Altered levels of circulating cytokines and microRNAs in lean and obese individuals with prediabetes and type 2 diabetes

Today obesity and type 2 diabetes (T2D) have both reached epidemic proportions. However, our current understanding of the primary mechanisms leading to these diseases is still limited due to the complex multifactorial nature of the underlying phenomena. We hypothesize that the levels of specific cytokines and miRNAs vary across the diabetes spectrum and unique signatures associated with them may serve as early biomarkers of the disease and provide insights into respective pathogenetic mechanisms. In this study, we measured the circulating levels of cytokines and microRNAs (miRNAs) in lean and obese humans with prediabetes (n = 21), T2D (n = 17), and healthy controls (n = 20) (ORIGINS trial, NCT02226640). Data were analyzed by fitting linear models adjusted for confounding variables (BMI, age, and gender in the diabetes context and age, gender, and diabetes status in the obesity context) and implementing nonparametric randomization-based tests for statistical inference. Group differences and correlations (r > 0.3) between variables with P < 0.05 were considered significant. False discovery rates (FDR) correcting for multiple testing were calculated using the Benjamini-Hochberg correction. We found a number of circulating cytokines and miRNAs deregulated in subjects with obesity, prediabetes, and T2D. Specifically, cytokines IL-6, IL-8, IL-10, IL-12, and SFRP4, as well as miRNAs miR-21, miR-24.1, miR-27a, miR-28-3p, miR-29b, miR-30d, miR-34a, miR-93, miR-126, miR-146a, miR-148, miR-150, miR-155, and miR-223, significantly changed across the diabetes spectrum, and were associated with measures of pancreatic islet beta cell function and glycemic control, among others. Notably, SFRP4 was the only studied cytokine that was significantly associated with obesity, prediabetes, and T2D, which underscores the important role of this molecule during disease development and progression. Our data suggest that changes in circulating miRNAs and cytokines may have clinical utility as biomarkers of prediabetes.

Summary: Given the havoc wreaked by the COVID-19 pandemic, researchers need to implement heightened laboratory safety measures for isolating extracellular vesicles (EVs) from patients to better understand their role in virus infectivity, disease progression, and transmission. Collaborative projects between the EV and virology scientific communities should be developed to address the relationship between endogenous EVs and potentially lethal enveloped viruses.

Plasma miRNA Profile of Crohn's Disease and Rheumatoid Arthritis Patients

A comparative Proteomics Analysis Identified Differentially Expressed Proteins in Pancreatic Cancer-Associated Stellate Cell Small Extracellular Vesicles

Summary: Small extracellular vesicles (sEVs) released from pancreatic stellate cells (HPSCs) play a role in pancreatic ductal adenocarcinoma (PDAC) progression. This study compares sEVs from normal pancreatic stellate cells (HPaStec) to sEVs from PDAC-associated stellate cells (HPSCs), and identifies differential protein expression between the two. Although sEVs do not directly affect cancer cells, they are efficiently taken up by both normal and cancer epithelial cells. The presence of certain protein markers, such as CSE1L, in HPSC sEVs suggests their significance in poor prognosis of pancreatic cancer patients.

Extracellular vesicle proteomics and phosphoproteomics identify pathways for increased risk in patients hospitalized with COVID-19 and type 2 diabetes mellitus

Summary: Recent studies indicate that extracellular vesicles (EVs) are involved in the development of SARS-CoV-2 infection and the severity of COVID-19. However, the connection between COVID-19 and type 2 diabetes (T2D) in relation to EVs has not been explored. This study examined the proteomic and phosphoproteomic profiles of circulating EVs in hospitalized patients with and without T2D, who had either COVID-19 or non-COVID-19 acute respiratory illness (RSP). Differential expression of certain proteins and phosphoproteins was observed, with a central cluster of immunomodulatory and coagulation proteins (C1QA, C1QB, and C1QC) identified in both COVID-19 and T2D networks. The study also revealed the involvement of cellular kinases and EV-shuttled kinases in severe COVID-19, particularly in patients with comorbid diabetes.

Insights and Strategies of Melanoma Immunotherapy: Predictive Biomarkers of Response and Resistance and Strategies to Improve Response Rates

Summary: Despite the success of immune checkpoint inhibitors (ICI) in cancer treatment, many patients, including those with melanoma, do not benefit long-term from these therapies. There is a need for predictive biomarkers to identify patients who will respond to targeted treatments. Genomic correlations with response and resistance to ICI are being studied to enhance patient outcomes. Emerging genomic signatures, such as neoantigens, antigen presentation, DNA repair, and oncogenic pathways, may play a role in ICI response. Understanding the genomic factors influencing response to ICI will aid in the development of novel biomarkers and strategies to overcome resistance.

Detectable Lipidomes and Metabolomes by Different Plasma Exosome Isolation Methods in Healthy Controls and Patients with Advanced Prostate and Lung Cancer

Summary: Circulating exosomes in the blood can be used as biomarkers for cancer. This study evaluated the effects of different methods of isolating plasma exosomes on the detection of multi-omic profiles in patients with lung cancer, prostate cancer, and healthy controls. The results showed that different isolation methods enriched distinct exosome cargos, and the size exclusion method captured the most diverse exosome populations. These findings suggest that the choice of isolation method can significantly impact cancer biomarker discovery.

Circulating microRNAs as Potential Biomarkers in Pancreatic Cancer-Advances and Challenges

Summary: There is an urgent need for robust and reliable biomarkers for early diagnosis, prognosis, and prediction of response to specific treatments in aggressive and deadly cancers, such as pancreatic cancer. Liquid biopsy-based miRNA profiling shows potential for fulfilling this need. MiRNAs are non-coding RNAs that regulate gene expression and have emerged as potential diagnostic, prognostic, predictive, and therapeutic biomarkers.

Advances in Liquid Biopsy Technology and Implications for Pancreatic Cancer

Although not yet approved for routine use, the increasing sensitivity and specificity of contemporary liquid biopsy platforms will likely change clinical practice in the near future.
