Randomized Clinical Trials on Deep Carious Lesions: 5-Year Follow-up

Deep caries presents a dilemma in terms of which treatment that will render an optimal prognosis by maintaining pulp vitality with absence of apical pathology. Previously, 2 randomized clinical trials were performed testing the short-term effects of stepwise carious tissue removal versus nonselective carious removal to hard dentin with or without pulp exposure. The aim of this article was to report the 5-y outcome on these previously treated patients having radiographically well-defined carious lesions extending into the pulpal quarter of the dentin but with a well-defined radiodense zone between the carious lesion and the pulp. In this long-term study, 239 of 314 (76.2%) patients were analyzed. The stepwise removal group had a significantly higher proportion of success (60.2%) at 5-y follow-up compared with the nonselective carious removal to hard dentin group (46.3%) (P = 0.031) when pulp exposures per se were included as failures. Pulp exposure rate was significantly lower in the stepwise carious removal group (21.2% vs. 35.5%; P = 0.014). Irrespective of pulp exposure status, the difference (13.3%) was still significant when sustained pulp vitality without apical radiolucency and unbearable pain was considered (95% confidence interval, 3.1-26.3, P = 0.045). After pulp exposure, only 9% (n = 4) of the analyzed patients were assessed as successful, indicating that the prognosis is highly dubious following conventional pulp-capping procedures (direct pulp capping or partial pulpotomy) in deep carious lesions in adults. In conclusion, the stepwise carious removal group had a significantly higher proportion of pulps with sustained vitality without apical radiolucency versus nonselective carious removal of deep carious lesions in adult teeth at 5-y follow-up.

Tooth survival after endodontic treatment

Summary: This article reviews the survival of teeth after root canal treatment and focuses on the factors that affect tooth survival. The study found that tooth restoration is the most important factor associated with the success of root canal treatment. Indirect restorations were found to have a higher survival rate compared to direct restorations.

Additional Treatment Indicative of an Unfavorable Endodontic Outcome in a Swedish County - A 10-year Observational Study

Summary: The purpose of this study was to determine the incidence of additional treatment or extraction following orthograde root canal treatment (RCT) performed by general dental practitioners. A cohort of 280 individuals with orthograde RCT was followed for 10 years. The results showed a cumulative 10-year survival rate of 92.7% for RCTs, with a higher incidence within the first 2 years. Five factors were associated with an unfavorable outcome, but confounders could not be ruled out.

Dimensional changes of endodontic sealers-An in vitro model simulating a clinical extrusion scenario during 18 months

Summary: This study used 3D surface scanning and subtraction radiography to examine the dimensional changes of endodontic sealers over 18 months. The results showed that Apexit Plus had the largest dimensional changes, followed by TubliSeal EWT and BioRoot RCS, while AH Plus remained stable throughout 18 months.

The effect of a lozenge combining prebiotic arginine and probiotics on caries increment in children during 10-12 months, a randomized clinical trial

Summary: This study investigated the effects of using a lozenge containing arginine and probiotics for 10-12 months on caries increment, gingivitis, and plaque occurrence in children aged 5-9 years. The results showed that the lozenge significantly reduced the caries increment, but had no significant effect on plaque or gingivitis occurrence.

Short-term sugar stress induces compositional changes and loss of diversity of the supragingival microbiota

Summary: Frequent intake of free sugars can cause dental caries, but the immediate effect of sugar intake on the supragingival microbiota is still unknown. This study aims to explore the impact of 14 days of sugar rinsing on the supragingival microbiota. The results show that temporary sugar stress can lead to a loss of diversity and compositional changes in the supragingival microbiota, but these changes can be reversed with oral care.

Characterization of patients with odontogenic necrotizing soft tissue infections in the head and neck area. A retrospective analysis

Periradicular surgery: A longitudinal registry study of 10-year outcomes and factors predictive of post-surgical extraction

Summary: This study examined the long-term survival of teeth after periradicular surgery in Sweden and identified factors associated with extraction within 10 years of the surgery. The results showed that approximately one-third of the teeth were extracted during the follow-up period, and mandibular molars were at the highest risk of extraction compared to maxillary incisors and canines.

The efficacy of combined arginine and probiotics as an add-on to 1450 ppm fluoride toothpaste to prevent and control dental caries in children - A randomized controlled trial

Summary: This study aimed to investigate the effect of daily consumption of a lozenge combining arginine and probiotics on the Relative Risk Reduction (RRR) of dental caries transitions and lesion activity in children. The intervention group received the lozenge containing arginine and probiotics while the placebo group received a placebo lozenge. The results showed a significant reduction in caries progression, increase in regression, and decrease in active caries lesions in the intervention group compared to the placebo group. Overall, the combined use of arginine and probiotics could be a promising approach for caries prevention and control.

The national child odontology registry (SCOR): a valuable resource for odontological and public health research

Summary: The National Child Odontology Registry in Denmark has collected data on the oral health of Danish children and adolescents since 1972. This study provides an overview of the registry's history and the changes in registration criteria for dental variables over time. It also highlights the prevalence of gingivitis and periodontitis following changes in registration criteria and suggests the need for further research in reporting accuracy. It concludes that the registry offers a unique opportunity for studying childhood oral health but recommends consulting with experts for the best use of the available data.
