Summary: This study aimed to investigate the adverse events and risk factors for serious outcomes associated with cetuximab therapy for head and neck cancer. Using data from the FDA Adverse Event Reporting System, it was found that mucosal inflammation was the most common adverse event. The strongest risk factor for a serious outcome was cetuximab therapy in countries outside the US, followed by polytherapy.

Summary: Remimazolam is an interesting anaesthetic and sedative that combines the pharmacodynamic properties of midazolam with pharmacokinetic properties similar to remifentanil. However, recent case reports of anaphylaxis, delayed emergence, and re-sedation have raised concerns and require further investigation.

Summary: The study analyzed the largest case series of serious CNS adverse drug reactions induced by ceftriaxone, identifying common symptoms including convulsions, myoclonia, and confusion, mainly in elderly female patients. It suggests caution in the use of ceftriaxone, particularly in older patients or those with renal impairment.

Summary: The timing of surgery for hip arthroplasty has a significant impact on the short-term outcome for patients. A study based on data from 2002 to 2014 found that surgery performed within 2 days showed more beneficial effects on adverse events than delayed surgery. However, surgeons should be aware of the potential increased risks of mechanical complications and post-hemorrhagic anemia.

Summary: This study analyzed the risk factors for postoperative delirium in patients with hip fracture, finding that a history of delirium, diabetes mellitus, hypoxemia, hypoalbuminemia, and low body mass index were independent risk factors. Cutoff values for postoperative blood sugar, albumin, and BMI were identified for predicting delirium occurrence.

Summary: This study identified several independent risk factors for postoperative delirium in hip fracture patients, including preoperative albumin level, history of delirium, TSH level, resting score on the first postoperative day, and age. Monitoring these factors may be beneficial for predicting and managing postoperative delirium.

Summary: Immune-related adverse events (irAEs), including cardiovascular events, are complications that can occur during treatment with immune checkpoint inhibitors (ICIs). The risk of immune-related cardiovascular adverse events (irACEs) has increased with the widespread use of ICIs. This review focuses on assessing the risk factors for irACEs and raising awareness for early risk assessment.

Summary: This study assessed the occurrence of thrombosis and major bleeding in children with congenital or acquired heart disease treated with VKA and identified risk factors for these serious adverse events. The results suggested that VKA therapy appears to be safe in children with CAHD, but suboptimal time in therapeutic range was associated with bleeding events.

Summary: In nonagenarian patients undergoing hip fracture surgery, the 30-day mortality rate was 7.4% and the 30-month mortality rate was 18.2%. Postoperative complications were found to be independently associated with higher mortality, particularly when occurring early after surgery.

Summary: This study aimed to identify patient and hospital-level characteristics associated with time to surgery (TTS) in Ireland for hip fracture care. The results showed that factors such as male sex, higher ASA grade, lower pre-morbid mobility, inter-hospital transfer, weekday presentation, pre-operative medical physician assessment, intracapsular fracture type, arthroplasty surgery, general anesthesia, consultant grade of surgeon, and lower hospital-level orthopaedic surgical capacity were associated with longer TTS. On the other hand, the oldest age-group and pre-fracture nursing home residence were associated with shorter TTS. None of the explored protocols for expediting surgery were associated with TTS.

Summary: This study analyzed factors related to early 30-day mortality in patients over 65 undergoing hip fracture surgery. The results showed that both preoperative and postoperative factors have a significant impact on mortality rate after hip fracture surgery. Postoperative factors, though previously unidentified or quantified, play a crucial role in influencing 30-day mortality.

Summary: This study examines the association between time-to-surgery and complications in hip fracture patients. The results show that early surgery within 24 hours after injury significantly reduces complications in extracapsular fractures but not in intracapsular fractures. Therefore, guidelines should consider the individual case scenario and fracture type when determining the optimal timing for surgery.

Summary: This study found a nonlinear relationship between clinical outcomes and dorsal tilt in distal radial fractures, with worse outcomes associated with increasing dorsal tilt. The decline in clinical outcomes starts at 5°, but a noticeable difference in capability as measured with the QuickDASH is unlikely until a dorsal tilt of 20° in a population up to 75 years old.

Summary: Vaccination provides protection against severe COVID-19 caused by SARS-CoV-2 omicron, but is less effective against infection. High levels of serum antibodies are shown to be protective against infection and linked to reduced viral load and time to viral clearance. Pre-omicron SARS-CoV-2 infection is independently associated with increased protection against omicron, largely mediated by mucosal spike-specific IgA responses.

Summary: This study aimed to explore the timing, severity, and preventability of adverse events (AEs) in patients undergoing hip arthroplasty surgery. The majority of AEs occurred perioperatively and postoperatively, with a wide range of median days for different AE types. Most AEs were deemed preventable and/or of major severity, emphasizing the need for a better understanding of the timing and occurrence of AEs to improve patient safety in hip arthroplasty surgery.

Summary: Digitization in orthopaedics and traumatology is a rapidly evolving field with various stakeholders. Effective communication among technologists, users, patients, and actors in the healthcare systems is crucial. Understanding technology requirements, potentials of digital applications, and the shared goal of improving patient health provides a great opportunity. Transparent expectations from patients and surgeons' capacity to use digital technologies are important. Ethical handling of big data and considerations for transparency are also necessary.

Summary: This study investigated the association between periprosthetic joint infection (PJI) and mortality. The results showed that the PJI group had a higher mortality rate, but the adjusted hazard ratio indicated that the increased risk was mainly due to the comorbidity of the patient, rather than the infection itself.

Summary: This study presents a deep learning model for fracture classification on shoulder radiographs using a convolutional neural network (CNN). The results show that the CNN model performs well in the classification of proximal humeral fractures (PHF) based on the AO/OTA classification system and other types of fractures. The study's generalizability is strong, further consolidating the position of CNN as an effective fracture classifier in plain radiographs. However, external validation of the model should be conducted before it is applied in clinical practice.

Summary: A study was conducted on 14 patients who had undergone elective surgery to explore their experience of participation in their care. The results showed that creating a meaningful relationship was essential for patient participation. Factors such as abilities, willingness, and lack of experience influenced their level of participation, while a professional approach with open communication and individualized information were important facilitators.
