Cognitive and behavioral disorders in Parkinson's disease: an update. I: cognitive impairments

Parkinson's disease (PD) is a progressive neurodegenerative disease characterized by motor symptoms such as rigidity, rest tremor, and bradykinesia. However, a growing body of evidence demonstrated that PD encompasses several non-motor disturbances as well, such as cognitive impairment. Cognitive defects can be present since early stages of the disease but tend to dominate the clinical picture as the disease progresses. Around 40% of patients with PD present with cognitive impairments in several cognitive domains including attention, working memory and executive functions, language, visuospatial skills, and episodic memory; in later stages of the disease, cognitive defects and associated behavioral disorders concur to determine clinically relevant PD-associated dementia. Part of these defects is ascribed to a dopamine-dependent dysfunction of fronto-striatal pathways, but there is a considerable heterogeneity in the cognitive impairments as well as a suggestion of the role of other neurotransmitter systems, such as the cholinergic one, mainly responsible for Parkinson-dementia syndrome. In this paper, we review recent literature with particular attention to the last 5 years on the main cognitive deficits described in PD patients as well as on the hypothesized neuro-functional substrate of such impairments. Finally, we provide some suggestions on how to test cognitive functions in PD appropriately.

Standardization and normative data for a new test of visual long-term recognition memory

Summary: This study introduced a new test for visual recognition memory, the MTCF-RT, to assess complex figure recognition in the Italian population. Normative data from 280 healthy Italian speakers aged 18 to 89 showed that age and education significantly predicted performance on the test through multiple regression analysis.

Empathy through the Pandemic: Changes of Different Emphatic Dimensions during the COVID-19 Outbreak

Summary: The COVID-19 pandemic has had varying effects on different dimensions of empathy. Empathic social skills have been negatively impacted, while cognitive and emotional empathy have significantly improved. This suggests that psychological interventions should focus on enhancing empathic social skills to help individuals cope with the mental health challenges associated with the pandemic.

Deep Brain Stimulation in Parkinson Disease: A Meta-analysis of the Long-term Neuropsychological Outcomes

Summary: This systematic review and meta-analysis examined the long-term effects of deep brain stimulation (DBS) on cognitive functions in patients with Parkinson's disease. The results showed that DBS can lead to both positive effects, such as a mild improvement in anxiety and depression, and negative effects, including a decline in long-term memory, verbal fluency, and specific subdomains of executive functions.

Cognitive reserve and emotion recognition in the context of normal aging

Summary: The Cognitive Reserve (CR) hypothesis explains individual differences in vulnerability to age- or pathological-related brain changes. Lifetime influences, such as education, are believed to enhance cognitive processing in later life. This study found that while CR proxies can predict cognitive performance in older adults, they do not attenuate age-related decline in social cognitive tasks.

The role of verbal short-term memory in complex sentence comprehension: An observational study on aphasia

Summary: By studying the sentence comprehension ability of individuals with aphasia and the role of phonological short-term memory (pSTM), it was found that there is a significant interaction between sentence type and pSTM. Participants with lower pSTM scores showed reduced comprehension of center-embedded object relatives and long coordinated sentences. There was also a significant interaction between sentence type and agrammatism, with a lower performance for passives in the agrammatic group.

Predicting Long-Term Recovery of Consciousness in Prolonged Disorders of Consciousness Based on Coma Recovery Scale-Revised Subscores: Validation of a Machine Learning-Based Prognostic Index

Summary: This study compared the prognostic accuracy of Clinical Diagnosis and Coma Recovery Scale-Revised (CRS-R) total score with that of a novel Consciousness Domain Index (CDI) for recovery of full consciousness in patients with prolonged Disorders of Consciousness (pDoC). The CDI, obtained by combining the CRS-R subscales using unsupervised machine learning, showed the highest prediction accuracy and sensitivity.

Long-term clinical evolution of patients with prolonged disorders of consciousness due to severe anoxic brain injury: A meta-analytic study

Summary: This study assessed the long-term outcome and predictive value of demographic and clinical information for patients with prolonged disorders of consciousness caused by anoxic brain injury. The results showed that the mortality rate was 26%, the rate of any clinical improvement was 26%, and the rate of full recovery of consciousness was 17%. Younger age, a baseline diagnosis of minimally conscious state, higher Coma Recovery Scale Revised total score, and earlier admission to intensive rehabilitation units were associated with a higher likelihood of survival, clinical improvement, and recovery of full consciousness.

Deficits in naming pictures of objects are associated with glioma infiltration of the inferior longitudinal fasciculus: A study with diffusion MRI tractography, volumetric MRI, and neuropsychology

Summary: The study aimed to prove that left ILF damage would cause selective impairment in object naming, while patients with glioma infiltrating the ATL would not be impaired due to functional reorganization of the lexical retrieval network elicited by the tumor. Evaluation was conducted on 48 right-handed patients with neuropsychological testing and MRI before and after surgery. Damage to ILF was significantly associated with naming deficit when considering all the fascicles together. Postoperative neuropsychological evaluation showed that naming scores of patients with tumor infiltration of the ATL were not significantly associated with the percentage of ILF damage.

The FACE test: a new neuropsychological task to assess the recognition of complex mental states from faces

Summary: This study introduces a new task, called the FACE test, to assess the recognition of complex mental states from faces. Normative data for the Italian population and its clinical application to 40 Parkinson's disease (PD) patients are reported. The FACE test was found to be accurate in distinguishing PD patients with socio-cognitive dysfunctions from both controls and PD patients without emotion recognition/attribution difficulties.

Which components of famous people recognition are lateralized? A study of face, voice and name recognition disorders in patients with neoplastic or degenerative damage of the right or left anterior temporal lobes

Summary: We administered a recognition battery to patients with neoplastic or degenerative damage affecting the right or left ATL. The results showed that patients with right ATL lesions had greater impairments in non-verbal recognition (face and voice), while patients with left ATL lesions had greater impairments in name familiarity. Additionally, results showed that patients with right ATL lesions exhibited a higher degree of right lateralization in the more complex task of voice recognition.

The Role of Working Memory and Short-Term Memory in Sentence Comprehension: A Systematic Review and Meta-Analysis in Probable Alzheimer's Disease

Summary: The aim of this review is to investigate the relationship between either short-term memory or working memory and sentence comprehension in probable Alzheimer's disease. The meta-analysis results supported the association between performance on memory tasks and comprehension tasks, but further research is needed to clarify the specific source of language comprehension deficits.

The Right Temporal Lobe and the Enhancement of Voice Recognition in Congenitally Blind Subjects

Summary: Experimental investigations have shown that both faces and voices are predominantly processed by the right hemisphere. Right brain-damaged patients have more difficulties with voice recognition than with face recognition. Moreover, healthy subjects undergoing right temporal anodal stimulation improve voice recognition but not face recognition.

Spontaneous eye blinking during an auditory, an interoceptive and a visual task: The role of the sensory modality and the attentional focus

Summary: Previous evidence has shown that eye blinking rate can be influenced by the focus of attention. Most studies have compared eye blinking during visual and mental imagery tasks and suggested that increased eye blinking rate is a mechanism to focus cognitive resources internally. However, this may be influenced by the sensory modality of the task. The present study investigated whether attentional focus affects spontaneous blinking behavior in non-visual tasks and found no significant differences in eye blink rate between internal and environmental conditions.
