Insulin resistance and the risk of hepatocellular carcinoma in chronic hepatitis B patients

Background and Aim: We analyzed whether insulin resistance (IR) assessed by homeostasis model assessment (HOMA2-IR) index can stratify hepatocellular carcinoma (HCC) risk in patients with chronic hepatitis B virus (HBV) infection. Methods: A retrospective cohort of 1696 chronic HBV-infected patients (age: 50.07.8years, men=964 [56.8%]) who underwent detailed health checkup program including C-peptide and fasting blood glucose measurement and followed up for more than a year were analyzed. Results: During a median follow-up of 5.0years (range, 1.0-10.5years), 24 patients (1.4%) developed HCC. The HCC incidence rate was higher for patients with higher HOMA2-IR value than those with lower HOMA2-IR value (1.7% vs 0.5% for HOMA2-IR >1.200 vs <= 1.200, P=0.009). HOMA2-IR was a significant factor associated with HCC development in multivariable-adjusted model (HR [95% CI]: 3.25 [1.13-9.31], adjusted for age, sex, cirrhosis, and HBV DNA levels). The association between HOMA2-IR and HCC was markedly attenuated and became no longer statistically significant (HR [95% CI]: 1.93 [0.57-6.51]) when further adjusted for obesity, hypertension, and diabetes. In subgroup analysis, HOMA2-IR value was an independent factor associated with HCC in patients without overt metabolic abnormalities (hypertension, diabetes, and metabolic syndrome) but not for those with overt metabolic abnormalities. Conclusion: Insulin resistance assessed by HOMA2 was associated with the risk of HCC, indicating that HOMA2-IR can be a useful tool for stratifying the risk of HCC in chronic HBV-infected patients, particularly in patients without overt metabolic abnormalities.

Placement of Plastic Stents after Direct Endoscopic Necrosectomy through a Novel Lumen-Apposing Metal Stent for Effective Treatment of Laterally Extended Walled-Off Necrosis: A Pilot Study

Summary: This study evaluated the efficacy of routinely placing long plastic stents after direct endoscopic necrosectomy (DEN) to treat laterally extended walled-off necrosis (WON). In 6 patients with symptomatic laterally extended WON, the clinical success rate of DEN with long plastic stent placement was 100%, without major adverse events. The size of WON ranged from 18.6 to 35.8 cm in length, and 2-3 long plastic stents were placed after each DEN session.

Serum S100B Levels in Patients with Liver Cirrhosis and Hepatic Encephalopathy

Summary: This study aimed to investigate the diagnostic ability of serum S100B in discriminating the grade of hepatic encephalopathy (HE) and its correlation with other parameters. The results showed that serum S100B level was not useful for differentiating the severity of HE, but it could be affected by age, serum bilirubin, and creatinine levels and was associated with MELD scores. Additionally, serum S100B levels showed the possibility of predicting 1-year mortality in cirrhotic patients.

Validation of non-invasive diagnosis of CSPH in patients with compensated advanced chronic liver disease according to Baveno VII

Summary: This study evaluated the usefulness of the Baveno VII consensus in predicting the risk of decompensation in patients with compensated advanced chronic liver disease (cACLD). The study found that patients with clinically significant portal hypertension (CSPH) had the highest risk of decompensation, followed by the grey zone high-risk group, grey zone low-risk group, and those without CSPH. Compared to the CSPH excluded group, the CSPH included group, grey zone high-risk group, and grey zone low-risk group had significantly higher risk of decompensation.

Validation of MELD 3.0 scoring system in East Asian patients with cirrhosis awaiting liver transplantation

Summary: A new predictive model, which considers both the MELD 3.0 and albumin, has been proposed for predicting the 3-month survival of East Asian patients with cirrhosis. The study found that the MELD 3.0 score with albumin showed slightly better discrimination compared to other MELD-based scores, but not significantly different. However, it was significantly better than the original MELD in predicting the short-term prognosis of East Asian patients on the liver transplantation waitlist.

Associations of physical activity domains and muscle strength exercise with non-alcoholic fatty liver disease: a nation-wide cohort study

Summary: This study investigated the associations between different types and domains of exercise and non-alcoholic fatty liver disease (NAFLD). It found that increasing overall physical activity and muscle strength exercise can reduce the risk of NAFLD, but the effects vary depending on the domain of physical activity. Therefore, the management of NAFLD should consider differentiating physical activity by its domains.

The intubation success rate using a cap-assisted colonoscope was higher in the SS-JJ group compared to the SE-JJ group. SS-JJ was a predictive factor for successful intubation.

A model for predicting poor survival in patients with cirrhosis undergoing portosystemic shunt embolization

Summary: This study aimed to develop and validate a prognostic model for identifying patients with a risk of poor short-term survival after PSSE. Results showed that baseline levels of serum albumin, total bilirubin, and international normalized ratio were significantly associated with 1-year overall survival after PSSE. The ABI score, developed based on these factors, showed good discrimination performance in predicting 3-month and 6-month survival.
