation, energy salvage, and the impact of the microbiota. Additionally, the effects of glucagon-like peptide-2 (GLP-2) analogs, a disease-modifying treatment, in SBS-IF patients with and without a colon are discussed. Overall, this review emphasizes the importance of preserving the colon in SBS-IF.

Review article: Functional dyspepsia-a gastric disorder, a duodenal disorder or a combination of both?

B. W. L. C. M. Broeders, F. Carbone, L. M. Balsiger, J. Schol, K. Raymenants, I. Huang, A. Verheyden, T. Vanuytsel, J. Tack

Summary: This study provides a comprehensive overview of the pathophysiology of functional dyspepsia (FD), focusing on the shift towards duodenal mechanisms. Recent studies have shown that duodenal factors such as acid, bile salt exposure, eosinophil and mast cell activation are associated with symptoms and can be related to gastric sensorimotor dysfunction. Future research should focus on inhibiting duodenal mast cell activation, eosinophilia, and loss of mucosal integrity for the treatment of FD.

Neuromodulating agents in functional dyspepsia: a comprehensive review

L. Bosman, L. Wauters, T. Vanuytsel

Summary: Functional dyspepsia is a common chronic condition characterized by upper abdominal symptoms in the absence of organic cause. First line treatments such as proton-pump inhibition or Helicobacter pylori eradication often do not provide enough symptom relief. This study reviewed randomized controlled trials and found that tricyclic antidepressants (TCAs) have been shown to be effective in treating functional dyspepsia, while other neuromodulating agents like tetracyclic antidepressants, levosulpiride, and anxiolytics may be beneficial but lack conclusive evidence. Serotonin and noradrenaline reuptake inhibitors (SNRIs) and selective serotonin reuptake inhibitors (SSRIs) were not found to be beneficial.

Identification of regional variation in gene expression and inflammatory proteins in donor lung tissue and ex vivo lung perfusate

Bonnie T. Chao, Andrew T. Sage, Jonathan C. Yeung, Xiaohui Bai, Jin Ma, Tereza Martinu, Mingyao Liu, Marcelo Cypel, Dirk Van Raemdonck, Laurens J. Ceulemans, Arne Neyrinck, Stijn Verleden, Shaf Keshavjee

Summary: This study investigated lung inflammatory biomarkers using human lung tissue biopsies and ex vivo lung perfusion perfusate. The findings suggest that lung biopsies are consistent in gene expression and reflect the entire lung, except when collected from the lingula or an area of focal injury. Additionally, ex vivo lung perfusion perfusate provides a representative measurement of lung inflammation. These results are important for clinicians in selecting donor lungs.

Aspergillus-Specific IgG Antibodies are Associated With Fungal-Related Complications and Chronic Lung Allograft Dysfunction After Lung Transplantation

Summary: Fungal exposure and sensitization negatively impact respiratory diseases, including lung transplant recipients. This study examines the correlation between fungal-specific antibodies, fungal isolation, chronic lung allograft dysfunction (CLAD), and overall survival in 311 lung transplant patients. The results show that elevated levels of Aspergillus fumigatus or Aspergillus flavus IgG are associated with more fungal isolation and CLAD. However, IgE levels and fungal isolation are not correlated with CLAD or death. Mold and Aspergillus species isolation are associated with CLAD occurrence, while Aspergillus species isolation is also linked to impaired survival. Fungus-specific IgG could serve as a non-invasive marker for fungal exposure and a diagnostic tool for identifying high-risk patients in long-term follow-up post-lung transplant.

Summary: This study describes the outcomes of lung transplantation in the largest population of PCD patients, including those with situs abnormalities. It found that lung transplantation is an acceptable treatment option for PCD, with no significant differences in survival or freedom from chronic lung allograft dysfunction between PCD patients with and without situs abnormalities.
