Angiogenic Markers and Cardiovascular Indices in the Prediction of Hypertensive Disorders of Pregnancy

Angiogenic and antiangiogenic factors have proven to be an accurate predictive means of preeclampsia. Echocardiographic studies have shown that women with preeclampsia exhibit significant cardiovascular strain, especially early-onset preeclampsia. The aim of this study is to determine preeclampsia risk with soluble fms-like tyrosin kinase 1/ placental growth factor ratio, serum NT-proBNP (N-terminal pro B-type natriuretic peptide), and biophysical markers of cardiovascular function in a prospective case-control study. We examined a cohort of 110 pregnant women with uneventful pregnancy outcome (controls) and 129 with hypertensive pregnancy disorders, including 77 with preeclampsia and 52 with pregnancy-induced hypertension. Cardiac indices were obtained with a USCOM-1A monitor, and soluble fms-like tyrosin kinase 1, placental growth factor, and NT-proBNP were measured in serum samples on automated platforms. Logistic regression, as well as Cox proportional hazard analysis, was performed. There were significant contributions from all variables tested, except for heart rate, stroke volume index, and cardiac index to the prediction model. When testing accuracy of respective markers in combination (full model) versus individual markers (soluble fms-like tyrosin kinase 1/ placental growth factor ratio and total peripheral resistance) was compared. The soluble Ems-like tyrosin kinase 1/ placental growth factor ratio and total peripheral resistance performed as good as the full model, except for hypertensive pregnancy disorders and pregnancy-induced hypertension, where the full model performed better. The additional assessment of biophysical and biochemical markers of cardiovascular strain in pregnancy increases the detection of the composite group of hypertensive pregnancy disorders, while not significantly improving detection of preeclampsia alone. This offers a more precise insight into the pathogenesis of the disease, as well as offering a window for intervention, possibly decreasing cardiovascular mortality in these women.

Summary: This study aimed to investigate the role of fetal brain MRI in detecting associated anomalies in fetuses with congenital CMV infection and normal neurosonography. The results showed that fetal brain MRI can detect additional anomalies in fetuses with congenital CMV infection and negative neurosonography, and the viral load in the amniotic fluid is an independent predictor of the risk of associated anomalies in these fetuses.

Prenatal exome sequencing and impact on perinatal outcome: cohort study

Effectiveness and safety of prenatal valacyclovir for congenital cytomegalovirus infection: systematic review and meta-analysis

Summary: The objective of this systematic review and meta-analysis was to investigate the safety and effectiveness of prenatal valacyclovir therapy in pregnancies with maternal CMV infection. The findings suggest that valacyclovir therapy can reduce the risk of congenital CMV infection. However, further research is needed to determine the impact of valacyclovir on the course of infection in the fetus and the risk of symptomatic fetal or neonatal infection.

Summary: Patients with COVID-19 may experience various neurological symptoms, but the role of the central nervous system (CNS) in COVID-19 is still unclear. The inflammatory response in the cerebrospinal fluid (CSF) of COVID-19 patients was studied, and it was found that there were attenuated inflammatory changes compared to patients with herpes simplex virus encephalitis (HSVE). The inflammatory mediators in the CSF of COVID-19 patients were likely derived from the systemic circulation rather than produced within the CNS. Further investigation is needed to understand the relevance of blood-derived mediators of inflammation in the CSF for neurological COVID-19 and post-COVID-19 symptoms.

Summary: This systematic review examines the outcomes of laser surgery for twin-twin transfusion syndrome (TTTS) in monochorionic diamniotic (MCDA) twin pregnancies complicated by selective fetal growth restriction (sFGR). The study finds that MCDA twin pregnancies with TTTS and sFGR have a higher risk of fetal loss after laser surgery, as well as a higher risk of neurological morbidity in the donor twin.

Summary: The objective of this study was to assess the predictive accuracy of first-trimester choroid-plexus-to-lateral-ventricle/head ratios for development of ventriculomegaly at a later gestational age. The study found that first-trimester fetal choroid-plexus-to-lateral-ventricle/head ratios have good predictive accuracy for the development of ventriculomegaly.

Transplacental non-steroidal anti-inflammatory drugs versus expectant management in fetal Ebstein anomaly with circular shunt: Systematic review and meta-analysis

Summary: This study is the first systematic review and meta-analysis comparing expectant management (EM) versus treatment with NSAIDs in fetuses with EA/TVD with a circular shunt. The results showed that the NSAID group had significantly higher rates of live birth and survival to hospital discharge compared with the EM group, but the number of cases treated with NSAIDs was small, with limited safety data.

Fetal cardiac intervention in hypoplastic left heart syndrome with intact or restrictive atrial septum, systematic review, and meta-analysis

Summary: This study investigated the outcomes of fetuses with hypoplastic left heart syndrome (HLHS) and intact or restrictive atrial septum (I/RAS) managed expectantly or with fetal atrial septal intervention (FASI). The study found no differences in survival rates between the two groups, but FASI may lead to a less restrictive septum at birth. Future multicenter studies are needed for better identification of cases with improved outcomes and standardized techniques and definitions.

Successful Prenatal Treatment of Cardiac Rhabdomyoma in a Fetus with Tuberous Sclerosis

Summary: Cardiac rhabdomyomas can potentially indicate the presence of Tuberous Sclerosis Complex (TSC). Though they usually regress without intervention, they can grow and disrupt heart function, posing a threat to the child's life. Administering rapalogs, like sirolimus, to a pregnant mother with TSC can effectively stop the growth of cardiac tumors and cause them to shrink. This article presents a successful case of treating a fetal cardiac rhabdomyoma in a TSC patient using sirolimus.

It is recommended to perform more attentive and comprehensive HPV screening and cytological examination for pregnant women with HIV infection.

Depression, anxiety and stress in women with breech pregnancy compared to women with cephalic presentation-a cross-sectional study
