Summary: This study characterized a Klebsiella pneumoniae isolate with high-level resistance to ceftazidime-avibactam due to the presence of KPC-53, a variant of KPC-3 with unique genetic features. Whole-genome sequencing revealed the presence of two copies of blaKPC-53 on different plasmids, shedding light on the mechanisms of resistance to ceftazidime-avibactam.

Description of novel resistance islands harbouring blaCTX-M-2 in IncC type 2 plasmids

Summary: This study sequenced two IncA/C plasmids carrying bla(CTX-M-2) in Klebsiella pneumoniae clinical isolates and identified complex transposons mediating resistance to multiple antibiotics. The plasmids confer resistance to oxyimino-cephalosporins, gentamicin, and chloramphenicol. The findings contribute to a better understanding of the resistance islands in these plasmids.

Activity of fosfomycin/colistin combinations against planktonic and biofilm Gram-negative pathogens

Resistance to Ceftazidime/Avibactam, Meropenem/Vaborbactam and Imipenem/Relebactam in Gram-Negative MDR Bacilli: Molecular Mechanisms and Susceptibility Testing

Summary: Multidrug resistance (MDR) is a serious global threat due to the rapid spread of MDR pathogens and limited treatment options. New beta-lactams/beta-lactamase inhibitor combinations (beta L-beta LICs) have shown promise for treating MDR Gram-negative infections. However, resistance to these novel molecules has started to emerge. This review summarizes the mechanisms of resistance to ceftazidime/avibactam, meropenem/vaborbactam, and imipenem/relebactam in MDR Gram-negative microorganisms, with a focus on antimicrobial activities and resistance traits related to novel beta L-PLICs. The detection methods for antimicrobial susceptibility testing of these molecules are also discussed.

Relevant increase of CTX-M-producing Escherichia coli carriage in school-aged children from rural areas of the Bolivian Chaco in a three-year period

Summary: This study examined the fecal carriage of CTX-M-producing Enterobacterales in school-aged children from rural areas of the Bolivian Chaco. The results showed a significant increase in the fecal carriage of CTX-M-producing Enterobacterales from 2016 to 2019. The study highlights the importance of understanding the dissemination dynamics of this resistance determinant.

Effectiveness of a Multifaced Antibiotic Stewardship Program: A Pre-Post Study in Seven Italian ICUs

Summary: This study demonstrates the successful implementation of an antibiotic stewardship program in seven Italian intensive care units. The program effectively improved infection management by reducing the duration of empirical therapy, lowering the use of quinolones, and decreasing the proportion of multi-drug-resistant bacteria in ICU-acquired infections. ICU mortality and length of stay remained unchanged, indicating that patient safety was not compromised by reducing antibiotic administration.

Activity of N-Acetylcysteine Alone and in Combination with Colistin against Pseudomonas aeruginosa Biofilms and Transcriptomic Response to N-Acetylcysteine Exposure

Summary: Chronic colonization by Pseudomonas aeruginosa is critical in cystic fibrosis (CF) and other chronic lung diseases. Evaluation of combination therapies for treating biofilm-associated P. aeruginosa lung infections has been of growing interest. N-acetylcysteine (NAC) alone has limited antibiofilm activity, but shows synergistic effects when combined with colistin. NAC exposure induces a Zn2+ starvation response, downregulates denitrification genes, and reduces flagellum-mediated motility in P. aeruginosa. NAC-colistin combinations may be effective in managing biofilm-associated P. aeruginosa lung infections.

In vitro activity of aztreonam/avibactam against isolates of Enterobacterales collected globally from ATLAS in 2019

Summary: This study assessed the in vitro activity of the drug combination aztreonam/avibactam (ATM-AVI) against drug-resistant Enterobacterales, including strains producing metallo-beta-lactamases (MBLs). The results demonstrated that ATM-AVI exhibited potent antimicrobial activity against Enterobacterales, including MBL-producing strains. ATM-AVI represents a potential option for treating infections caused by antibiotic-resistant Enterobacterales, including MBL-producing strains.
