Growth monitoring as an early detection tool: a systematic review

Growth monitoring of apparently healthy children aims at early detection of serious underlying disorders. However, existing growth-monitoring practices are mainly based on suboptimal methods, which can result in delayed diagnosis of severe diseases and inappropriate referrals. We did a systematic review to address two key and interconnected questions underlying growth monitoring: which conditions should be targeted, and how should abnormal growth be defined? We systematically searched for studies reporting algorithms for growth monitoring in children and studies comparing the performance of new WHO growth charts with that of other growth charts. Among 1556 identified citations, 69 met the inclusion criteria. Six target conditions have mainly been studied: Turner syndrome, coeliac disease, cystic fibrosis, growth hormone deficiency, renal tubular acidosis, and small for gestational age with no catch-up after 2 or 3 years. Seven algorithms to define abnormal growth have been proposed in the past 20 years, but their level of validation is low, and their overall sensitivities and specificities vary substantially; however, the Grote and Saari clinical decision rules seem the most promising. Two studies reported that WHO growth charts had poorer performance compared with other existing growth charts for early detection of target conditions. Available data suggest a large gap between the widespread implementation of growth monitoring and its level of evidence or the clinical implications of early detection of serious disorders in children. Further investigations are needed to standardise the practice of growth monitoring, with a consensus on a few priority target conditions and with internationally validated clinical decision rules to define abnormal growth, including the selection of appropriate growth charts.

Summary: This study aimed to examine the associations of assisted reproductive technology (ART) conception with offspring cardiometabolic health outcomes and whether these associations differ with age. The researchers found small and statistically non-significant differences in offspring cardiometabolic outcomes between ART-conceived and naturally conceived offspring. The study concluded that these findings should reassure individuals receiving ART, but longer-term follow-up is needed to investigate changes in the risks of hypertension, dyslipidaemia, and cardiovascular disease over adulthood.

Parental feeding practices as potential moderating or mediating factors in the associations between children's early and later growth

Summary: This study examines the impact of early parental feeding practices on children's growth. It shows that parental restriction for weight at 2 years is positively associated with child BMI z-scores from 4 to 8 years. Additionally, high use of food as a reward is positively associated with later BMI z-scores among boys. Parental feeding practices mediate the relationship between early and later growth.

Sudden Unexpected Death in Infancy: Current Practices in Virological Investigations and Documentation in the French Registry

Summary: This study investigated virological investigations in sudden unexpected death in infancy (SUDI) and found that 3.6% of infants died from established viral infections. Among the 639 infants examined, 43.3% carried a virus with no significant difference between SIDS infants and the control group. The most commonly detected virus was rhinovirus, but its presence did not associate with SIDS.

Summary: This narrative systematic review examined the effectiveness of interventions during pregnancy and early childhood in improving energy balance-related behaviors and preventing obesity in children from disadvantaged families. The review found that interventions targeting breastfeeding initiation and multi-behavioral interventions were most effective in reducing obesity risk. Interventions involving first-time mothers, professional delivery agents, multidisciplinary teams, and peer groups were also more likely to be effective. The review highlighted the importance of co-creation with stakeholders and adherence to theoretical frameworks for developing inclusive and effective programs.

The Committee on Nutrition of the French Society of Pediatrics believes that more clinical studies, particularly randomized and placebo-controlled trials, are needed before making a recommendation.

Associations of screen use with cognitive development in early childhood: the ELFE birth cohort

Availability, accessibility, and use of green spaces and cognitive development in primary school children

Summary: Green space availability, accessibility, and use showed a social gradient that favored higher socioeconomic groups. However, there were no statistically significant associations between green space exposures and cognitive function outcomes in the overall study population. Living within 300 m of a major green space was associated with improved working memory in children from less deprived areas, while more time spent playing in green spaces was associated with better working memory in children with highly educated mothers. However, studying within 300 m of a major green space increased inattention scores in children from more deprived areas.
