BackgroundInfant colic (IC) is a prevalent physiological event of infants, which can disrupt the child's home environment. We aimed to investigate the effectiveness of a mixture of Matricariae chamomilla L., Melissa officinalis L. and tyndallized Lactobacillus acidophilus (HA122) compared with Lactobacillus reuteri DSM 17938 and with simethicone for the treatment of IC. MethodsA multicenter randomized comparative study was conducted in infants with colic, according to Rome III criteria, who were randomly assigned to receive M.chamomilla L., M.officinalis L. and tyndallized L.acidophilus (HA122) (Colimil((R)) Plus((R)); Milte Italia Spa, Milan, Italy) (Group A), L.reuteri DSM 17938 (Group B) and simethicone (Group C). Treatment was given to subjects for 28days. Key ResultsOne-hundred and seventy-six patients completed the study. Mean daily crying time at day 28 was significantly lower in group A (-44, 95% CI: -58 to -30, P<.001) and group B (-35, 95% CI: -49 to -20, P<.001) when compared to group C. No significant difference was observed between Group A and Group B (mean difference: -9minutes, 95% CI -23 to +5, P=.205). At day 28, 39 of 57 (68.4%) of infants in Group C responded to the treatment compared with 57 out of 60 patients (95%) of Group A and 51 out of 59 (86.4%) of Group B (P<.001). ConclusionsThis study suggests that administration of M.chamomilla L., M.officinalis L. and tyndallized L.acidophilus (HA122) and L.reuteri DSM 17938 are significantly more effective than simethicone in IC. Clinical Trial Registration: ClinicalTrials.gov: NCT02708238.

Summary: Weaning infants with adult foods typical of Mediterranean Diet may promote adherence to a healthy diet and result in a more diverse and beneficial gut microbiota. A randomized controlled clinical trial showed that at 36 months, children in the Mediterranean Diet group were more likely to maintain good adherence to the diet, and their mothers were also more inclined to follow the Mediterranean Diet. At 4 years old, children in the Mediterranean Diet group had a more abundant population of beneficial bacteria.

Summary: A comprehensive evaluation of obstructive sleep apnoea (OSA) in children with Down syndrome (DS) revealed a high prevalence of sleep problems and enlarged tonsils and adenoids. Accompanied by orthodontic examination, overnight polygraphy (PG) identified OSA in a significant proportion of DS children, with moderate-severe cases showing higher levels of oxygen desaturation. The study highlights the importance of a coordinated multidisciplinary approach, including PG, in the diagnosis and management of OSA in DS.

Summary: The widespread use of PSA for prostate cancer diagnosis has led to high rates of overdiagnosis and overtreatment. A combination of the Prostate Health Index (PHI) and Proclarix tests can improve risk stratification of PCa patients at initial diagnosis. An artificial neural network model combining kallikrein markers in PHI and cancer-related markers in Proclarix showed increased accuracy in identifying aggressive PCa at initial diagnosis.

Summary: This study aimed to understand the incidence of MIS-C in southern Italy. The cumulative incidence rate of MIS-C in children aged 0-18 years was found to be 3.27 per 100,000 residents. The study analyzed clinical features, treatment, and outcomes, and found gastrointestinal, mucocutaneous, and cardiac involvement to be the most common. Treatment was effective, as no intensive care unit admissions were required, and no cardiac sequelae were found in follow-up echocardiograms after 6 months.

Multiple Sclerosis-Related Dietary and Nutritional Issues: An Updated Scoping Review with a Focus on Pediatrics

Claudia Mandato, Angelo Colucci, Roberta Lanzillo, Annamaria Staiano, Elena Scarpato, Luigi Schiavo, Francesca Felicia Operto, Maria Rosaria Serra, Cristina Di Monaco, Julia Sara Napoli, Grazia Massa, Pietro Vajro

Summary: Purpose of this study was to explore the role of lifestyle and diet in the development and progression of multiple sclerosis (MS) and investigate the potential benefits of diets and dietary supplementations in improving patient outcomes. Findings revealed that there is no universally best diet for MS, but healthy and balanced diets rich in fruits, vegetables, whole grains, and lean proteins while limiting processed foods, sugar, and saturated fat appear to have antioxidant and anti-inflammatory properties and promote a gut microbiota that supports gut and brain barriers. Vitamin D and polyunsaturated fatty acids are the most studied supplements for reducing MS-associated inflammation. In addition, counseling on appropriate diet and maintaining physical activity is recommended for MS prevention and management in both adults and children until more research is available on newer approaches.

Summary: This study aimed to determine the prevalence and predictive factors of Crohn's disease (CD) in children with ulcerative colitis (UC) after ileal pouch-anal anastomosis (IPAA). The results showed that children with UC undergoing IPAA had a high risk of developing subsequent CD, and a decreased preoperative body mass index was identified as a possible predictive factor for new-onset CD.
