Summary: This study found that pre-transplant diabetes mellitus increases the risk of cancer after heart and/or lung transplantation. However, new-onset diabetes after transplantation is associated with a much greater cancer risk. These findings are relevant for screening during follow-up.

Summary: This study examined the demographics and outcomes of heart transplant recipients with type 2 diabetes. The proportion of patients with type 2 diabetes undergoing heart transplantation has increased in the past ten years, but they have a lower likelihood of transplantation and a higher likelihood of post-transplant mortality.

Summary: The association between diabetes mellitus and ovarian cancer has been supported by recent studies, with emerging evidence suggesting that metformin may have protective effects against ovarian cancer. Metformin exerts anti-cancer properties by activating the MAPK pathway, inhibiting the PI3K/AKT/mTOR pathway, and increasing tumor suppressor genes.

Summary: The presence of pre-transplant type 2 diabetes mellitus (T2DM) in cardiac transplant recipients is associated with increased graft failure and 5-year mortality after transplantation. Well-controlled T2DM is clinically relevant for improving the survival rate of heart transplant recipients.

Summary: In patients with type 2 diabetes, the use of metformin is associated with significantly lower risks of bacterial pneumonia, invasive mechanical ventilation, and respiratory cause of death. Longer duration of metformin use is also associated with lower hazard ratios for these risks.

Summary: Organ transplantation is an effective treatment for end-stage organ failure, but post transplantation diabetes mellitus (PTDM) is a severe complication that affects the survival of recipients. However, PTDM is often overlooked or poorly managed in its early stage. This article provides an overview of the incidence, pathogenesis, and risk factors for PTDM, aiming to improve the quality of life of recipients.

Summary: This retrospective cohort study compared the effects of metformin and sulfonylureas on heart failure hospitalization in patients with diabetes and worsening kidney function. The results showed that among patients with type 2 diabetes mellitus and declining kidney function, persistent use of metformin was associated with reduced risk of heart failure hospitalization compared to sulfonylureas.

Summary: This study found that diabetes mellitus among heart transplant recipients did not have a direct impact on survival rates, but was associated with a higher risk of infections. Post-transplant diabetes diagnosis was linked to worse survival, and could be identified through predictive factors.

Summary: Recent reports have shown that patients with heart failure (HF) and chronic kidney disease (CKD) may have an increased risk of developing malignancies, particularly after heart transplantation (HT). However, the duration of HF pre-HT was not found to be directly associated with the risk of malignancy post-HT, while CKD pre-HT was identified as an independent risk factor for malignancies after HT.

Summary: Cardiogenic shock complicating acute myocardial infarction is a complex clinical condition with poor prognosis. Early target vessel revascularization is the most effective treatment, but mortality remains high. Temporary circulatory support devices aim to enhance cardiac unloading and improve organ perfusion, but there is a lack of quality evidence to guide device selection and management, highlighting the importance of multidisciplinary expert care.

Summary: This study aimed to assess the mortality rate and outcomes of patients treated with blood transfusion (BT) in a modern intensive cardiac care unit (ICCU). The study found that BT was associated with higher mortality rates in both the short and long term. Summary: Coronary artery disease is a common condition in older adults but managing it is challenging due to the complexity of patient characteristics including frailty burden, comorbidities, and body composition abnormalities. The aim of this narrative review is to emphasize the importance of assessing frailty in guiding therapeutic decision-making and tailoring coronary artery disease management to the specific needs of older adults, taking into account age-related changes and potential drug interactions. The review also discusses gaps in evidence and offers perspectives on optimizing coronary artery disease management in older adults.

Summary: This study aimed to estimate the burden and risk factors for ischaemic heart disease (IHD) in 195 countries and territories from 1990 to 2017. The results showed that IHD remains a major public health challenge worldwide, with variations in burden and temporal trends across different geographical locations. High blood pressure, high cholesterol, high blood sugar, smoking, and obesity were identified as the main risk factors for IHD deaths.

Summary: Approximately 20% of STEMI patients have angiographically significant non-IRA CAD. Older age, male gender, and diabetes mellitus are independent predictors for non-IRA CAD, while smoking and family history of CAD predict IRA-only CAD. The presence of non-IRA CAD is associated with higher long-term all-cause mortality rates.

Summary: This study aimed to investigate the prognostic impact of D-dimer in intensive coronary care unit (ICCU) patients. The results showed that high D-dimer levels were associated with older age, more comorbidities, diverse admission diagnoses, and higher mortality rate. Therefore, D-dimer levels can serve as an independent poor prognostic factor in ICCU patients.
