The Gut Microbiota and Immune System Relationship in Human Graft-versus-Host Disease

Gut microbiota has gained increasing interest in the pathogenesis of immune-related diseases. In this context, graft-versus-host disease is a condition characterized by an immune response which frequently complicates and limits the outcomes of hematopoietic stem cell transplantations. Past studies, carried mostly in animals, already supported a relationship between gut microbiota and graft-versus-host disease. However, the possible mechanisms underlying this connection remain elusory. Moreover, strategies to prevent graft-versus-host disease are of great interest as well as the potential role of gut microbiota modulation. We reviewed the role of gut microbiota in the development of immune system and its involvement in the graft-versus-host disease, focusing on data available on humans.

They also have the potential to be used as biomarkers and therapeutic targets.

Branched chain amino acids in hepatic encephalopathy and sarcopenia in liver cirrhosis: Evidence and uncertainties

Summary: Liver cirrhosis is often accompanied by nutritional alterations, affecting about 20% of patients with compensated disease and over 60% of patients with decompensated cirrhosis. Imbalance of amino acids, especially branched-chain amino acids (BCAAs), has been shown to be associated with complications and prognosis in these patients. Various studies have explored the use of BCAAs supplementation as a therapeutic option, but uncertainties persist regarding its real efficacy, optimal administration route, and dosage.

Reduced Prognostic Role of Serum PCT Measurement in Very Frail Older Adults Admitted to the Emergency Department

Summary: This study evaluates the prognostic role of serum PCT in older patients with suspect sepsis or infective diagnosis in the Emergency Department, with a focus on frailty status. The results show that PCT values at ED admission do not predict infective diagnosis or higher odds of in-hospital death. However, in frail older adults, PCT values in ED can be a useful predictor of bloodstream infection.

Molecular and Clinical Features of Hepatocellular Carcinoma in Patients with HBV-HDV Infection

Summary: Hepatitis D virus (HDV) infection affects over 10 million people worldwide, with a prevalence of around 4.5% among HBsAg-positive individuals. Chronic HDV infection has been associated with a significant increase in hepatocellular carcinoma (HCC) compared to chronic hepatitis B virus (HBV) monoinfection. However, the molecular oncogenic mechanisms of HDV and its role in promoting HCC development are still not well understood. This review focuses on the direct oncogenic action of HDV, its impact on the tumor microenvironment, and the genetic signature of HDV-related HCC, comparing it with HBV-related HCC.

Serological and Molecular Characterization of Hepatitis C Virus-Related Cryoglobulinemic Vasculitis in Patients without Cryoprecipitate

Summary: Prolonged B cells stimulation by HCV can lead to autoimmunity characterized by elevated levels of cryoglobulins, rheumatoid factor, and free light chains of immunoglobulins. This study aimed to identify an immunological signature for early-stage vasculitis when cryoprecipitate is undetectable. The results showed that patients with supposed circulating cryoglobulins exhibited serological parameters similar to type II and III cryoglobulinemia. Analysis of clonal IgH and TCR rearrangements proved to be the best option for early diagnosis of extrahepatic complications.

Fecal microbiota transplantation for recurrent Clostridioides difficile infection in frail and very old patients

Summary: Fecal microbiota transplantation (FMT) is effective and safe for very old patients with recurrent Clostridioides difficile infection (rCDI). Frailty and high comorbidity do not limit the use of FMT in these patients. Frailty assessment has the potential to better identify patients at higher risk for worse outcomes or repeat treatment with FMT.

Summary: The segmentation of patients into homogeneous groups can improve the effectiveness of eradication therapy. The study aims to determine the most important treatment strategies used in Europe and evaluate the effectiveness of first-line treatments based on year and country. The analysis found that the overall treatment effectiveness increased from 87% in 2013 to 93% in 2022, with the highest effectiveness achieved in 2022 using bismuth-quadruple therapy in Spain. Cluster analysis identified homogeneous treatment groups and assessed the effectiveness of different first-line treatments.
