TOTALL: high cost of allergic rhinitis-a national Swedish population-based questionnaire study

Allergic rhinitis is a global illness with a well-recognised impact on quality of life and work performance. Comparatively little is known about the extent of its economic impact on society. The TOTALL study estimates the total cost of allergic rhinitis using a sample representing the entire Swedish population of working age. A questionnaire focused on allergic rhinitis was mailed out to a random population of Swedish residents, aged 18-65 years. Health-care contacts, medications, absenteeism (absence from work) and presenteeism (reduced working capacity at work) were assessed, and the direct and indirect costs of allergic rhinitis were calculated. Medication use was evaluated in relation to the ARIA guidelines. In all, 3,501 of 8,001 (44%) answered the questionnaire, and 855 (24%) of these reported allergic rhinitis. The mean annual direct and indirect costs because of allergic rhinitis were (sic)210.3 and (sic)750.8, respectively, resulting in a total cost of (sic)961.1 per individual/year. Presenteeism represented 70% of the total cost. Antihistamines appear to be used in excess in relation to topical steroids, and the use of nasal decongestants was alarmingly high. The total cost of allergic rhinitis in Sweden, with a population of 9.5 million, was estimated at (sic)1.3 billion annually. These unexpectedly high costs could be related to the high prevalence of disease, in combination with the previously often underestimated indirect costs. Improved adherence to guidelines might ease the economic burden on society.

Caregivers' absence from work before and after tonsil surgery in children with sleep-disordered breathing

Summary: This study investigated the expression of PD-1, CD69, and HLA-DR on T cells in OSCC patients' PBMCs, TDLNs, and tumor samples. The results showed highly activated T lymphocytes expressing high levels of PD-1 in OSCC tissue, with lower levels in TDLNs and peripheral blood. Patients with a high percentage of CD3(+) PD-1(+) cells in TDLNs had significantly lower disease-free and overall survival rates. This suggests that analyzing PD-1 levels in TDLNs may help predict survival and response to anti-PD-1 therapy in OSCC.

Surgery to relieve nasal obstruction: outcome for 366 patients operated on by one senior surgeon

Summary: In a study involving 366 patients operated on by one experienced surgeon, septoplasty and septoplasty + turbinoplasty were shown to be more effective at relieving nasal obstruction than turbinoplasty alone. Patients who underwent septoplasty + turbinoplasty experienced a greater improvement in general health compared to those who underwent septoplasty alone, despite similar improvements in nasal obstruction. This suggests a beneficial effect of additional turbinoplasty in septoplasty procedures.

BMI as a risk factor for the development of chronic rhinosinusitis: a prospective population-based study

Summary: This study found an association between obesity and new-onset chronic rhinosinusitis. The risk of new-onset chronic rhinosinusitis was 53% higher in the obese group compared to the normal weight group. Therefore, it is important to consider obesity when assessing patients with chronic rhinosinusitis in a clinical setting.

Chronic rhinosinusitis associated with chronic bronchitis in a five-year follow-up: the Telemark study

Prolonged inflammatory resolution in allergic asthma relates to dysfunctional interactions between neutrophils and airway epithelium

Summary: This study investigated the interaction between neutrophils and airway epithelium during inflammatory resolution in patients with allergic asthma. The results showed that healthy epithelial cells regenerated faster than asthmatic epithelial cells, and autologous neutrophils improved the regeneration of healthy epithelial cells but not asthmatic epithelial cells. Furthermore, the expression of IL-8 and β-catenin was down-regulated in healthy epithelial cells after resolution, but not in asthmatic epithelial cells.

It further emphasizes the significance of Notch signaling in allergic rhinitis, and suggests that blocking JAG-1 and DLL-1-induced Notch signaling by nasal epithelial cells and neutrophils may reduce allergic airway inflammation.
