Levosimendan for Hemodynamic Support after Cardiac Surgery

BACKGROUND Acute left ventricular dysfunction is a major complication of cardiac surgery and is associated with increased mortality. Meta-analyses of small trials suggest that levosimendan may result in a higher rate of survival among patients undergoing cardiac surgery. METHODS We conducted a multicenter, randomized, double-blind, placebo-controlled trial involving patients in whom perioperative hemodynamic support was indicated after cardiac surgery, according to prespecified criteria. Patients were randomly assigned to receive levosimendan (in a continuous infusion at a dose of 0.025 to 0.2 mu g per kilogram of body weight per minute) or placebo, for up to 48 hours or until discharge from the intensive care unit (ICU), in addition to standard care. The primary outcome was 30-day mortality. RESULTS The trial was stopped for futility after 506 patients were enrolled. A total of 248 patients were assigned to receive levosimendan and 258 to receive placebo. There was no significant difference in 30-day mortality between the levosimendan group and the placebo group (32 patients [12.9%] and 33 patients [12.8%], respectively; absolute risk difference, 0.1 percentage points; 95% confidence interval [CI], -5.7 to 5.9; P = 0.97). There were no significant differences between the levosimendan group and the placebo group in the durations of mechanical ventilation (median, 19 hours and 21 hours, respectively; median difference, -2 hours; 95% CI, -5 to 1; P = 0.48), ICU stay (median, 72 hours and 84 hours, respectively; median difference, -12 hours; 95% CI, -21 to 2; P = 0.09), and hospital stay (median, 14 days and 14 days, respectively; median difference, 0 days; 95% CI, -1 to 2; P = 0.39). There was no significant difference between the levosimendan group and the placebo group in rates of hypotension or cardiac arrhythmias. CONCLUSIONS In patients who required perioperative hemodynamic support after cardiac surgery, low-dose levosimendan in addition to standard care did not result in lower 30-day mortality than placebo. Increased duration and similar outcomes of V-V ECLS in patients with COVID-19 ARDS compared to non-COVID ARDS: Single center experience

Summary: This study analyzed the demographic features, clinical course, and outcomes of COVID-19 patients treated with V-V ECLS during the Italian 'third wave'. The unadjusted mortality rate of COVID-19 ARDS patients receiving V-V ECLS was similar to pre-pandemic cases, but the duration of ECLS was significantly longer and complications were frequent. This may be attributed to the selection of critically ill patients at baseline who developed multiorgan failure during the course of ECLS.

Summary: This study aimed to evaluate the diagnostic value of optical coherence tomography (OCT) for pulmonary microvascular thromboinflammatory syndrome in patients with COVID-19 pneumonia. The results showed that OCT assessment of the distal pulmonary arteries in hospitalized COVID-19 patients is safe and accurate, enabling the first in vivo documentation of distal pulmonary arterial thrombosis in patients with elevated thromboinflammatory markers, even when their CT angiogram was negative for pulmonary thrombosis.

Left ventricular assist devices promote changes in the expression levels of platelet microRNAs

Summary: This study found significant differences in the expression levels of platelet miR-126, miR-374b, miR-223, and miR-320a between LVAD patients and healthy controls. The expression levels of platelet miR-25, miR-144, miR-320, and miR-451a changed significantly during LVAD support, and these miRs were implicated in cardiac- and coagulation-associated pathways. Patients who experienced bleeding had significantly higher expression levels of platelet miR-151a and miR-454 before LVAD implantation, and these miRs were also differentially expressed before the clinical manifestation of bleeding events. This study provides evidence of significant modulation of platelet miRs expression driven by LVADs and suggests the potential predictive value of platelet miRs in bleeding events.

Summary: This study aims to investigate the efficacy of high dose esomeprazole in reducing organ dysfunction in patients with sepsis or septic shock. Through a randomized trial, the study will compare the mean daily Sequential Organ Failure Assessment (SOFA) score over 10 days between the high dose esomeprazole group and the placebo group, as well as secondary outcomes such as antibiotic-free days, single organ failure severity, intensive care unit-free days at day 28, and mortality. The results of this study are expected to provide new insights for the treatment of sepsis or septic shock.

Del Nido Cardioplegia in Adult Cardiac Surgery: Meta-Analysis of Randomized Clinical Trials

Summary: This systematic review and meta-analysis compares the outcomes of patients receiving del Nido solution with other types of cardioplegia in cardiac surgery. The results indicate that del Nido cardioplegia is associated with a lower rate of postoperative stroke and/or transient ischemic attack. It also reduces aortic cross-clamp time, the need for defibrillation, and the risk of postoperative acute kidney injury, without affecting mortality.

Propofol and survival: an updated meta-analysis of randomized clinical trials

Summary: This study conducted a systematic review and meta-analysis, and found that propofol may reduce survival in perioperative and critically ill patients compared to other hypnotic agents. Therefore, a careful assessment of the risk versus benefit of propofol compared to other agents is needed when planning for large, pragmatic multicentric randomized controlled trials to provide a definitive answer.
