Morbidity and mortality of childhood-and adolescent-onset epilepsy: A controlled national study

Purpose: Epilepsy is associated with significant morbidities and mortality. We aimed to evaluate the 30-year morbidities and mortality in a national group of patients after a first diagnosis of epilepsy. Methods: From the Danish National Patient Registry (NPR), in total, 3123 patients with epilepsy aged 0-5 years and 5018 patients aged 6-20 years diagnosed in 1998-2002 were identified and compared with, respectively, 6246 and 10,036 persons matched for age, gender, and place of living with randomly chosen citizens from the Danish Civil Registration System Statistics. In the NPR, all morbidities in the following 30 years were grouped into major WHO disease classes. Key findings: Patients with epilepsy had significantly higher rates of comorbidities including almost all health related comorbidities compared with controls. Mortality rates were elevated: the hazard ratio (5%; 95% CI) was 14.46 (11.8; 17.7, p < 0.001) and 5.58 (4.9; 6.4, P < 0.001) for patients aged 0-5 years and 6-20 years at first diagnosis of epilepsy, respectively. Significance: Epilepsy is associated with significant comorbidities and mortality including all health care domains, especially among persons who were young at the onset of epilepsy. Cause-specific mortality and life years lost in people with epilepsy: a Danish cohort study

Summary: People with epilepsy have a higher mortality rate compared to the general population, resulting in a reduced life expectancy of around 12 years. The causes of death vary, but psychiatric comorbidity and modifiable risk factors such as accidents and cardiovascular disorders contribute significantly to the excess mortality. Preventive measures should focus on addressing these factors to reduce epilepsy-related deaths.

Estimates of epilepsy prevalence, psychiatric co-morbidity and cost

Summary: This study estimated the prevalence, psychiatric co-morbidity, and annual costs associated with epilepsy in Denmark. The results showed an epilepsy prevalence of 0.67%, with 37.4% of epilepsy patients having a psychiatric disorder or using drugs for psychiatric disorders. The estimated annual individual net costs associated with epilepsy were €30,683.

Pregnancy and Use of Antiseizure Medication

Summary: Antiseizure medications (ASMs) are commonly used by pregnant women for various conditions, but there is limited evidence on the cognitive risks of in utero exposure. Healthcare professionals and pregnant women face the challenge of balancing the risks and benefits of different treatments due to the lack of robust safety data. High-quality data on the safety of ASMs used in pregnancy are urgently needed, but randomized clinical trials are not possible in pregnant women. Therefore, evidence must be obtained from observational studies.

Traumatic brain injury, stroke, and epilepsy: A mediation study in a Danish nationwide cohort

Summary: Traumatic brain injury (TBI) and stroke are well-known causes of acquired epilepsy, and TBI is also a risk factor for stroke. This study investigated the extent to which the effect of TBI on epilepsy operated through intermediary stroke. The results showed that less than 6% of the risk of epilepsy following TBI is mediated by stroke, but this mechanism becomes more significant with age and for late onset epilepsies.

Directionality of the Association Between Epilepsy and Depression A Nationwide Register-Based Cohort Study

Summary: This study investigates the magnitude and long-term association between epilepsy and depression, comparing with the risks of the 2 disorders after another chronic medical illness (asthma). Results show a long-term bidirectional relationship between depression and epilepsy, with higher risk estimates than for epilepsy or depression after asthma.

Neonatal Phototherapy and Clinical Characteristics: The Danish National Patient Registry 2000-2016

Summary: This study aimed to investigate the registration of phototherapy in the Danish National Patient Registry (DNPR) and the clinical characteristics of neonates treated with phototherapy. The results showed incomplete registration of phototherapy in the DNPR, and varying registration proportions among different hospitals. The highest registration proportion was observed in a university hospital, where phototherapy was inversely associated with gestational age and birth weight, and positively associated with clinical characteristics such as birth asphyxia and infections.

Automated detection of focal seizures using subcutaneously implanted electrocardiographic device: A proof-of-concept study

Summary: Phase 2 studies have shown that algorithms using heart rate variability (HRV) can detect focal seizures in patients with autonomic ictal changes. However, wearable electrocardiographic devices using electrode patches may cause skin irritation. This study reports the first use of a subcutaneously implantable cardiac monitor (ICM) for automated seizure detection, showing that it is feasible and accurate in patients with focal seizures and autonomic ictal changes.

The clinical use of electroencephalography in a general psychiatric setting

Summary: This study aimed to evaluate and optimize the clinical use and diagnostic value of EEG in psychiatric settings. The results showed that EEG was rarely used and seldom had treatment consequences in the study period. However, in specific clinical settings, the EEG result could lead to a change in clinical management, highlighting the need for refinement of clinical guidelines to optimize the use of EEG.

Personalized seizure detection using logistic regression machine learning based on wearable ECG-monitoring device.

Burden of illness among intermittent catheter users with non-neurogenic urinary retention in Denmark

Summary: This study examines the burden of illness among patients with urinary retention (UR) caused by non-neurogenic conditions. The results show that patients requiring intermittent catheterization (IC) have significantly higher healthcare utilization and costs compared to matched controls, primarily driven by hospitalizations. Urinary tract infections (UTIs) are the most frequent bladder complications often requiring hospitalization.

Identification of drug resistance in a validated cohort of incident epilepsy patients in the Danish National Patient Register

Summary: The study aims to validate the epilepsy diagnosis in the Danish National Patient Registry (DNPR) and identify individuals with drug-resistant epilepsy (DRE) according to the ILAE criteria. Medical records of individuals with a first diagnosis of epilepsy or seizures from 2010 to 2019 were reviewed. The study found a 79% positive predictive value (PPV) for the epilepsy diagnosis and identified factors associated with drug resistance.

Using design thinking to strengthen the community pharmacist's role in epilepsy care

Insomnia in epilepsy is associated with nocturnal seizures and anxiety
