Effects of varicocele and microsurgical varicocelectomy on the metabolites in semen

Summary: The influence of varicocele and microsurgical varicocelectomy on semen quality remains unclear. This study investigated the changes in semen metabolism after microsurgical varicocelectomy and found that varicocele significantly affects semen metabolism and microsurgical varicocelectomy can reverse this metabolic abnormality. Additionally, three dipeptides were identified in the seminal plasma of varicocele patients, which decreased after varicocelectomy coincident with an improvement in semen quality. Furthermore, eight metabolites may be helpful to distinguish varicocele patients from normal subjects.

Proteomic Profiling of Seminal Plasma Proteins in Varicocele Patients

Summary: This study compared the seminal plasma proteome of healthy fertile men with varicocele patients, identifying differentially expressed proteins associated with processes such as acrosome binding, protein folding, and oxidative stress. Validation confirmed abnormal expression levels of key proteins in varicocele patients. These findings suggest that aberrant seminal plasma protein expression could serve as an indicator of sperm pathology in varicocele patients.

Seminal Extracellular Vesicles and Their Involvement in Male (In)Fertility: A Systematic Review

Summary: This systematic review focused on the relationship between seminal plasma extracellular vesicles (sEVs) and male (in)fertility. After screening and eligibility assessment, nine studies were selected, which identified differences in sEV molecules (specifically proteins and small non-coding RNAs) between fertile and subfertile/infertile males. The content of sEVs was also found to be related to sperm fertilizing capacity, embryo development, and implantation.

Summary: The introduction of ICSI has addressed limitations in human reproduction related to male and female infertility, but the live-birth rate has not surpassed 30%. Advanced sperm function analysis, particularly regarding sperm DNA fragmentation (SDF), has become a focus for improving assisted reproduction outcomes. Studies have shown that using testicular sperm with lower SDF levels may lead to better ICSI results, especially in non-azoospermic males with high SDF levels.
