Summary: Crohn's disease (CD), a subtype of inflammatory bowel disease (IBD), is prevalent in Western countries and has been associated with intestinal dysbiosis. A systematic review and meta-analysis evaluated the efficacy of a high fiber diet in improving CD remission rates, with or without other treatments. Eleven studies involving 2389 CD patients were included. The results showed that a high fiber diet, in conjunction with routine therapies, can enhance CD remission rates. This study provides evidence that dietary fiber intake can effectively improve remission rates in CD patients, regardless of additional treatment.

Summary: This study assessed the efficacy of medical treatments for ulcerative proctitis (UP). The results showed that topical application of 5-aminosalicylic acid and corticosteroids were effective for active UP. They may also be effective for maintenance of remission. Tacrolimus may be effective for induction of remission.

Summary: Based on published data, this study analyzed the relationships between apparent diffusion coefficient (ADC) and activity parameters of Crohn's disease. The results showed that ADC had strong inverse correlations with CDAI, MaRIA, and SES-CD scores. However, its role in assessing fibrotic changes in the bowel wall is limited.

Summary: This study updated the clinical efficacy and safety of budesonide in the treatment of eosinophilic esophagitis using high-quality randomized controlled trial results, showing that oral budesonide is superior to placebo in terms of efficacy. Additionally, it was found that the efficacy of budesonide in patients is not dose-dependent.

Summary: Using a network meta-analysis and systematic review, this study evaluated the efficacy and safety of combination therapy and infliximab monotherapy in Crohn's disease patients. The results showed that there was no statistical difference between different combination therapies in inducing and maintaining remission. For inducing remission, infliximab + enteral nutrition ranked highest; for maintaining remission, infliximab + azathioprine ranked highest. There was no treatment that was significantly safer than the others.

Summary: Information on the comparative efficacy of biologics and small targeted molecules for Crohn's disease (CD) remains unclear. This study aimed to quantify the relative efficacy of investigational and approved biological treatments for CD. Mathematical models were developed to describe the time course and dose-response of efficacy measures, and simulations were performed to compare efficacies across different drugs. The most effective biologics were tumor necrosis factor (TNF)-alpha inhibitors, while the highest efficacy of small targeted molecules was observed in janus kinase (JAK) inhibitors. These findings have important implications for clinical practice in CD.

Summary: A systematic literature review and meta-analysis confirmed the efficacy and safety of oral Pentasa in treating active UC and maintaining remission. Pentasa demonstrated superiority over placebo in inducing and maintaining remission, and had similar efficacy to other 5-ASAs. Additionally, Pentasa was found to be better tolerated than sulfasalazine in maintaining remission.

Summary: Differentiating between perianal fistulas related to cryptoglandular disease and Crohn's disease can be achieved through various diagnostic strategies. MRI and ultrasound characteristics are the most reliable methods, but they have relatively low sensitivity. Further research is needed to confirm these findings and explore whether combining multiple diagnostic tests can improve diagnostic sensitivity.

Summary: This study found that switching to SMART treatment was associated with a longer time to the first severe asthma exacerbation for patients with poorly controlled asthma. Compared to escalating or continuing GINA treatment with inhaled corticosteroid-long-acting beta(2)-agonist plus short-acting beta(2)-agonist reliever, the SMART regimen was preferred.

Summary: Gastroparesis is a functional gastrointestinal disorder that primarily affects women during childbearing age, but there is a lack of data and guidelines for managing it during pregnancy. Despite the absence of approved therapies, healthcare providers must navigate the challenge of counseling and managing gastroparesis patients. This systematic review aims to fill the knowledge gaps regarding the impact of pregnancy on gastroparesis and vice versa, while highlighting the available therapies and safe dietary approaches during pregnancy.

Summary: This study examines the association between rheumatologist access, early treatment, and ongoing care of older-onset rheumatoid arthritis (RA) and healthcare utilization and costs following diagnosis. The results show that access to rheumatologists, timely treatment, and ongoing care are associated with lower total healthcare costs at 5 years. Investments in improving access to care may lead to long-term health system savings.

Summary: As the patents for biologic originator drugs expire, biosimilars are emerging as cost-effective alternatives within healthcare systems. A global survey was conducted to shed light on physicians' knowledge, beliefs, practical approaches, and concerns related to biosimilar adoption. Based on the survey results, a consensus was reached on key statements covering biosimilar effectiveness, safety, indications, rationale, multiple switches, therapeutic drug monitoring, non-medical switching, and future perspectives. The consensus affirmed that biosimilars are equally effective and safe compared to originator drugs.
