Maternal obesity and pregnancy: Evolution of prevalence and of place of birth

Objective. The primary objective was to describe the evolution of the prevalence of maternal obesity in France. The secondary objectives were to assess changes in the French regions and the place of birth according to type (public/private) and level of care. Material and methods. Study based population of all births between 1999 and 2009 included in the AUDIPOG database. Our judgment criterion was the body mass index (BMI) divided into classes. Results. Seventy-four thousand and forty-six pregnancies were included. The mean BMI was 22.9 +/- 4.5 kg/m(2) with an increase between 1999-2001 and 2008-2009 (22.6 +/- 4.3 kg/m(2) versus 23.1 +/- 4.7 kg/m(2), P < 10(-4)). North was the first region in terms of overweight patients (18%) and obese (10.2%). There was an increase of the birth rate in public maternity, and level II and III regardless of BMI class. Only the class of patients with a BMI >= 40 kg/m(2) delivered more frequently in level III with an increase between the first and the last period of the study (20.8% versus 36.8%, P=0.02). Conclusion. The BMI is still increasing with more and more patients with a BMI >40 kg/m(2) referred to level III. This raises the question of the organization of care and follow-up of these patients. The study results did not show a statistically significant difference in adverse outcomes between the study group and control group, indicating the need for further research due to low statistical power.

Risk factors associated with shortened latency before delivery in outpatients managed for preterm prelabor rupture of membranes

The main finding was that oligohydramnios was the main risk factor linked to a reduced latency period in PPROM patients receiving home care.

Risk factors for severe neonatal thrombocytopenia in cases of maternal immune thrombocytopenia

Summary: Maternal immune thrombocytopenia (ITP) can cause neonatal thrombocytopenia (nTP) and increase the risk of neonatal hemorrhagic complications. Risk factors such as maternal splenectomy and previous severe nTP have been confirmed, while others like maternal platelet count are still inconclusive. The study identified additional risk factors for severe nTP, such as maternal treatment during pregnancy and low maternal platelet count at delivery.

Summary: This study found that diabetes mellitus and overweight or obesity are risk factors for COVID-19 in pregnancy, and gestational diabetes mellitus is associated with the disease, especially among women requiring insulin. Therefore, women with these comorbidities should be vaccinated.

Is it reasonable to propose vaginal delivery with twin pregnancies, when the first twin is in breech presentation?

Summary: This study evaluated the impact of delivery mode on neonatal and maternal outcomes in twin pregnancies with the first twin in breech presentation. The results showed that there was no significant difference in morbidity between vaginal delivery and cesarean delivery for both twins, but the rate of postpartum hemorrhage was significantly lower in the vaginal delivery group.

Prognostic factors for successful induction of labor in intrauterine growth restriction after 36 weeks of gestation

Summary: This study aimed to determine the fetal and maternal predictive factors for induction of intrauterine growth restriction (IUGR) fetuses. The study found that successful IUGR induction is mainly influenced by maternal factors. Maternal factors such as low BMI, multiparity, nonscarring uterus, absence of preeclampsia, and normal fetal heart rate before induction were found to be good prognostic factors for successful IUGR induction.

Development of placental lesions after recovery from COVID-19 during pregnancy: case-control study

Summary: This study aims to explore whether the occurrence and type of placental lesions vary according to the time of onset of COVID-19 in pregnant women. The results showed that the COVID-19 group had a higher rate of vascular complications, fetal and maternal vascular malperfusion, and signs of inflammation compared to the control group. Moreover, the occurrence of chronic villitis differed between the COVID-19 group that delivered within 14 days after infection and the group that delivered more than 14 days after infection.

CNGOF clinical practice guidelines: Evaluation one year after revision of the methodology

Summary: This study aimed to evaluate the revision of the methodology of the clinical practice guidelines (CPG) of the French National College of Gynecologists and Obstetricians (CNGOF). Three specific topics were studied in 2020. The results showed that all CPG groups adhered to the new methodology, but there were heterogeneities in the presentation of arguments, formulation of recommendations, and assessment of evidence. It is suggested to standardize the presentation and provide a guide for critical analysis of literature.
