INFLUENCE OF ARM CRANK ERGOMETRY ON DEVELOPMENT OF LYMPHOEDEMA IN BREAST CANCER PATIENTS AFTER AXILLARY DISSECTION: A RANDOMIZED CONTROLLED TRAIL

Objective: To investigate the safety and efficacy of arm crank ergometry in breast cancer patients after axillary lymph node dissection, with regard to changes in bioelectrical impedance analysis, arm circumference, muscular strength, quality of life and fatigue. Design: Randomized controlled clinical intervention trial. Subjects: Forty-nine patients with breast cancer after axillary lymph node dissection. Methods: Arm crank ergometer training twice-weekly was compared with usual care over 12 weeks. Results: The arm crank ergometer group improved significantly in terms of lean body mass and skeletal muscle mass, and showed a significant decrease in body fat. In the arm crank ergometer group, as well as the usual care group, a significant increase in armpit circumference was detected during the training period. The magnitude of the gain was higher in the usual care group. For all other measured regions of the arm a significant decrease in circumference was seen in both groups. Muscular strength of the upper extremity increased significantly in both groups, with a greater improvement in the arm crank ergometer group. In both groups a non-significant trend towards improvement in quality of life was observed. The arm crank ergometer group showed significant improvements in physical functioning, general fatigue and physical fatigue. Conclusion: These results confirm the feasibility of arm crank ergometer training after axillary lymph node dissection and highlight improvements in strength, quality of life and reduced arm symptoms with this training.

Intratumoral heterogeneity affects tumor regression and Ki67 proliferation index in perioperatively treated gastric carcinoma

Summary: The study investigates the problem of intratumoral heterogeneity (ITH) in gastric cancer (GC), and tests the effects of Ki67 and tumor regression on ITH after neoadjuvant/perioperative chemotherapy. The results suggest that Ki67 is significantly associated with tumor regression and may serve as a predictor for chemotherapy sensitivity and an indicator of ITH.

Summary: The resistance to systemic therapies in PDAC remains a major challenge in clinical practice, and NF-kappa B activity plays a crucial role in this process. A20, identified as an NF-kappa B target gene, is associated with elevated RelA expression in PDAC patients. Elevated A20 expression leads to resistance to TRAIL-induced apoptosis but not to chemotherapeutic-induced cell death, mediated by its E3-ligase activity-mediating Zink finger domain. Additionally, p62 is found to be indispensable for the TRAIL-mediated apoptosis-inducing pathway affected by A20.

Using comprehensive analysis methods, it was determined that there are two different types of amyloid affecting the annular ligament, ATTR amyloid and AFib amyloid, with distinct patient characteristics and deposition patterns. The unknown amyloid was identified as most likely of fibrinogen origin.

LAG3 in gastric cancer: it's complicated

Combining Solid and Liquid Biopsy for Therapy Monitoring in Esophageal Cancer

Summary: Esophageal cancer (EC) has a high mortality rate, and optimizing therapies and dynamically adapting to individuals is crucial. Liquid biopsy is an increasingly important method for residual disease monitoring, but conflicting detection rates and varying levels of circulating tumor DNA (ctDNA) have been observed in previous studies. This study aims to resolve this discrepancy and found that ctDNA in blood can be used for therapy monitoring of EC patients. A combination of solid and liquid samples should be used to guide individualized EC therapy.

Human APOE4 Protects High-Fat and High-Sucrose Diet Fed Targeted Replacement Mice against Fatty Liver Disease Compared to APOE3

Summary: Recent studies have found that the human APOE epsilon 4 allele protects against non-alcoholic fatty liver disease (NAFLD), while epsilon 3 promotes hepatic steatosis and steatohepatitis. In this study, mice expressing the APOE3 or APOE4 protein isoforms were fed a high-fat and high-sucrose diet to induce obesity. The results showed that APOE3 mice had higher liver weights and exhibited more severe hepatic steatosis compared to APOE4 mice. Proteome analysis also revealed differences in proteins related to inflammation, damage response, and lipid storage between APOE3 and APOE4 mice. These findings support the association of increased NAFLD risk with the APOE epsilon 3 allele and suggest potential mechanisms of protection conferred by APOE epsilon 4.

Impact of peritumoral brain edema on pre- and postoperative clinical conditions and on long-term outcomes in patients with intracranial meningiomas

Summary: This study systematically examined 696 patients with primary intracranial meningiomas to assess the effect of preoperative peritumoral brain edema (PTBE) on preoperative symptoms, neurological deficits, postoperative complications, and long-term outcomes. Results showed that PTBE significantly increased the incidences of specific preoperative symptoms, neurological deficits, and postoperative complications. Patients with preoperative PTBE required more medical support after surgery, but all patients had favorable outcomes.
