Developing a system to estimate the severity of influenza infection in England: findings from a hospital-based surveillance system between 2010/2011 and 2014/2015

The UK Severe Influenza Surveillance System (USISS) was established following the 2009 influenza pandemic to monitor severe seasonal influenza. This article describes the severity of influenza observed in five post-2009 pandemic seasons in England. Two key measures were used to assess severity: impact measured through the cumulative incidence of laboratory-confirmed hospitalised influenza and case severity through the proportion of confirmed hospitalised cases admitted into intensive care units (ICU)/high dependency units (HDU). The impact of influenza varied by subtype and age group across the five seasons with the highest crude cumulative hospitalisation incidence for influenza A/H1N1pdm09 cases in 2010/2011 and in 0-4 year olds each season for all-subtypes. Case severity also varied by subtype and season with a higher hospitalisation: ICU ratio for A/H1N1pdm09 and older age groups (older than 45 years). The USISS system provides a tool for measuring severity of influenza each year. Such seasonal surveillance can provide robust baseline estimates to allow for rapid assessment of the severity of seasonal and emerging influenza viruses.

Screening at ports of entry for Ebola Virus Disease in England-a descriptive analysis of screening assessment data, 2014-2015

Summary: Between 14 October 2014 and 13 October 2015, a total of 12,648 passengers from affected countries were screened at five major ports of entry in England. The majority of passengers were assessed as having no direct contact with EVD cases or high-risk events (12,069, 95.4%), although 535 (4.2%) passengers were assessed as requiring public health follow-up. Out of 39 passengers referred to secondary care, none were diagnosed with EVD, but one high-risk passenger later diagnosed with EVD.

Summary: This review assessed the efficacy, effectiveness, and safety of high-dose inactivated influenza vaccines (HD-IIV) for preventing laboratory-confirmed influenza in individuals aged 18 years or older. The results showed that HD-IIV had higher relative vaccine efficacy compared to standard-dose influenza vaccines (SD-IIV3) for older adults. HD-IIV also demonstrated significant effects in preventing influenza-related hospitalizations and office visits. However, HD-IIV was associated with more adverse events compared to SD-IIV.

Summary: This study investigated the efficacy, effectiveness, and safety of cell-based trivalent and quadrivalent influenza vaccines. The results showed that cell-based trivalent influenza vaccines have overall efficacy of 70% in adults and demonstrate efficacy against different influenza subtypes. However, the effectiveness of cell-based vaccines compared to traditional vaccines is limited.

Summary: Non-pharmaceutical interventions for COVID-19 have continued to slow down the circulation of influenza in the WHO European Region in the 2021/22 season. The A(H-3) virus strain has dominated, co-circulating with A(H-1)pdmo9 and B/Victoria viruses in some countries. No detections of the B/Yamagata virus have been confirmed. Many of the circulating virus subtypes or lineages have shown antigenic differences from their respective vaccine components in the northern hemisphere. Adequate levels of influenza virus characterizations should be maintained until the end of the season and in future seasons, when surveillance is adapted to include SARS-CoV-2.

Factors associated with early uptake of COVID-19 vaccination among healthcare workers in Azerbaijan, 2021

Summary: We evaluated the uptake and factors associated with COVID-19 vaccination among health workers in Azerbaijan. Among 1575 health workers, 73% had received at least one dose and 67% received two doses, all of which were CoronaVac. Factors associated with vaccination uptake included no previous COVID-19 infection, older age, belief in the vaccine's safety, previous vaccination for influenza, having patient-facing roles, and good or excellent self-assessed health.

Summary: Between January and June 2022, a total of 427 pediatric cases of severe acute hepatitis of unknown cause were reported in 20 countries in the European Region. Demographic, epidemiological, clinical, and microbiological data were analyzed, revealing that 77.3% of the cases were in children aged 5 or younger, 53.5% tested positive for adenovirus, 10.4% tested positive for SARS-CoV-2, and 10.3% were coinfected with both pathogens. Adenovirus-positive cases had a higher likelihood of being admitted to intensive care or high-dependency units and undergoing transplantation.

Summary: A study conducted in six large hospitals in Georgia found that factors associated with COVID-19 vaccination among healthcare workers (HWs) included age, perception of vaccine effectiveness, previous seasonal influenza vaccination, and previous SARS-CoV-2 infection. Nurses, administrative staff, and ancillary staff were less likely to have received the vaccine compared to physicians. Tailoring the vaccine communication campaign to younger and non-physician HWs and emphasizing the benefits of the vaccine could help increase vaccine coverage among HWs in Georgia.

Summary: Optimal age-based vaccine prioritisation strategies vary based on country characteristics, decision-making metrics, and roll-out speeds. Prioritising older adults has more benefits when the roll-out is slow and vaccine profiles are less favourable. Broad age groups targeting (V60) might lead to similar or even better health and economic outcomes compared to a strategy involving more age-based stages (V75).
