Breast milk feeding (BMF) is associated with lower neonatal morbidity in the very preterm infant (<32weeks gestation) and breastfeeding is beneficial for maternal health. Previous studies show large variations in BMF after very preterm birth and recognize the need for targeted breastfeeding support in the neonatal intensive care units (NICU). In a European collaboration project about evidence-based practices after very preterm birth, we examined the association between maternal, obstetric, and infant clinical factors; neonatal and maternal care unit policies; and BMF at discharge from the NICU. In multivariable analyses, covariates associated with feeding at discharge were first investigated as predictors of any BMF and in further analysis as predictors of exclusive or partial BMF. Overall, 58% (3,826/6,592) of the infants received any BMF at discharge, but there were large variations between regions (range 36-80%). Primiparity, administration of antenatal corticosteroids, first enteral feed <24hr after birth, and mother's own milk at first enteral feed were predictors positively associated with any BMF at discharge. Vaginal delivery, singleton birth, and receiving mother's own milk at first enteral feed were associated with exclusive BMF at discharge. Units with a Baby Friendly Hospital accreditation improved any BMF at discharge; units with protocols for BMF and units using donor milk had higher rates of exclusive BMF at discharge. This study suggests that there is a high potential for improving BMF through policies and support in the NICU.

RIPK3 dampens mitochondrial bioenergetics and lipid droplet dynamics in metabolic liver disease

Marta B. Afonso, Tawhidul Islam, Julie Magusto, Ricardo Amorim, Veronique Lenoir, Rui F. Simoes, Jose Teixeira, Liana C. Silva, Dominique Wendum, Isabelle Jeru, Corinne Vigouroux, Rui E. Castro, Paulo J. Oliveira, Carina Prip-Buus, Vlad Ratziu, Jeremie Gautheron, Cecilia M. P. Rodrigues

Summary: Ripk3 deficiency protects against mitochondrial and lipid droplet dysfunction in CDAA-fed mice and fat-loaded hepatocytes, accompanied by upregulation of antioxidant systems. Ripk3 deficiency leads to smaller and increased numbers of lipid droplets in Ripk3(-/-) hepatocytes, and upregulates LD-associated proteins PLIN1 and PLIN5. Upregulation of PLIN1 not only controls LD structure but also alleviates mitochondrial stress.

Heterogeneity of design features in studies included in systematic reviews with meta-analysis of cognitive outcomes in children born very preterm

Summary: Meta-analyses of studies on the impact of very preterm birth on cognition found a marked IQ deficit among VPT children compared to term-born peers, but with unexplained heterogeneity in effect size. An umbrella review was conducted to examine the design and methodology of primary studies, revealing that methodological heterogeneity mainly influenced the choice of cognitive test used.

Summary: This study investigated the risk factors for cerebral palsy (CP) and non-CP movement difficulties (MD) in extremely preterm (EPT) infants. The results showed that CP and non-CP MD have different risk factor profiles, with non-CP MD being more associated with sociodemographic factors.

Spatial metabolomics and its application in the liver

Summary: Hepatocytes in different lobule zones show distinct gene expression profiles, metabolic features, regenerative capacity, and susceptibility to damage due to the heterogeneity in oxygen, nutrient, and hormone gradients across the radial axis of the hepatic lobule. Metabolomic approaches provide a deeper understanding of the spatial metabolic profile and intercellular heterogeneity, contributing to the study of liver disease. These approaches allow for a comprehensive characterization of liver metabolic function with high spatial resolution. The challenges in achieving metabolome coverage at the single-cell level are also discussed.

Summary: This study aimed to assess the feasibility of recruiting and following up an e-cohort of adults born preterm worldwide and provide estimations of participation, characteristics of participants, the acceptability of questions, and the quality of data collected. The results showed that a total of 1004 persons had registered on the platform, with 333 participants completing the baseline questionnaire and 119 participants participating in the follow-up. The participants were diverse in sociodemographic and health characteristics, and the data collected had good quality.

Summary: The increasing prevalence of obesity brings forward its importance as a risk factor for cancer development, particularly in the gastrointestinal tract. Obesity may trigger cancer development through several mechanisms, where metabolic deregulation of adipokines can modulate multiple oncogenic molecular pathways.

Summary: This study tested the efficiency and safety of bacteria-derived copper binding agents (MBs) in depleting excess copper from the livers of WD rats, and found that a copper-binding agent called ARBM101 can effectively deplete liver copper to normal levels within 8 days. They developed a new treatment approach consisting of repetitive cycles of ARBM101 applications followed by rest periods to ensure long-term survival in WD rats.

Summary: This study evaluated the role of hsa-miRNA-21-5p in the development of hepatocellular carcinoma (HCC) in non-alcoholic fatty liver disease (NAFLD) patients. The expression of hsa-miRNA-21-5p was found to increase with NAFLD severity and was associated with hepatic lipotoxicity. Inhibition of hsa-miRNA-21-5p was shown to prevent the progression of NAFLD to NASH and HCC.

Apgar Score and Neurodevelopmental Outcomes at Age 5 Years in Infants Born Extremely Preterm

Harald Ehrhardt, Adrien M. Aubert, Ulrika Aden, Elizabeth S. Draper, Anna Gudmundsdottir, Heili Varendi, Tom Weber, Michael Zemlin, Rolf F. Maier, Jennifer Zeitlin

Summary: This study found no association between Apgar scores and long-term outcomes in infants born extremely preterm (EPT). This may be due to high interobserver variability in Apgar scoring, poorer response to resuscitation after birth among EPT infants, and the influence of more deleterious exposures in the neonatal intensive care unit or socioeconomic factors on outcomes during the first 5 years of life.

Gestational weight gain adequacy among twin pregnancies in France

Melissa Amyx, Diane Korb, Jennifer Zeitlin, Thomas Schmitz, Camille Le Ray

Summary: The objective of this study was to describe gestational weight gain (GWG) and assess the applicability of the 2009 Institute of Medicine (IOM) guidelines in a French cohort. The study found that almost half of the women had insufficient GWG according to the IOM recommendations, with only a small percentage having excessive GWG, regardless of BMI or parity. The JUMODA-derived GWG adequacy classification was lower than the IOM recommendations, indicating that a significant proportion of French women had inadequate GWG.
