Summary: This study systematically evaluated the safety of non-TNFi drugs and tsDMARDs during pre-conception, pregnancy, and post-partum period, with reassuring findings that maternal and paternal exposures to these drugs do not pose a major concern for adverse pregnancy outcomes, and breastfeeding exposures to non-TNFi bDMARDs are also not a cause for major concerns.

Outcomes following biosimilar TNF inhibitors use for inflammatory-mediated immune disorders in pregnancy

Summary: The study found that patients with non-criteria antiphospholipid syndrome (APS) or undifferentiated connective tissue disease (UCTD) appear to have an increased burden of poor pregnancy outcomes compared with the general population, despite the heterogeneity and poor quality of the studies. Additionally, non-criteria APS and criteria APS patients may have similar rates of obstetric complications with standard and/or non-standard APS treatment regimens.

PEGylated Domain I of Beta-2-Glycoprotein I Inhibits Thrombosis in a Chronic Mouse Model of the Antiphospholipid Syndrome

Summary: Antiphospholipid syndrome (APS) is an autoimmune disorder characterized by vascular thrombosis and pregnancy morbidity. The current treatment options have limitations, and there is a need for more targeted therapies. Researchers developed a modified protein variant using site-specific chemical addition of polyethylene glycol (PEG), which effectively inhibited thrombus formation caused by pathogenic antibodies. These findings provide further evidence supporting the potential efficacy of PEG-modified protein as a treatment option for APS.

Summary: This study investigated the outcomes of children born to mothers with SLE exposed to HCQ or AZA during pregnancy. The results showed that there were no significant negative outcomes in children exposed to HCQ during pregnancy. However, the use of AZA was associated with an increased reporting of childhood infection, which requires further study.

Antibodies to FXa and thrombin in patients with SLE differentially regulate C3 and C5 cleavage

Summary: The significance of antibodies directed against activated factor X (FXa) and thrombin (Thr) in patients with SLE and/or antiphospholipid syndrome (APS) is unknown. This study found that aThr IgG increased Thr-mediated activation of C3 and C5, while aFXa IgG did not increase C3 or C5 activation. Structural analysis suggested that aFXa IgG may have a higher likelihood of steric hindrance on FXa compared to the AT-Thr-aThr IgG complex. Although there was no significant association between positivity for aFXa or aTHr IgG and C3 levels or disease activity, there was a trend for patients positive for aFXa IgG alone or both aFXa and aThr IgG to have lower levels of C3.
