Summary: We present a case series showing that bone involvement seems to be associated with poor prognosis in primary cutaneous diffuse large B-cell lymphoma, leg-type. Bone localization is not diagnosed with the initial recommended method of thoracic-abdominal-pelvic computed tomography when asymptomatic and affecting the limbs only. If this condition is suspected, patients should be investigated systematically with further imaging, including positron emission tomography, both at baseline and if there is any concern during follow-up.

Summary: Primary cutaneous diffuse large B-cell lymphoma, leg type (PCDLBCL, LT) is a rare and aggressive lymphoma with skin involvement predominantly in the lower extremities. Front-line treatment with R-CHOP combined with involved-site radiation therapy (ISRT) showed significantly longer progression-free survival (PFS) compared to R-CHOP alone. Immune checkpoint inhibitors (ICIs) may have reasonable activity in treating relapsed/refractory PCDLBCL, LT based on observations in this study.

Whole exome sequencing was performed on DNA samples from tumor and healthy tissue of 18 patients, leading to the identification of three pathogenic variants. These variants were specific to patients with diffuse gastric cancer and not present in healthy individuals.

Summary: Diffuse large B-cell lymphoma (DLBCL) patients with early disease progression or relapse have poor outcomes. This study found that patients with late relapse had better outcomes after second-line therapy and autologous stem-cell transplantation compared to those with primary refractory or early relapse. Despite concordance in cell-of-origin classification and genetics-based subgroup between diagnostic and relapse biopsies, late relapses showed significant genetic differences and demonstrated branching evolution patterns. In highly divergent tumors, the same genes acquired new mutations independently, suggesting that shared precursor cells constrain tumor evolution towards the same genetics-based subgroups at both diagnosis and relapse.

Summary: A retrospective analysis of 20 PCDLBCL, LT patients showed that radiotherapy provided good local disease control but did not significantly improve the overall risk of disease progression. Following progression, radiotherapy could still easily be combined with systemic treatment. The strength of this analysis needs to be evaluated in a larger patient cohort.

Summary: In this study, the tumor-infiltrating lymphocytes (TILs) were characterized in 20 cases of primary cutaneous B-cell lymphoma (PCBCL). It was found that TILs were sparsely intermingled within tumor infiltrate in aggressive PCBCL and were scattered around tumor nodule edges in indolent PCBCL. The abundance of proliferative TILs was associated with better progression-free survival in aggressive PCBCL.

Summary: This study investigated the treatment modifications and COVID-19 infection incidence among psoriasis patients in France during the COVID-19 pandemic. The results found that discontinuation or reduction of treatment was associated with a higher rate of disease flares, and the risk of COVID-19 infection was related to close contact with infected individuals and living in areas with a high incidence of COVID-19. Therefore, it is important to maintain and adapt patient-physician communication during crises to avoid unnecessary treatment interruptions and inform patients about the risk of infection and the importance of hygiene rules.
