Rome Foundation-Asian working team report: Asian functional gastrointestinal disorder symptom clusters

Objective Functional gastrointestinal disorders (FGIDs) are diagnosed by the presence of a characteristic set of symptoms. However, the current criteria-based diagnostic approach is to some extent subjective and largely derived from observations in English-speaking Western patients. We aimed to identify latent symptom clusters in Asian patients with FGID. Design 1805 consecutive unselected patients with FGID who presented for primary or secondary care to 11 centres across Asia completed a cultural and linguistic adaptation of the Rome II Diagnostic Questionnaire that was translated to the local languages. Principal components factor analysis with varimax rotation was used to identify symptom clusters. Results Nine symptom clusters were identified, consisting of two oesophageal factors (F6: globus, odynophagia and dysphagia; F9: chest pain and heartburn), two gastroduodenal factors (F5: bloating, fullness, belching and flatulence; F8 regurgitation, nausea and vomiting), three bowel factors (F2: abdominal pain and diarrhoea; F3: meal-related bowel symptoms; F7: upper abdominal pain and constipation) and two anorectal factors (F1: anorectal pain and constipation; F4: diarrhoea, urgency and incontinence). Conclusion We found that the broad categorisation used both in clinical practice and in the Rome system, that is, broad anatomical divisions, and certain diagnoses with long historical records, that is, IBS with diarrhoea, and chronic constipation, are still valid in our Asian societies. In addition, we found a bowel symptom cluster with meal trigger and a gas cluster that suggests a different emphasis in our populations. Future studies to compare a non-Asian cohort and to match to putative pathophysiology will help to verify our findings.

Association between irritable bowel syndrome and micronutrients: A systematic review

Predicting Driver Takeover Time in Conditionally Automated Driving

Summary: Ensuring a safe takeover transition in conditionally automated driving is of utmost importance, with takeover time being a critical factor. Previous studies have identified various factors affecting takeover time, but few have considered them all simultaneously. In this study, we used a dataset and eXtreme Gradient Boosting (XGBoost) to predict takeover time, and SHAP (SHapley Additive exPlanation) to analyze and explain the effects of predictors. We identified seven key predictors that yield the best prediction performance and examined their main effects and interactions. The proposed approach demonstrates good performance and explainability, with implications for designing in-vehicle monitoring and alert systems to facilitate driver-automated vehicle interaction.

They identified putative pathophysiology and provided a management algorithm for primary and secondary care clinicians.

Childhood Acid Suppressants May Increase Allergy Risk-A Systematic Review and Meta-Analysis

Summary: Recent studies indicate that the use of acid suppressants in early childhood may elevate the risk of allergic diseases. This systematic review examined the associations between childhood acid suppressant use and the development of allergic diseases. The findings suggest that childhood use of acid suppressants is significantly associated with an increased risk of asthma, atopic dermatitis, and allergic rhinitis. However, larger studies are needed to establish causality, and more stringent guidelines should be implemented for the judicious use of these drugs in pediatric patients.

Real-Time Workload Estimation Using Eye Tracking: A Bayesian Inference Approach

Summary: Workload management is crucial for shared control of unmanned ground vehicles. Previous studies have used physiological data to estimate operators' workload, but they mostly utilized a single-model-single-feature or a single-model-multiple-feature approach. This study proposes a Bayesian inference model that leverages different machine learning models for different features to estimate workload. Experimental results with 24 participants show that the Bayesian inference model outperforms the single models with a 0.823 F-1 score, 0.824 precision, and 0.821 recall.

Volatile organic compounds as potential biomarkers of irritable bowel syndrome: A systematic review

Summary: This systematic review aimed to identify volatile organic compounds (VOCs) in the diagnosis of irritable bowel syndrome (IBS) through Medline and Cochrane's databases. 57% of the studies showed that VOCs could differentiate IBS patients from healthy controls with AUC ranging from 0.83 to 0.99. Studies distinguishing IBS from inflammatory bowel disease (IBD) patients had slightly higher AUC of 0.87-0.98. Combining VOC into panels allowed the creation of discriminative algorithms. Despite limitations in VOC sampling heterogeneity and small sample sizes, our review suggests that VOC analysis has the potential to be a noninvasive point-of-care test that differentiates IBS from other organic gastrointestinal diseases.
