Recent research suggests targeting epigenetic modulators in neuroendocrine cells as a new therapeutic avenue, with studies showing potential functional effects and pathways affected.

Summary: The study found that there was no difference in survival rates between patients with PNETs <2 cm who underwent enucleation versus oncologic resection. Ten percent of clinically node-negative patients were node-positive after surgery. Therefore, patients who receive enucleation may benefit from enhanced surveillance for locoregional recurrence, especially those with higher grade and larger size tumors.

For patients with G1 and a tumor diameter <= 4 cm, the type of surgery was an independent factor affecting their long-term prognosis, with enucleation showing better OS/CSS compared to other surgical methods.

Summary: This study compared the postoperative and long-term outcomes of parenchyma-sparing resections (PSR) and lymph node dissection with oncologic resections in patients with small nonfunctional pancreatic neuroendocrine tumors (PNET). The results showed that PSR and lymph node-sparing resections have advantages in terms of less blood loss, shorter operative times, and lower complication rates compared to oncologic resections.

ALT emerged as a reliable indicator of worse prognosis for PanNET, aiding in clinical stratification and identification of high-risk patients, and supporting the pancreatic origin of neuroendocrine tumors for improving diagnostic workflow in patients with neuroendocrine metastasis from unknown primary.

Data from the National Cancer Database were used, and patients diagnosed with NF-pancreatic neuroendocrine neoplasms between January 1, 2004, and December 31, 2017 were included. Surgical resection was associated with longer survival in select patients with NF-PanNETs measuring 1.1 to 2.0 cm who were younger than 65 years, had no comorbidities, received treatment at academic institutions, and had tumors of the distal pancreas.
